Book excerpt: This is the first practical guide for nurses on how to incorporate the knowledge, skills, and tools of Strength-Based Nursing Care (SBC) into everyday practice. The text, based on a model developed by the McGill University Nursing Program, signifies a paradigm shift from a deficit-based model to one that focuses on individual, family, and community strengths as a cornerstone of effective nursing care. The book develops the theoretical foundations underlying SBC, promotes the acquisition of fundamental skills needed for SBC practice, and offers specific strategies, techniques, and tools for identifying strengths and harnessing them to facilitate healing and health. The testimony of 46 nurses demonstrates how SBC can be effectively used in multiple settings across the lifespan.

Book excerpt: We also would like to thank the members of the committee of decision-making partners for their support at the beginning of the project, their participation in meetings to discuss progressive results of the project, and in the validation and improvement of the recommendations presented in this report. [...] Research questions were related to (1) the key factors associated with the intent to stay or to leave; (2) the impact of leadership style on the nurses' perception of the practice environment and the psychological experience at work (empowerment, job satisfaction, and organizational commitment); (3) the "system problems" affecting care units; and (4) the impact of an "action learning" intervention [...] The evaluation of the project showed many significant positive impacts on the nurses' perception of nurse managers' leadership and support ability, staffing and resource adequacy, and leadership and human resources management satisfaction for all units and, in some units, on the perceptions of the nursing foundations for quality of care, affective commitment, and global organizational commitment. [...] To increase the level of job satisfaction, the following five sources of dissatisfaction should be prioritized by decisions makers: (1) increase professional support and recognition; (2) provide balanced workloads; (3) ensure the appropriateness of technical equipment and material resources; (4) improve the quality of the physical work environment; and (5) solve problems related to the delivery of [...] The approach used with wave 1 focused on the following principles: exploration of assumptions, creativity in ways of working on the project and developing competencies, openness to complexity, development of dialectical thinking, and attention and presence given to themselves, to others, and to the context.

Book excerpt: Hospitals and nursing homes are responding to changes in the health care system by modifying staffing levels and the mix of nursing personnel. But do these changes endanger the quality of patient care? Do nursing staff suffer increased rates of injury, illness, or stress because of changing workplace demands? These questions are addressed in Nursing Staff in Hospitals and Nursing Homes, a thorough and authoritative look at today's health care system that also takes a long-term view of staffing needs for nursing as the nation moves into the next century. The committee draws fundamental conclusions about the evolving role of nurses in hospitals and nursing homes and presents recommendations about staffing decisions, nursing training, measurement of quality, reimbursement, and other areas. The volume also discusses work-related injuries, violence toward and abuse of nursing staffs, and stress among nursing personnelâ€"and examines whether these problems are related to staffing levels. Included is a readable overview of the underlying trends in health care that have given rise to urgent questions about nurse staffing: population changes, budget pressures, and the introduction of new technologies. Nursing Staff in Hospitals and Nursing Homes provides a straightforward examination of complex and sensitive issues surround the role and value of nursing on our health care system.

Book excerpt: The health care dilemma of increasing patient acuity coupled with decreasing reimbursement, is forcing acute care agencies to explore new ways to increase productivity. Work redesign in the hospital setting is one strategic response to this dilemma. The restructuring of health care delivery systems and the redesign of nursing roles is creating dramatic changes in the work environment for nurses. Empirical evidence supports a relationship between work environment and job satisfaction of nurses. A successfully redesigned system is one that meets its' refined goals and objectives and makes its visions a reality (Spitzer-Lehman & Flarey, 1995). Therefore, the purpose of this study was to measure perceived RN satisfaction with the work environment pre and post work redesign. This study used a non-experimental, descriptive design. Registered nurses who work on a 40-bed medical telemetry unit in a large midwestern teaching hospital were identified for this study. A 38-item questionnaire was given prior to implementation of work redesign by another researcher and repeated 18 months after implementation by this researcher. The Work Quality Index (Whitley & Putzier,1994) contained six subscales: Professional work environment, autonomy, work worth, professional relationships, role enactment, and benefits. Demographic profiles were also obtained. Measures of central tendency and t-tests were employed to answer the research questions. Mean scores for the defined variables of RN satisfaction revealed the absence of statistically significant results, but did reflect a positive perception of work environment satisfaction pre and post work redesign. Discussion of subscales is included along with percentile rankings. The findings support empowerment/autonomy as being a positive marker for satisfaction. Work relationships with peers was also identified as being highly satisfying. Professional work relationships with physicians was identified as an area for further investigation. The results of this study provided objective data for benchmarking and for addressing unit based issues, as well as areas for improvement. Replication of the study using a multi-unit design would allow greater generalizability of the findings as well as give a more accurate measure of outcomes of the organizational redesign.
