Book excerpt: The purpose of this study was to compare examiner reliability of cephalometric measurements from cone beam computed tomography (CBCT) and conventional digital cephalograms. Three imaging modalities were compared: CBCT using maximum intensity projection and ray-sum algorithms, and conventional digital cephalograms. Blinded examiners were calibrated, and intra- and inter-examiner reliability were assessed. Data were collected on a convenience sample of seven subjects. Examiners obtained linear and angular measures on each of the subjects' images, and rater consistency among the three methods were assessed using repeated measures analysis of variance of intraclass correlation coefficient ratios. A statistically significantly greater level of consistency for linear measurements was seen with the CBCT images. For the conventional digital images, linear measurements showed less consistency than angular measurements. This statistical significance unlikely correlates to a clinical significance. For orthodontic diagnosis, treatment planning, and treatment evaluation, CBCT generated cephalograms are equally effective to conventional digital cephalograms.

Book excerpt: Introduction:With attention being given to the deleterious effects of radiation exposure from dental radiographs and inaccuracies in cephalometric soft tissue measurements, an alternative method of facial analysis with sufficiently reliable soft tissue landmarks should be developed. The goals of this study were threefold: (1) to define a new, low-cost method for capturing standardized frontal and sagittal facial images, (2) to determine on which photographic view that landmarks can be more reliably located, and (3) to determine which landmarks are appropriate for quantitative facial analysis. Materials and Methods:Simultaneous frontal and right sagittal facial images of 10 male and 10 female dental student subjects were captured using high-definition webcams as part of a low-cost set-up. Seventeen identical predefined facial soft tissue landmarks were located by 5 examiners on both types of images and were recorded as coordinate values. These coordinate values were used to calculate the best estimate of the true value for each landmark, mean deviation from this best estimate, and reliability in the X- and Y-axes using the Shrout-Fleiss intraclass correlation coefficient with corresponding 95% confidence intervals. Two examiners repeated the landmark location to evaluate intra-examiner reliability. Results:With a 95% confidence interval range of>0.950, nose and mouth landmarks were among the most reliable landmarks on frontal and sagittal facial images. Converselyright soft tissue gonionwas one of the least reliable landmarks located in this study. In general, landmarks located by a single examiner showed greater reliability than when there were multiple examiners. Conclusions:This low-cost method yielded frontal and sagittal images sufficient for landmark identification. The magnitude of error varies between landmarks, is largest for poorly demarcated landmarks, and most had a non-circular envelope of error. Certain landmarks were more reliable on sagittal images and others were more reliable on frontal images. All landmarks had greater reliability and less mean deviation when located by a single examiner.

Book excerpt: Abstract: Orthodontic practitioners are acquiring digital records and analyzing cephalometric radiographs with imaging software. However, few studies have compared hand traced cephalometric superimpositions with those produced by current computer programs. The specific aim of this study was to quantitatively compare hand traced and digital computer based superimpositions. Sixty four high quality initial (T1) and final (T2) cephalometric film radiographs were selected from an orthodontic archival database after Institutional Review Board approval. Cephalometric landmarks were identified. Cranial base and regional (maxillary and mandibular) superimpositions were completed independently for each pair of radiographs by the two methods. Digital superimpositions were created using the free-form feature in Dolphin Imaging 10 (Dolphin Imaging and Management Solutions, Chatsworth, CA). Twenty sets of hand and digital superimpositions were repeated one week apart to examine for intraoperator error. Both hand and digital superimpositions were imported into the GetData (GetData Graph Digitizer, S. Fedorov) digitizing program to obtain the x-y coordinates of each landmark. Linear distance between multiple paired (hand and digital) T2 cephalometric landmark locations (e.g., A point) were measured. After accounting for intraoperator error, a T2 landmark distance of zero would suggest that there was no difference between the hand and digital method. To account for skewed distributions, the mean and median linear distances and 95% confidence intervals between the T2 cephalometric landmark locations were calculated. A Friedman test was applied to examine for error between repeated hand superimpositions, error between repeated digital superimpositions, and the T2 landmark distances between the hand and digital methods. The upper 95% confidence limit for the median of the linear distances was 1 mm for all landmarks except maxillary incisor tip (1.02 mm). The error calculations indicated no statistically significant differences (p 0.05) for the reliability of each method and, more importantly, between superimposition methods. There was no difference between cranial base and regional superimpositions created by the traditional hand tracing method when compared to digital superimpositions. This study validates the use of digital superimpositions produced by Dolphin Imaging version 10.
