Il existe peu d'études à long terme sur ce sujet. Aussi, après plus de cinq ans d'implantation, quel peut être le niveau de langage oral de jeunes implantés? Qu'en est-il de leur(s) mode(s) de communication, de leur intégration et de leurs éventuelles difficultés? Dans la partie théorique, nous abordons les données actuelle,s concernant l'implant cochléaire et les résultats des différentes études se rapportant au langage oral chez des jeunes sourds appareillés et chez des jeunes sourds implantés. Après avoir étudié les dossiers des patients de la population, nous avons effectué des bilans évaluant la perception auditive, la compréhension et l'expression orale à l'aide de tests (ELO, ECOSSE) et de listes de mots et de phrases. Nous avons également réalisé des questionnaires à l'intention des familles et des orthophonistes pour nous renseigner sur les aspects sociaux et communicatifs. Dans la partie pratique, nous relatons les données issues des bilans et des questionnaires. Nous comparons aussi ces résultats à ceux de jeunes sourds appareillés, étude ayant fait l'objet d'un mémoire sur le devenir linguistique du jeune déficient auditif, il y a quinze ans. Globalement, nous avons constaté une évolution différente des jeunes implantés par rapport aux jeunes appareillés. Nous avons observé une évolution positive du langage oral malgré une morphosyntaxe laborieuse et sur un autre plan, des difficultés d'intégration sociale. Ces résultats sont à nuancer néanmoins par des variabilités interindividuelles.

Book excerpt: Lorsqu'elle touche les enfants dès leur naissance, la surdité est un handicap qui a d'importantes répercussions sur le développement général et langagier. Les progrès technologiques dans ce domaine ont permis la création et l'amélioration de l'implant cochléaire, un dispositif médical qui restaure en partie l'audition et qui offre une alternative lorsque les limites des prothèses auditives sont atteintes. La prise en charge orthophonique des enfants implantés a pour objectifs principaux l'interprétation des stimuli qui constituent le monde sonore environnant, et l'acquisition d'une langue fonctionnelle et correcte. A l'adolescence, de nombreux bouleversements perturbent l'équilibre émotionnel des jeunes. Afin de gérer au mieux cette période, il nous semble essentiel que les jeunes puissent verbaliser leurs émotions, les comprendre, mais également comprendre celles d'autrui pour pouvoir ajuster leurs comportements et leurs réactions. Pour les jeunes sourds, le domaine émotionnel est particulièrement complexe à appréhender puisqu'il reste abstrait et que les éléments perceptibles ne peuvent pas être directement décodés. Nous avons choisi d'aborder, avec les 7 jeunes qui constituent notre population, 4 émotions fréquemment rencontrées : la peur, la joie, la colère et la tristesse. Afin de leur offrir une approche complète et plaisante, et de favoriser la mémorisation, nous leur avons présenté des supports qui proposaient des modalités d'entrée variées (auditive, visuelle, audiovisuelle). Au fil des séances, nous avons analysé l'évolution de leur vocabulaire émotionnel actif et passif ainsi que leur aisance à comprendre les émotions d'autrui. Les données ont été recueillies grâce à des enregistrements vidéo et par la complétion de grilles créées spécifiquement. A l'issue des séances, nous avons remarqué que pour tous les jeunes, le vocabulaire émotionnel s'était enrichi, diversifié et précisé. Leur compréhension des émotions était satisfaisante dès le début, mais nous avons constaté que la difficulté concernait surtout la verbalisation de ce qu'ils avaient compris, imaginé ou interprété.
