Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. It aims to contribute to knowledge of the actual and potential roles of partnership in international agricultural research for development. The paper summarizes conclusions and insights from four distinct professional literatures: research studies; professional evaluation literature; practitioner-oriented reviews, guidelines and assessment tools; and CGIARrelated reviews, evaluations and policy documents. It identifies and analyzes key cross-cutting themes and success factors, highlights gaps in current knowledge, and identifies high-potential areas for further study. A wide range of research-based publications is reviewed, including studies in such fields as management and organizational development, public administration, economics and international development. Work in these fields covers such diverse topics as the role of inter-organizational collaboration in strategic management, public–private and cross-sector partnerships, North–South partnerships, roles of partnership in linking research with action, networking and transactions costs. The different literatures talk little to each other and are highly self-referential. Nevertheless, some common patterns, themes and concerns emerge related to definitions, partnership drivers and dynamics, trust and mutuality, power asymmetries and inequities, and success factors. It is noteworthy that empirical studies of partnerships are rare, particularly in-depth case studies. Theoretical pieces seldom present empirical tests of hypotheses, and practical guidelines are seldom grounded in theory. There is a clear need for more systematic and in-depth empirical research on partnership experiences. Although partnership is now considered an essential way of working in many fields, several authors caution that the costs of working in partnership may often exceed the benefits. Before establishing a partnership, one should identify a clear value-added proposition. Many reports on partnership prepared for the CGIAR are available only in grey literature, leading to difficulties in accessing them and risking a loss of knowledge. Gaps in knowledge are identified at the level of individual partnerships, the level of the organizations that participate in or manage portfolios of partnerships, and the level of research or innovation domains that are characterized by networks of partnerships

This thesis sought to identify public discourses around international collaboration in health research and to then conduct a discursive case analysis of an organisation that claimed to achieve international collaboration in health research. The analysis was informed by the work of Michel Foucault, whose notions of truth and power and the human experience were central to this thesis. This enabled an in-depth exploration of the utility of language in context but was also accompanied by more pragmatic investigative techniques that assisted with analysis of organisational specific aspects of the research. Without a doubt, the results of this study demonstrate that international collaboration in health research was deemed a worthy and valuable cause that holds great potential to advance knowledge across borders and enhance the health of the global community. The resulting model for international collaboration aims to mobilise organisational discourse in such a way as to promote more comprehensive understandings and frameworks within which collaborative processes can take place. By developing such a model it is hoped that the collective goal of improved health for all may not be such a distant possibility.

Book excerpt: A critical problem in resource-scarce countries across the globe is the shortage of appropriately trained health care providers. According to the World Health Organization, the current global health workforce shortage of 7.2 million providers is estimated to increase to 12.9 million by 2035. This disproportionately affects resource-scarce countries, denying basic health care to millions and limiting access to life-saving treatments. Due to limited resources in these countries, not enough health professionals receive training, few have the opportunity for continuing education, and the ability to develop or implement educational programs and curricula is constrained. Additionally, many existing providers choose to emigrate in pursuit of professional advancement opportunities, contributing to the overall shortage of qualified health care providers in these environments. Efforts to strengthen health workforce capacity not only increases access, safety and availability of care, but is critical to building resilient health systems capable of caring for the world’s neediest populations. This requires not only cultivating new health care providers, but also providing ongoing professional development to retain and support current providers, advancing the level of practice in accordance with current clinical science, cultivating educators, and enhancing training curricula. It is critical also to contribute to the limited body of research documenting the effectiveness and impact of various models of collaborative education and partnership to improve health worker training and retention. This Research Topic examines strategies for building health workforce capacity through the prism of educational partnerships, offering significant examples of effective models of international collaborative education as well as insight and guidance on the structure and operation of successful global partnerships. Collectively, the 31 articles accepted and included in this eBook represent a diversity of health professions and geographies across academic, non-governmental organizations and other global partnership forms. The published manuscripts highlight various elements of partnerships with several consistent themes emerging: capacity building, local empowerment, mutual trust and respect, long-term commitment, equity, collaboration, and the importance of integrating theory and practice, for a balance of academic and clinical development. The manuscripts provide examples of partnership and educational programs that are in the formative, early stages of implementation and others which have been sustained long term, some for decades. The following eBook is divided into two parts, with each part broken down into sections. Part I of the eBook includes 18 manuscripts that showcase long-term educational programs that strongly exemplify multiple, foundational aspects of international partnerships in education including mutual collaboration and project management, empowerment of host partners to lead and sustain programs, and capacity building. While individual manuscripts included in Part I look broadly at multiple aspects of successful, international partnerships in education, Part II manuscripts focus intently on one-two elements. Part II includes 13 articles that highlight partnership through short- rather than long-term educational initiatives as well as program development and broad academic partnerships.
