Book excerpt: "Lesbian, gay, bisexual, and transgender (LGBT) patients experience significant health disparities that affect morbidity, mortality, and overall quality of life. Nurses who express negative biases in the delivery of care influence many of the disparities facing this vulnerable patient population. Educational interventions that are focused on increasing nurses' knowledge of LGBT healthcare needs have proven effective in improving patient compliance and enhancing patient care outcomes. This evidence-based practice change project was designed to educate nurses (n=25) on the healthcare needs of the LGBT population. The project included a pretest and posttest with an educational intervention. The project manager, in collaboration with subject matter experts, developed a questionnaire, which consisted of 15 knowledge and five attitude questions. The questionnaire was administered before and after an educational intervention to measure any changes in nurses' knowledge and attitudes toward LGBT patients. Findings indicated that knowledge scores increased from a baseline of 69.1%, which was below the 80% benchmark, to a post intervention score of 97.6%, which was above the intended benchmark. The average mean scores measuring attitudes increased significantly, t = 2.06 and p = .04. The findings of this evidence-based practice change project have significant implications on the effects of educating nurses on the healthcare needs of the LGBT population, that dramatically influence practice and patient care outcomes. Book excerpt: The general aim of the project was to describe the situation of lesbians and gay men in a nursing environment by studying the attitudes of nursing staff and students, and the experiences of gay nursing staff in their work environment and of gay patients and partners in their encounters with nursing. The study for papers I and II had a descriptive, comparative design. Nurses, assistant nurses, and nursing students completed the Attitudes Toward Homosexuality Scale (ATHS), the Causes of Homosexuality Questionnaire (CHQ), and the Affect Adjective Checklist (AAC), along with the Nursing Behaviour Questionnaire (NBQ). In general, the participating nursing staff and students expressed positive attitudes, though some subjects reported very negative attitudes toward gay people. The participants also expressed a full spectrum of emotions from delight to anger. The studies for papers III and IV had a descriptive, explorative design. Here, the data collection was performed by semi-structured interviews. The informants were gay nursing staff, gay patients and partners of patients. The gay personnel reported fear and concern about heterosexuals' reactions when these informants "come out," and were constantly assesing the risk being open about their sexual orientation among their workmates. Nearly all patients and partners described several situations where heteronormative assumptions were communicated by the nursing staff. Almost all informants offered recommendations for nursing staff to facilitate communication. According to the presented findings, probably only a small minority in Swedish nursing have negative attitudes toward lesbians and gay men. However, the informants told of heteronormative language and behaviour in nursing contexts that were percieved as insensitive, insulting and humaliating. Nursing staff need to learn how to communicate in a more natural way and to be aware of the norms they communicate through the language and behaviour they use.

An historical survey will present the forces which shaped the current attitude of the mental health profession about homosexuality. I will present a general examination of the social ideology toward homosexuality in western society. I will also review the history of those homosexuals who have banded together for companionship, reform, and protest. This will be a context against which I will examine the attitudes of a specific group of people who care for the mentally ill--nurses and aides at a state mental hospital. The questionnaire I used to examine their attitudes surveys a broad range of possible attitudes about homosexuality. The responses will demonstrate what changes have occurred in society's attitudes toward homosexuality. It will show, specifically, what the attitudes are of those who care for economically disadvantaged, severely disturbed homosexuals.

Bachelor of Science in Nursing (BSN) students are at a high risk of making assumptions based on personal stigma, stereotypes, education, low exposure, prejudice, and religious beliefs toward the lesbian, gay, bisexual and transgender (LGBT) population, thus compromising the quality of health care provided. The purpose of this descriptive, correlational and exploratory research was to report the presence of homophobia in undergraduate nursing students. Methods. Data was collected from a convenience sampling of traditional and accelerated Bachelor of Science in Nursing students enrolled at a California State University in the Spring of 2016. Quantitative data was collected from a purposive sample of 256 nursing students using a demographic profile sheet, the Index of Attitudes Towards Homosexuals and Homophobic Behavior of Student Scale. Results. The cutting score for higher level of homophobia was set at 50 by the authors of the survey instruments. The result revealed lower level of homophobia among the BSN students. The mean IAH score (n= 256, M = 24.421 SD =15.91) and the mean HBSS score (n= 256, M = 12.37, SD = 15.29) were both lower than 50 suggesting lower-level of homophobia. There was a strong positive correlation between the IAH scores and HBSS scores of BSN students toward homosexuals, r = .682. These results suggested that the students’ behavior were consistent with their attitudes. Non-accepting religion to homosexuality accounted for strongest predictor for the homophobic scores. Conclusion: The results of this research revealed that nurses have lower level of homophobia. Future qualitative research should be built on this study to further investigate why nursing students continue to have homophobic attitudes and behaviors. The nursing faculty and students should continue to participate in cultural or diversity class to achieve neutrality regardless of their religious belief to truly advocate for patient’s health.
