There is limited research on the efficacy of interventions to reduce SB and improve physical function and mobility in older adults of moderate-to-low physical function. Purpose: To examine the efficacy of a 12-week intervention, Stand Up Now (SUN), to reduce SB and improve physical function and mobility in older adults of moderate-to-low physical function in Continuing Care Retirement Communities (CCRCs). Methods: SUN was conducted in four CCRCs that were randomized to two intervention groups with different SB change goals: one group focused on reducing total sedentary time (SUNSL) and one group focused on increasing sit-to-stand (STS) transitions (SUNSTS). Participants (N = 71; Mage=87±7yrs) in both SUN groups had weekly health coaching sessions over 12 weeks. SB, physical function, and mobility were measured at baseline, 6, and 12 weeks via the activPAL, Short Physical Performance Battery (SPPB), and the 8-foot up-and-go (8ft UG), respectively. Linear Mixed Models (LMM) were used to examine the efficacy of SUN on activPAL measured variables (i.e., sitting, standing, STS transitions, stepping, and steps), physical function, and mobility at 6 and 12 weeks. Results: Both SUN groups significantly decreased sedentary time (1.3±0.3hrs, p0.001) and increased standing time (0.5±0.2hrs, p0.02) at 6 weeks that was maintained at 12 weeks, compared to their baseline. Older adults accumulate large amounts of ST and low amounts of physical activity, which may compound the health risks. There is little research examining strategies to reduce ST among older adults, especially within assisted living residences (ALRs). The purpose of this study was to develop and pilot-test an evidence-informed intervention to reduce ST within ALRs. Stand to Strengthen is a flexible, multi-level intervention that can be delivered in a variety of ALRs. A 6-week feasibility trial using Stand to Strengthen was conducted to examine changes in function, quality of life (QoL), and ST patterns. There was a trend towards less self-reported weekday ST post intervention (p = 0.086) but no significant differences in other variables overall or for higher function participants. Lower function participants demonstrated significant, clinically meaningful improvements in physical function post-intervention (p=0.035, Cohen's d = 0.89). This study demonstrated that a multi-level intervention to reduce ST may delay functional declines as older adults transition to assisted living.

Book excerpt: Sedentary behavior has emerged as a major public health threat because accumulating evidence links excessive sitting with deleterious health consequences, independent of physical activity. On average, older adults spend more than 60% of their waking hours each day sitting -- the most out of any segment of the population. Understanding the factors that contribute to this high volume health behavior and its consequences will facilitate the development of effective interventions to reduce sedentary time that also promote healthy and successful aging. This dissertation aims to (1) identify the motivational processes that contribute to sedentary behavior, (2) test relations between sedentary behavior and life satisfaction -- an important indicator of well-being and successful aging in older adults, and (3) evaluate the feasibility, acceptability, safety, and preliminary efficacy of a brief psychoeducational intervention designed to reduce older adults' sedentary behavior. A 14-day ecological momentary assessment study with ambulatory monitoring was conducted to capture time-varying and time-invariant motivation, behavior, and evaluations among older adults (n = 100). These data were used to test hypotheses about the motivational antecedents of daily sedentary behavior and hypotheses about relations between daily sedentary behavior and life satisfaction. Next, a brief psychoeducational intervention to reduce older adults' sedentary behavior was developed and implemented at local senior centers (n = 5). This intervention was evaluated in terms of feasibility, acceptability, safety, and preliminary efficacy. Results of these studies led to five major conclusions. First, older adults' sedentary behavior is regulated by both controlled and automatic motivational processes. Second, both between-person and within-person motivational processes were associated with sedentary behavior. Third, sedentary behavior and life satisfaction were negatively linked through within-person processes. Fourth, older adults found the psychoeducational intervention approach to be engaging. Finally, the intervention reduced older adults' self-reported weekday sedentary behavior by approximately two hours more per day compared to the control group. Ultimately, this dissertation informs the development of interventions designed to reduce older adults' sedentary behavior to promote healthy and successful aging. Prioritizing healthy aging interventions of this nature may reduce health care costs associated with treatment and management of chronic diseases and improve care for older adults.

It is estimated that 4 out of every 10 Americans never engage in physically active behaviors, and that approximately 60% of an adult's non-sleeping hours are spent in sedentary behaviors. As sedentary behavior increases, so do diagnoses of chronic illnesses such as diabetes, hypertension, cardiovascular disease, and kidney disease. Older adults are particularly at risk for sedentary behavior and the related chronic illnesses due to the challenges they face with balancing physical and/or cognitive limitations, medical conditions, and the requirements to remain physically active. Often times older adults feel that their only option for safe physical activity is low-speed walking. Occupational therapy intervention options that can decrease sedentary behavior in older adults in the community are scarce, thus the Everyday Meaningful Activities (EMA) intervention is proposed. The EMA Intervention combines concepts from Habit Formation Theory and the Ecology of Human Performance model. It is an individualized, client-centered intervention that aims to increase adherence to meaningful active lifestyle behaviors in older adults' lives. Meaningful active lifestyle behaviors are tasks or activities that promote enjoyment in time spent moving around (i.e. cooking, gardening, creating art, etc.). Participants created new habits by attaching these new active lifestyle behaviors to currently existing daily routines. The aims of this study are (1) to investigate the feasibility of implementing the EMA intervention with medically stable older adults, (2) to investigate the effectiveness of the EMA intervention in forming new active lifestyle behavior habits, and (3) to investigate the effectiveness of the EMA intervention in decreasing sedentary time. Method: Twelve medically stable older adults in the Greenville, NC community were recruited for this pretest/posttest, experimental design study. During six intervention sessions over six weeks, participants selected two new active lifestyle behaviors to make habitual and created action plans for implementation. Results: The recruitment rate for this study was 18%. The Rapid Assessment Disuse Index (RADI) was found to not be an appropriate measure for sedentary behavior for this population. The Short Blessed Test (SBT) was found to be an appropriate measure for screening cognitive impairment over the phone. Video conferencing was found to be an appropriate delivery method for selected intervention sessions if the participant is comfortable. Participants' subjective report of activity performance times is not a reliable subjective data collection method. The EMA intervention was effective in forming new active lifestyle behavior habits according to the Self- Report Habit Index (SRHI) and in decreasing perceived sedentary time spent according to the Sedentary Behavior Questionnaire (SBQ). Discussion: The EMA intervention is a viable intervention for decreasing sedentary behavior in medically stable older adults. With minor changes to the assessments used for screening, a wellrounded sample of participants can be created for pilot testing. In future applications of the EMA intervention, more reliable subjective data on activity performance should be collected in order to compare subjective and objective reports of activity performance.

A mixed methods exploratory analysis was completed to examine the cardio-metabolic and psychologic health effects of a sedentary behavior intervention. Participants were obese older adults with depressed mood, pre-diabetes/diabetes or hypertension, who were a part of the I-STAND study at Kaiser Permanente Washington Health Research Institute. Linear regression models compared pre and post measures between intervention and control groups adjusting for baseline values. A post-hoc qualitative analysis assessed I-STAND participant interview data. There were 60 participants in the study population. In all three sub-groups, participants were primarily female and white. Significant mean reductions in sitting time were observed for intervention groups compared to controls in each sub-group. There was not strong evidence for health benefits with reductions in sitting time. Key qualitative themes included barriers to stand, perceived physical health improvements and perceptions of physical activity. Further research should examine whether sitting reduction can act as a facilitator to increased physical activity and benefit cardio-metabolic health and psychologic health.

At workplaces, managers and companies should encourage sitting breaks and work standing options, and for the special population such as children or older adults, new strategies must be considered to reduce sitting time.

Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. Book excerpt: Sedentary behavior (SB) increases the risk for functional decline and poor health outcomes in older adults, thus, it is important to develop effective interventions to reduce SB. The purpose of this dissertation was to examine the feasibility, acceptability, and effectiveness of a 'Stand Up and Move More' (SUMM) intervention for African American elders to reduce sedentary behavior and improve physical function and health-related quality of life. To do so, two preliminary studies were conducted. Study 1 was a single-arm pilot study where participants (N=8; age 71+8 years) completed assessments at baseline (wk0), post-intervention (wk4), and follow-up (wk12). Study 2 was a small semi-randomized controlled trial where participants completed the SUMM workshop or an attention matched control group (i.e, Stress Management). Participants in Study 2 (N=18, age=70+9) completed assessments at wk0 and wk4. Effect size calculations (Hedges' g) were used to determine the magnitude of the treatment effect. In Study 1, there were moderate to large improvements in physical function (overall function g=0.54, balance g=0.60, chair stand test g=-0.82) and health-related quality of life (SF-36 subscales: role limitations-physical g=0.94, role limitations-emotional g=0.55, mental health g=0.64, and social functioning g=1.07) from wk0 to wk4. Also, there were moderate to large improvements in sit-to-stand transitions (g=0.93), light intensity physical activity (g=1.97) and gait speed (g=-0.75) when assessed at wk12. In Study 2, there were moderate to large improvements in physical function following both workshops (SUMM balance g=0.88; Stress Management chair stand test g=-0.51) from wk0 to wk4. There was a moderate improvement in health-related quality of life (i.e., limitations due to emotional health g=0.54) in the SUMM group from wk0 to wk4. In both studies, the SUMM workshop was safe; participants found the workshop was encouraging and increased awareness of their sitting time; and the workshop leader found the content acceptable and beneficial. Together, the results of this dissertation indicate the SUMM workshop was a safe, feasible, and acceptable intervention and may provide function and quality of life benefits for African American elders. Future work with larger sample sizes is needed to confirm the benefits of the SUMM intervention for African American elders.

Following 2 participant dropouts, 18 sedentary adults were randomly allocated to a Control (3 older females, 84±8 years; and 6 younger adults, 3♀, 23±3 years) and Intervention (5 older adults, 4♀, 75±15 years; and 4 younger adults, 3♀, 24±3 years) group. The Intervention group watched an educational video that highlighted the negative health consequences of excessive ST and received 2-4 messages/week via text or email prompting them to decrease their ST. At Baseline, 3 months, 6 months, and 9 months, a thigh-worn activPAL inclinometer recorded habitual physical and sedentary activities for 7-days and assessments of frailty and brachial FMD were conducted. No Group × Time effects were observed for ST, sedentary breaks, light-intensity physical activity, moderate-to-vigorous intensity physical activity, sleep time or standing time (all, p>0.122). There were also no interaction effects for frailty (p=0.667) or brachial FMD (p=0.502). Based on monthly follow-up phone calls with the Intervention group, there were several life factors that may have acted as barriers to changes in habitual behaviour including: mental health, work/school schedule, weather changes, and/or willingness to change. This intervention was feasible based on low drop out (10%) and high acceptability among participants. However, it was not effective at decreasing ST in adults. Future interventions may need to include more frequent prompt/phone calls and/or a better individualized approach to reducing ST in adults.

Book excerpt: Background: Society is sitting more than ever before. Large-scale epidemiological evidence indicates that prolonged sitting time has negative health impacts including increased risk of metabolic syndrome, heart disease, weight gain, cancer, and premature mortality. Older adults are an important population to target because they represent the most sedentary segment of the population who struggle to meet activity guidelines. Based on these negative health associations, research on sedentary behavior interventions, especially focused on older adults, is a public health priority. Methods: This dissertation uses data from a pilot sedentary behavior intervention in 30 adults aged 50-70 years to understand how to measure and prompt sedentary behavior change. The intervention successfully targeted two distinct sitting interruption modalities (i.e., sit less, increase sit-to-stand transitions). Chapter 1 explored differences in self-reported and objectively-measured sitting time to evaluate participants' ability to self-assess behavior during an intervention. Chapter 2 included a mixed methods analysis of tool use to disrupt sitting time during the pilot intervention. Chapter 3 used focus groups to explore participants' perceptions regarding wearable devices to track and change sedentary behavior. Results: Chapter 1 found significant differences in self-reported sitting time by day of week, employment status, and participation in a sedentary behavior intervention. Chapter 2 showed that participants who used effective tools were most successful in reducing sitting time. In contrast, current tools for increasing sit-to-stand transitions were ineffective. The focus groups from Chapter 3 revealed that participants were amenable to using wearable devices; however, current devices lack key features necessary for sedentary behavior including the ability to accurately measure sitting time and distinguish "inactivity" from standing. Discussion: Given the negative health outcomes associated with excessive sitting, more interventions are targeting sedentary behavior. The three themes explored in this dissertation (specificity of measurement, tools, and behaviors) and the combination of analysis methods help increase our understanding of sedentary behavior in older adults. This dissertation provides recommendations to improve the field by using specific measures for sitting time to capture differences across the week, designing interventions to include tools with a specific focus on sedentary behavior, and exploring how technology can help change behavior.
