The computer survey included questions on IPV and other health risks. Computer-generated risk reports were attached to patients' medical charts for review by their physician. Participants in the control arm received standard medical care. Data were collected by audio taping the medical visits and an exit survey of patients. The results reveal that the overall prevalence of IPV in the family practice was high; 14.6% in the pencil and paper survey and 13.7% in the RCT study. The RCT demonstrated that, compared to standard medical care, the computer-assisted screening resulted in a 1.8-fold increase in the likelihood of physician and patients discussing IPV risk and the detection of women at risk of IPV increased by a factor of 2.5-fold. Further, RCT participant patients showed acceptance of the computer-assisted screening, after its use. The interviewed physicians perceived computer-assisted screening as most feasible for periodic health exams and follow-up visits, given the existing logistical limitations. Overall, physicians expressed general acceptance of the computer screening. The results provide impetus for improvements in clinical practice, medical training, policy and research to address the issue of IPV. The use of the screening tool may be enhanced by continued research into its generalizability by inclusion of multiple family practice clinics and various primary care settings. The overall aim of the dissertation research was to advance knowledge on the phenomenon of computer-assisted screening for intimate partner violence (IPV) within a Canadian family practice context. A series of inter-linked studies (quantitative and qualitative) was conducted to answer three research questions: (1) what is the effectiveness of computer-assisted screening for increasing physician-patient discussions on and detection of IPV risk? (2) what is the acceptance of computer-assisted screening among patients? and (3) what is the feasibility of computer-assisted screening from the perspectives of family physicians? To answer the first two questions, a randomized controlled trial (RCT) was conducted at an academically-affiliated family practice clinic in Toronto with adult female patients in a current or recent intimate relationship (n = 314). The RCT planning was facilitated by studies in the pre-trial phase (a pencil and paper survey of patients, and a psychometric scale validation). To answer the third question, in-depth qualitative interviews were conducted with the trial physicians (n = 10).

Too often this preventable form of violence is repetitive in nature, occurring at multiple points across the lifespan. The prevalence of intimate partner violence is on the higher end of this spectrum in East Africa, with in-country demographic and health surveys indicating that approximately half of all women between the ages of 15-49 in Uganda, Kenya, and Tanzania having experienced physical or sexual abuse within a partnership. It is now widely accepted that preventing intimate partner violence is possible and can be achieved through a greater understanding of the problem; its risk and protective factors; and effective evidence-informed primary, secondary, and tertiary prevention. To that end, on August 11-12, 2014, the Institute of Medicine's Forum on Global Violence Prevention, in a collaborative partnership with the Uganda National Academy of Sciences, convened a workshop focused on informing and creating synergies within a diverse community of researchers, health workers, and decision makers committed to promoting intimate partner violence-prevention efforts that are innovative, evidence-based, and crosscutting. This workshop brought together a variety of stakeholders and community workers from Uganda, Kenya, and Tanzania to engage in a meaningful, multidirectional dialogue regarding intimate partner violence in the region. Book excerpt: Intimate Partner Violence (IPV), the most prevalent type of violence against women, has been recognized as a human rights violation by intergovernmental organizations and international law. Due to its physical and psychological sequelae, the World Health Organization has considered IPV a major global public health issue since 1997. Primary care health care professionals (HCPs) are considered especially well-positioned to identify women experiencing IPV, yet screening rates are low. Utilizing the Integrated Behavioral Model (IBM) and Intersectional Feminism as guiding theoretical frames, this mixed method study (qualitative, N = 35; quantitative, N = 176) explored Barbadian HCPs' attitudes, beliefs and knowledge regarding IPV screening, including personal and professional factors that facilitate or inhibit screening. Future screening intention and past screening behaviors were assessed. Findings from the study indicate that the IBM model explains a significant amount of variance in predicting screening intention, with the IBM construct of self-efficacy being the strongest theoretical predictor of screening intention. Overall, practitioners' personal experience of physical abuse was the strongest predictor of screening intention. Implications for social work include future IPV research using a community-based participatory research methodology; in terms of practice and education, increasing the number of medical social workers in primary care settings and public health social workers, capitalizing on social work's person-in-environment perspective to engage systems at multiple levels in addressing IPV. Policy recommendations are to engage in a chronic disease model of care and trauma-informed practice to empower women experiencing violence in primary care settings.

Book excerpt: This review is an update for the U.S. Preventive Services Task Force (USPSTF) recommendation on screening women for intimate partner violence (IPV) and elderly and vulnerable adults for abuse and neglect. The USPSTF defines screening as obtaining information about abuse from individuals in health care settings who do not have complaints or obvious signs of abuse, such as physical injuries. In 2004, based on results of a previous review, the USPSTF found insufficient evidence to recommend for or against routine screening of women for IPV or of older adults or their caregivers for elder abuse. The USPSTF could not determine the balance between the benefits and harms of screening because of the lack of critical evidence, particularly the lack of trials of the effectiveness of screening in health care settings and the effectiveness of interventions to reduce harm from abuse. The USPSTF reviewed several existing screening instruments that demonstrated adequate internal consistency and were validated with longer instruments. However, none were evaluated against measurable violence or health outcomes. Also, despite reviewing an extensive literature on IPV, few studies provided data on screening and management to guide clinicians in practice, and there was little to no evidence from studies of elder abuse or neglect. The Centers for Disease Control and Prevention recognizes four categories of IPV, including physical violence, sexual violence, threat of physical or sexual violence, and psychological or emotional abuse. For this review, abuse and neglect of vulnerable adults is also considered with elder abuse. A vulnerable adult is a person age 18 years or older whose ability to perform the normal activities of daily living or to provide for his or her own care or protection is impaired due to a mental, emotional, long-term physical, or developmental disability or dysfunction, or brain damage. Definitions vary by State, and sometimes included in the definition is the receipt of personal care services from others. Types of elder abuse that also apply to vulnerable adults include physical abuse, sexual abuse, emotional or psychological abuse, neglect, abandonment, financial or material exploitation, and self neglect. Based on evidence gaps identified from the previous review and using the methods of the USPSTF, the USPSTF and Agency for Healthcare Research and Quality determined Key Questions for this review. Key Questions for IPV include: 1. Does screening asymptomatic women in health care settings for current, past, or increased risk for IPV reduce exposure to IPV, physical or mental harms, or mortality? 2. How effective are screening techniques in identifying asymptomatic women with current, past, or increased risk for IPV? 3. What are the adverse effects of screening for IPV? 4. For screen-detected women with current, past, or increased risk for IPV, how well do interventions reduce exposure to IPV, physical or mental harms, or mortality? 5. What are the adverse effects of interventions to reduce harm from IPV? Key Questions for elder and vulnerable adult abuse and neglect include: 1. Does screening asymptomatic elderly and vulnerable adults in health care settings for current, past, or increased risk for abuse and neglect reduce exposure to abuse and neglect, physical or mental harms, or mortality? 2. Book excerpt: BACKGROUND: Intimate partner violence (IPV) is a worldwide epidemic that is often overlooked, ignored, and underreported. However, it greatly affects the health of women, especially in pregnancy. Healthcare providers and other staff may be in the optimal positions to screen and to identify victims of IPV. The objective of this project is to implement an evidencebased IPV screening tool and protocol for all pregnant and postpartum women, visiting the Women’s Health Department at Waianae Coast Comprehensive Health Center (WCCHC). METHODS: Based on a literature review, the Abuse Assessment Screen (AAS), a five-item tool that assesses for physical, emotional, and sexual abuse, was implemented in the Women’s Health department from August to November 2014. In addition, an updated IPV screening and intervention protocol was implemented. Data was collected prior to and after the implementation of the new screening tool and protocol. Descriptive statistics and trend analysis was completed. OUTCOMES: The AAS, used to screen pregnant women during initial OB visits, revealed an overall IPV rate of 14.6% (6% current IPV, 9% history of IPV), when screened by APRNs. The APRNs had an overall 90.2% screening rate. The perinatal case managers revealed a 6.7% current IPV rate and 10.8% history of IPV in pregnant women, with an overall 63.2% screening rate. Postpartum screening by APRNs revealed an overall 56.5% screening rate, with a 9.8% IPV rate. The most common interventions for women who screened positive for IPV were being counseled directly by the provider and being given a phone resource card, which includes the IPV shelter numbers and local IPV information line. A small percentage of women were referred to Behavioral Health. CONCLUSION: Routine IPV screening for all women seeking prenatal and postpartum care is critical in order to identify and help victims of IPV. Use of the AAS and the new screening and intervention protocol has increased the identification of victims of IPV. Providers should continue to screen for IPV and provide services to pregnant and postpartum women.
