No effective treatments are available for T2D, although there have been many developments in the therapeutic arena. Nitric oxide (NO) is an endocrine agent with multiple and important biological roles in most mammalian tissues. NO has emerged as a central regulator of energy metabolism and body composition. NO bioavailability is decreased in T2D. Several of the pharmaceuticals used in T2D affect the NO system and perhaps even more so by the drugs we use to treat diabetic cardiovascular complications. Experimental works in animal models of T2D show promising results with interventions aimed to increase NO signaling. However, translation into human studies has so far been less successful, but more large-scale prolonged studies are clearly needed to understand its role. This book is a collection of reviews that deal with the role of nitric oxide in type 2 diabetes, providing a unique overview of NO signaling, and pointing out key areas for more detailed research. The endothelium is a major player in the control of blood fluidity, platelet aggregation and vascular tone, a major actor in the regulation of immunology, inflammation and angiogenesis, and an important metabolizing and an endocrine organ. Endothelial cells controls vascular tone, and thereby blood flow, by synthesizing and releasing relaxing and contracting factors such as nitric oxide, metabolites of arachidonic acid via the cyclooxygenases, lipoxygenases and cytochrome P450 pathways, various peptides (endothelin, urotensin, CNP, adrenomedullin, etc.), adenosine, purines, reactive oxygen species and so on. Additionally, endothelial ectoenzymes are required steps in the generation of vasoactive hormones such as angiotensin II. An endothelial dysfunction linked to an imbalance in the synthesis and/or the release of these various endothelial factors may explain the initiation of cardiovascular pathologies (from hypertension to atherosclerosis) or their development and perpetuation. Book excerpt: Activation of inducible nitric oxide synthase (iNOS) and oxidative stress have been shown to be associated with compromised cardiovascular function in streptozotocin (STZ)-induced type 1 diabetes. The aim of the project is to investigate cardiovascular abnormalities in a rat model of type 2 diabetes (Zucker diabetes fatty or ZDF rats) and two models of metabolic syndrome (fructose-fed rats and Zucker obese rats), and to provide direct evidence linking iNOS and oxidative stress to abnormal cardiovascular function in these disorders. Blood pressure, cardiac contractility, cardiac index, regional flow, vascular resistance and venous tone were measured in diseased as well as normal rats. Biochemical analyses such as activities of iNOS, immunostaining of iNOS and western-blot analysis of iNOS in the heart tissue were carried out. The results showed that cardiac contractile response to dobutamine was compromised in the ZDF rats, and this was associated with increased myocardial protein expression as well as activity of iNOS. The formation of peroxynitrite was increased in the heart tissue of the ZDF rats. Selective inhibition of iNOS by 1400W (N-3-aminomethyl-benzyl-acetamidine) did not alter responses to dobutamine in the control rats, but augmented the contractile effects of dobutamine in the diabetic rats. The regional blood flow was altered in the ZDF rats, and iNOS played a negligible role in regulating regional flow in the ZDF rats. Although venous response to noradrenaline was also altered in the Zucker obese rats, NOS may not be involved in venous tone regulation. Anti-oxidative treatment with N-acetylcysteine inhibited the development of insulin resistance, blood pressure elevation and the increase of 8-isoprostane formation in the fructose-fed rats. We conclude that heart function is compromised and regional blood flow is altered in the ZDF rats. Activation of iNOS plays an important role in suppressing heart dysfunction but does not affect regional blood flow.

Book excerpt: Neurodegenerative disease (e.g. Alzheimer's disease, dementia, stroke) affects 1 in 6 people worldwide, leads to chronic disability and a loss of quality-of-life, and costs hundreds of billions of healthcare dollars in the US alone. Reduced cerebral blood flow (CBF) is associated with cognitive impairment, many neurodegenerative disorders, and is observed in the early stages of neurodegenerative disease development. Insulin resistance (IR) is an independent risk factor for neurodegenerative disease and early changes in resting CBF in adults with IR may contribute to neurodegenerative disease development and worse disease outcomes. Despite the link between IR, neurodegenerative disease, and altered CBF, we do not know enough about how vascular dysfunction manifests or the role of IR in neurodegenerative disease development. Thus, it is essential that we identify key vascular signaling pathways involved in maintenance of resting CBF in healthy adults as a fundamental understanding of healthy physiology of the cerebral circulation will help us understand how this physiology is altered during IR. Given that the role of NOS signaling in control of resting CBF remains controversial, Aim 1 of this dissertation will determine the contribution of NOS signaling to control of resting CBF in healthy adults. Next, animal data indicate insulin triggers a robust NOS-mediated dilation in cerebral arteries, and is dysfunctional in animals with IR. An oral glucose tolerance test (OGTT) induces a physiologic insulin surge, which may produce vasodilation in human cerebrovasculature. Furthermore, an OGTT may provide a new challenge for assessing cerebrovascular function in adults with IR. Aim 2 of this dissertation will evaluate CBF responses to an OGTT and the contribution of NOS to changes in CBF during OGTT to characterize the role of insulin signaling in regulation of CBF. Finally, studies in older adults with type 2 diabetes reveal lower resting global and regional CBF, yet younger adults with less severe IR remain largely understudied and early changes in CBF may contribute to worse disease outcomes. Aim 3 of this dissertation will determine if adults with IR exhibit diminished resting CBF compared to healthy counterparts to identify if vascular dysfunction is already present in adults with IR.

Book excerpt: One of the major biomedical triumphs of the post-World War II era was the defmitive demonstration that hypercholesterolemia is a key causative factor in atherosclerosis; that hypercholesterolemia can be effectively treated; and that treatment significantly reduces not only coronary disease mortality but also all cause mortality. Treatment to lower plasma levels of cholesterol - primarily low density lipoprotein (LDL) cholesterol - is now accepted as best medical practice and both physicians and patients are being educated to take aggressive measures to lower LDL. We can confidently look forward to important decreases in the toll of coronary artery disease over the coming decades. However, there is still uncertainty as to the exact mechanisms by which elevated plasma cholesterol and LDL levels initiate and favor the progression of lesions. There is general consensus that one of the earliest responses to hypercholesterolemia is the adhesion of monocytes to aortic endothelial cells followed by their penetration into the subendothelial space, where they differentiate into macrophages. These cells, and also medial smooth muscle cells that have migrated into the subendothelial space, then become loaded with mUltiple, large droplets of cholesterol esters . . . the hallmark of the earliest visible atherosclerotic lesion, the so-called fatty streak. This lesion is the precursor of the more advanced lesions, both in animal models and in humans. Thus the centrality of hypercholesterolemia cannot be overstated. Still, the atherogenic process is complex and evolves over a long period of time.
