La principale conséquence d'un accident exposant au sang (AES) est le risque de contamination par des agents pathogènes transmis par le sang tels que les virus de l'hépatite B, l'hépatite C, le VIH et le virus HTLV-I. L'identification, depuis l'avènement du SIDA, des AES comme un risque mesurable et évitable concernant les professionnels de soins a conduit les autorités sanitaires et les établissements de santé à mettre en oeuvre une stratégie de prévention. L'étude réalisée au CHU de Pointe-à-Pitre a pour but d'analyser la prise en charge et la surveillance des AES entre 2000 et 2002 et d'évaluer les différentes actions de prévention entreprises. Le service de médecine du travail a été chargé de la surveillance des AES, sous l'égide du C.CLIN Sud Ouest, sous forme de recueil épidémiologique standardisé de données permettant de documenter chaque AES : circonstances de l'accident, informations concernant le patient source, réaction à l'accident et prévention. Les enquêtes réalisées ont permis d'identifier plusieurs facteurs de risque d'AES, guidant ainsi la politique de prévention. Les accidents surviennent le plus souvent chez les infirmières mais la proportion de jeunes médecins concernés a tendance à augmenter. L'exposition la plus fréquente est la piqûre, elle survient le plus souvent lors des prélèvements, des injections, des perfusions et des sutures. Les constats effectués soulignent la nécessité de poursuivre les actions de préventions afin de réduire le risque professionnel d'exposition au sang car même si la prise en charge après un accident est bien gérée et qu'aucun cas de séroconversion n'a encore été observé au CHU de Pointe-à-Pitre, les actions de prévention entreprises n'ont pas encore toutes fait preuve de leur efficacité.

Between 2005 and 2013, the case fatality rate for United States service members injured in Afghanistan decreased by nearly 50 percent, despite an increase in the severity of injury among U.S. troops during the same period of time. But as the war in Afghanistan ends, knowledge and advances in trauma care developed by the Department of Defense (DoD) over the past decade from experiences in Afghanistan and Iraq may be lost. This would have implications for the quality of trauma care both within the DoD and in the civilian setting, where adoption of military advances in trauma care has become increasingly common and necessary to improve the response to multiple civilian casualty events. Intentional steps to codify and harvest the lessons learned within the military's trauma system are needed to ensure a ready military medical force for future combat and to prevent death from survivable injuries in both military and civilian systems. This will require partnership across military and civilian sectors and a sustained commitment from trauma system leaders at all levels to assure that the necessary knowledge and tools are not lost. A National Trauma Care System defines the components of a learning health system necessary to enable continued improvement in trauma care in both the civilian and the military sectors. This report provides recommendations to ensure that lessons learned over the past decade from the military's experiences in Afghanistan and Iraq are sustained and built upon for future combat operations and translated into the U.S. civilian system.

This process acquired new momentum with the passage of the North American Free Trade Agreement in 1994, which lowered barriers to the movement of goods, capital, services, and information. But rather than include labor in this new regime, the United States continues to resist the integration of the labor markets of the two countries. Instead of easing restrictions on Mexican labor, the United States has militarized its border and adopted restrictive new policies of immigrant disenfranchisement. Beyond Smoke and Mirrors examines the devastating impact of these immigration policies on the social and economic fabric of the Mexico and the United States, and calls for a sweeping reform of the current system. Beyond Smoke and Mirrors shows how U.S. immigration policies enacted between 1986–1996—largely for symbolic domestic political purposes—harm the interests of Mexico, the United States, and the people who migrate between them. The costs have been high. The book documents how the massive expansion of border enforcement has wasted billions of dollars and hundreds of lives, yet has not deterred increasing numbers of undocumented immigrants from heading north. The authors also show how the new policies unleashed a host of unintended consequences: a shift away from seasonal, circular migration toward permanent settlement; the creation of a black market for Mexican labor; the transformation of Mexican immigration from a regional phenomenon into a broad social movement touching every region of the country; and even the lowering of wages for legal U.S. residents. What had been a relatively open and benign labor process before 1986 was transformed into an exploitative underground system of labor coercion, one that lowered wages and working conditions of undocumented migrants, legal immigrants, and American citizens alike. Beyond Smoke and Mirrors offers specific proposals for repairing the damage. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. This publication summarizes the presentations and discussions from the workshop.
