Available in PDF, EPUB and Kindle. Book excerpt: Introduction : Le dispositif intra-utérin (DIU), moyen de contraception le plus utilisé dans le monde, reste sous employé en France. Encore perçu comme une méthode d'arrêt des naissances, de nombreuses idées reçues persistent à son propos. Objectif : Explorer les connaissances et les représentations des femmes sur le DIU au sein du choix contraceptif existant. Méthode : Étude qualitative par entretiens semi-dirigés, retranscrits et analysés par thèmes, de décembre 2013 à avril 2014, auprès quatorze femmes utilisatrices ou non du DIU. Résultats : Les participantes ont perçu le DIU comme une contraception de longue durée d'action, moins contraignante que la contraception orale, indemne d'hormones pour le dispositif au cuivre, pouvant offrir une certaine liberté. Toutefois, certaines craintes empêchaient son utilisation : un doute sur son efficacité : risque de grossesse intra ou extra-utérine sous DIU, un aspect invasif : port d'un corps étranger intra-corporel, douleur ou gêne à l'insertion. Les connaissances sur le mode d'action et le lieu d'insertion du DIU, émanant d'informations délivrées principalement par l'entourage semblaient fréquemment approximatives, inquiétantes voire erronées. Pour la majorité, la nulliparité ne paraissait pas une contre-indication à son utilisation, mais le risque de stérilité après une possible infection pondérait cette vision. Aucune participante ne connaissait le DIU en tant que contraceptif d'urgence. Le partenaire intervenait peu dans le choix contraceptif. Conclusion : Le DIU devrait faire l'objet de plus d'information en France pour lutter contre les fausses croyances et permettre aux femmes d'en accepter les inconvénients afin de bénéficier de ses avantages, déjà perçus dans le reste du monde.

Book excerpt: Malgré un contexte français favorisant l'utilisation plus large des LARCs, le dispositif intra-utérin est peu utilisé par les femmes nullipares en 2016. Pour étudier le regard des femmes nullipares sur ce dispositif, nous avons réalisé une étude qualitative par focus groupe et entretiens semi-dirigés dans un cabinet de médecine générale au printemps 2016. Les femmes volontaires nullipares majeures ne portant pas de DIU étaient inclues. Le recrutement a été stoppé à saturation des données, soit après un focus-groupe de 4 participantes et 14 entretiens. Une analyse thématique du verbatim a été réalisée. Les femmes nullipares méconnaissaient le DIU et son indication en particulier quant à la nulliparité. En revanche, elles connaissaient ses avantages. Les craintes principales étaient celles du corps étranger, de la pose, des douleurs, d'un déplacement potentiel ou d'une gêne lors des rapports sexuels. L'entourage rapportait des informations aux nullipares sur le DIU : la non-indication chez les nullipares, ses effets indésirables connus dont les douleurs et à l'inverse, la satisfaction d'utiliser ce dispositif. La mère prenait une place importante dans le choix contraceptif, elle est un interlocuteur à privilégier en matière d'information. Les femmes nullipares sont en demande d'informations médicales sur le DIU, elles souhaitent être actrices de leur contraception et qu'on ne leur impose plus de choix. Après information, elles sont prêtes à inclure le DIU dans leur panel contraceptif mais passer le cap de la pose reste difficile. Le DIU va très probablement devenir une méthode contraceptive prisée par les nullipares dans les années à venir.

Il persiste un « effet parité » dans toutes les tranches d'âge. L'objectif principal de l'étude est de comparer la tolérance du DIU défini par le taux de continuation à 18 mois entre des patientes nullipares et des patientes multipares, et secondairement d'évaluer les taux d'expulsion, d'échec de contraception (grossesse sous DIU) et de discontinuation liée aux effets secondaires selon la parité, l'âge et le type de DIU. Matériels et méthodes : l'étude réalisée est une étude rétrospective de cohorte menée sur deux centres : au cabinet privé de gynécologie Gynétopia situé dans le centre-ville de Marseille (France) et en service de consultation de gynécologie à l'hôpital public de la Conception situé dans le centre-ville de Marseille (France). Nous avons inclus toutes les femmes ayant bénéficié de la pose d'un DIU pour un motif contraceptif du 1er Janvier 2017 au 1er Janvier 2020 dans l'un des deux centres. Nous avons inclus toutes les données de suivi disponibles jusqu'en Mai 2022. Les patientes ont ainsi été suivies jusqu'à la date de discontinuation du DIU ou pendant au minimum 29 mois après l'insertion. Résultats : au total, 595 poses de DIU ont pu être suivies chez 546 patientes. La parité n'est pas retrouvée comme influençant l'échec global de la contraception par DIU OR=0.8, IC [0.5-1.1], p =0.2. En revanche, on retrouve significativement plus d'échecs de la contraception par DIU chez les plus jeunes et cet effet de l'âge est linéaire avec pour les patientes âgées de 25 à 35 ans un OR = 2.5 [1.7-3.5], p

Book excerpt: The mechanism of action, safety, and efficacy of IUDs were reviewed by a WHO Scientific Group in 1986. The Scientific Group concluded that the IUD should continue to be supported, in both developed and developing countries, as a safe, reliable method of fertility regulation. The newer copper-releasing devices are comparable to oral contraceptives in terms of safety and efficacy. When compared to women who use other reversible methods of contraception, IUD users have the lowest mortality resulting from deaths directly attributable to those methods or to the consequences of unwanted pregnancy. In the past decade, research has concentrated on the development of new devices that have both higher continuation rates and lower rates of expulsion and removal for bleeding abnormalities. An important recent concern has been the possible increased risk of pelvic inflammatory disease (PID) and subsequent tubal infertility associated with IUD use. However, it now appears that methodological problems have caused the IUD-associated risk of PID to be overestimated. The increased risk with IUDs seems to be limited to the 1st 4 months of use. No increased risk of tubal infertility has been found among IUD users in stable, monogamous sexual relationships. The use of a copper IUD after the 1st pregnancy is not associated with secondary infertility due to tubal disease. Finally, the newer copper IUDs have low rates of ectopic pregnancy.

In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
