In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.

Book excerpt: INTRODUCTION - Le système de santé est organisé en France autour des soins de santé primaire dont les médecins généralistes sont les principaux acteurs. Le système de santé est aujourd'hui saturé. Les inégalités en termes d'accès aux soins s'accroissent et le médecin-proche devient l'ultime solution en raison de son accessibilité. Toutefois, dispenser des soins à un proche n'est pas sans risques car l'affect et l'émotionnel influencent la relation thérapeutique médecin-proche. C'est pourquoi, le médecin doit être formé à la prise en charge d'un proche. Pour autant, il convient de vérifier si les médecins perçoivent ces risques spécifiques, comment ils les prennent en compte et s'ils ressentent le besoin d'être formés afin de mieux gérer leur réponse aux demandes de soins de leurs familles. MÉTHODE - Une étude qualitative par entretien individuel a été menée auprès d'un panel de médecins généralistes de l'ancienne Haute-Normandie, avec retranscription et anonymisation des enregistrements. L'analyse des données a été réalisée selon la méthode de théorisation ancrée avec un codage manuel axial et transversal. RÉSULTATS - Sur les dix-neuf médecins généralistes interrogés, 89% admettent ne jamais avoir abordé le sujet des soins aux proches durant leurs parcours professionnel, 58% ne se sentent pas en confiance dans la prise en charge de leurs familles, aucun n'a connaissance de supports spécifiques aux soins des proches et 74% n'ont pas d'information sur la réglementation française à ce sujet. Pourtant, si l'on constate que 53% des participants ne ressentent pas le besoin d'être formés, 74% souhaiteraient se former sur la réponse aux demandes de soins des membres de leurs familles. DISCUSSION - Les données recueillies dans cette étude soulignent la volonté d'une majorité des médecins généralistes de recevoir une formation pour mieux prendre en compte les risques liés à la complexité des situations de demandes de soins de leurs familles, alors même que spontanément ils n'en ressentent pas le besoin. Cela démontre leur préférence pour une approche proactive plutôt que réactive de la gestion des risques liés aux soins dispensés aux membres de leurs familles. CONCLUSION - Cette étude a pu démontrer que les médecins généralistes ne ressentaient pas le besoin actuel d'être formés sur la réponse aux demandes de soins de leurs familles, mais étaient intéressés par être formés afin d'être mieux préparés aux défis qu'apportent les soins aux proches.
