Au niveau osseux, soutien des tissus mous, l'application des techniques de régénération osseuse guidée permet de limiter la résorption physiologique post-extractionnelle ou de la corriger. Concernant les tissus mous, des chirurgies peuvent être utilisées à différents moments de la prise en charge thérapeutique, afin d'augmenter ou de maintenir la quantité et la position de ces tissus. Applicables avant, pendant ou après la chirurgie, ces techniques ont pour but d'obtenir un biotype parodontal épais, plus favorable au maintien des tissus péri-implantaires. La prothèse, par l'intermédiaire de son pilier, possède également un rôle important dans cette optique. De nouveaux protocoles sont proposés et offrent des résultats encourageants pour le futur. Les techniques chirurgicales de gestion des tissus mous étant très praticien-dépendantes, il est difficile de les comparer bien qu'il se dégage un consensus clinique sur l'utilisation systématique de greffons conjonctifs, bénéfique à l'esthétique muco-gingivale du secteur antérieur.

En effet, la présence d'une bande de muqueuse kératinisée serait à même de favoriser la stabilité des tissus péri-implantaires. Il s'agit d'un sujet très controversé actuellement dans la littérature scientifique. L'objectif de ce travail est donc de faire un point, à travers une analyse bibliographique, sur l'intérêt d'avoir ou d'assurer un minimum de fibromuqueuse autour d'un implant - comme il en a déjà été discuté autour des dents naturelles - et éventuellement de suggérer des recommandations cliniques pour le chirurgien-dentiste. Nous porterons une attention particulière à l'examen de la muqueuse orale pour reconnaître un parodonte « à risque » et éventuellement prévoir un apport de tissus mous supplémentaire. Les techniques de chirurgie muco gingivale constituent ainsi un véritable panel d'outils au service de l'implantologie.
