Book excerpt: Introduction : les programmes de réhabilitation multidisciplinaires (MPR) pour les patients souffrant de lombalgie chronique associent des interventions physiques, comportementales, éducatives et socioprofessionnelles. Le devenir des patients reste pourtant variable et difficile à prédire Se pose donc la question de l'impact des facteurs professionnels. Cette étude a pour objectif principal de déterminer si l'évolution clinique après un programme de rééducation est corrélée la satisfaction au travail. Matériel et méthode : cette étude rétrospective a inclus 77 patients qui ont participé à un programme de réhabilitation en 2016 ou 2017. Les scores cliniques (seuil douloureux, FABQ, DALLAS / OSWESTRY et QUEBEC) ont été recueillis avant et un an après le programme alors que la satisfaction au travail (MSQ), le stress (EVA stress), la satisfaction concernant le médecin du travail ou l'impact du travail sur les douleurs, seulement un an après. Les patients ont été différenciés en deux groupes selon les données cliniques (évolution favorable versus défavorable) et les résultats concernant les critères professionnels ont été comparés. Résultats et discussion : trente-six patients (sur 77) ont évolué défavorablement (moins de 30% d'amélioration des scores cliniques). Parmi ce groupe, 23.53% se sont dit satisfaits de leur travail (versus 53.66%, p=0.01) ; le MSQ était en moyenne de 65.18 +/-12.58 (versus 72.49 +/- 10.94, p=0.01) ; 66.7% ont estimé que leur travail avait un impact sur leurs douleurs lombaires (versus 31.7%, p=0.02) ; l'impact du travail sur les douleurs lombaires était en moyenne de 7.32 +/-2.17 (versus 4.41 +/- 3.05, p=0.0001). Conclusion : la satisfaction au travail et la perception de l'impact du travail sur la lombalgie semblent expliquer une part non négligeable de l'évolution des patients lombalgiques chroniques après ce programme de réhabilitation. L'implication du médecin du travail semble essentielle.

Book excerpt: Contexte : Une proportion considérable d'absence au travail est attribuée à la lombalgie chronique. Cette dernière représente une perte de temps de production et des dépenses de santé élevées, surtout après une blessure liée au travail. L'Assurance Maladie française tente d'améliorer le retour au travail de ces patients en leur accordant une prise en charge précoce dans un centre de réadaptation, en association avec un suivi personnalisé rapproché. L'objectif de cette étude est de déterminer si une prise en charge précoce (dès trois mois après un arrêt de travail) dans un contexte d'accident de travail améliore le retour au travail au cours des trois mois suivants et de déterminer les facteurs influençant ce retour au travail. Matériel et méthodes : Cinq centres de rééducation français ont pris part au protocole, dans une étude rétrospective. 147 patients lombalgiques (manutentionnaires) ont été inclus en 2016, trois mois après un accident de travail. L'intervention était un programme de restauration fonctionnelle du rachis (RENODOS) pour 67 patients. Les autres étaient suivis uniquement sur le retour au travail. Le critère de jugement principal était le taux de retour au travail trois mois après l'intervention. Résultats : Sur les 52 patients analysés, 26 (50%) étaient de retour au travail à 3 mois: 16 à temps plein au même poste. 9 patients étaient en reconversion professionnelle. Pour les 80 autres patients, 70 (87,5%) avaient repris le travail à trois mois. La prise en charge kinésithérapique active, les échelles TAMPA, FABQ et DALLAS et l'échelle visuelle analogique (EVA) douleur après le programme ont montré un lien statistiquement significatif sur le retour au travail. Les antécédents de la lombalgie et l'activité sportive avant ou après le programme ne sont pas statistiquement significatifs. Conclusion : Nos résultats suggèrent un effet inverse du programme de restauration fonctionnelle multidisciplinaire du rachis chez les patients souffrant de lombalgie chronique en arrêt de travail après un accident de travail. Il est possible que la surmédication, ou le drame causé par ce programme d'hospitalisation, influence la situation des travailleurs. Étonnamment, la pratique sportive n'a montré aucune influence sur les résultats. Une kinésithérapie active adaptée devrait être proposée à tout patient dans cette situation pour favoriser le retour au travail.

A J0 et cinq semaines, les données cliniques évaluant les déficiences (douleurs lombaires et radiculaires sur une échelle visuelle analogique (EVA), indice de Schöber Mac Rae, distance mains-sol (DMS), test de Shirado, test de Sorensen, consommation maximale d’oxygène (VO2max)), les incapacités fonctionnelles (indice de Waddell, indice de Québec) et l’état psychologique (indice de Beck, indice HAD, FABQ) ont été recueillies. Un lien est recherché entre les données socio-démographiques et cliniques d’une part, et la reprise du travail à un mois, 3 mois et 6 mois d’autre part.Résultats. Parmi les données socio-démographiques, seul le nombre de jours d’arrêt de travail dans l’année précédant le programme de RAE est lié à la reprise du travail à 1, 3 et 6 mois. Dans l’analyse des déficiences, l’EVA des lombalgies à l’entrée dans le programme est liée à la reprise du travail à 1 et 3 mois, l’évolution du DMS est liée à la reprise du travail à 3 et 6 mois. L’indice de Schöber en fin de programme, le VO2 max en fin de programme, l’évolution de l’indice de Schöber et l’évolution de l’EVA lombalgie sont liés à la reprise du travail à 6 mois. Dans l’analyse des incapacités, l’indice de Québec en fin de programme est lié à la reprise du travail à 1 mois, le FABQ 1 en fin de programme est lié à la reprise du travail à 1 et 6 mois, le FABQ 1 en début de programme et l’indice de Waddell en fin de programme sont liés à la reprise du travail à 3 mois. Objectifs : décrire la stratégie de prise en charge des salariés lombalgiques chroniques par les médecins du travail Bourguignons, étudier les attentes de ces derniers vis-à-vis des programmes de réadaptation spécialisés et analyser leur connaissance du programme REAGIR du CHU de Dijon. Méthode : il s’agit d’une étude descriptive. Un questionnaire a été transmis par mail aux médecins du travail Bourguignons afin de recueillir les données nécessaires à l’étude. Résultats : 75 questionnaires ont été remplis. La délivrance de conseils concernant l’hygiène de vie et l’hygiène posturale au travail est réalisée en priorité face à un salarié souffrant de lombalgie chronique. Le recours à la pluridisciplinarité est faible et seulement 9 % des médecins du travail interrogés ont connaissance du programme REAGIR. Ils sont pourtant nombreux à penser que ce type de programme pourrait être une aide dans leur mission de prévention de la désinsertion professionnelle. Conclusion : les médecins du travail Bourguignons connaissent et orientent peu leurs salariés lombalgiques chroniques vers des programmes de réadaptation. Développer la communication entre professionnels de santé en charge de ces patients permettrait une meilleure coordination des soins et bénéficierait aux salariés notamment en matière de maintien dans l’emploi.
