Book excerpt: Recent disputes over the creation of a new Ministry of Justice have demonstrated the need for a better way of making changes to the machinery of government. At present there is no mechanism to ensure that changes are fully considered before implementation and that the reasoning behind them stands up to scrutiny. The Ministers of the Crown Act 1975 ought to act as a Parliamentary check but at present it does not serve that function well as the Government has the power to decide whether Parliament gets a vote or even a debate. The Committee think that at the very least there ought to be timely debates because the need for Parliamentary assent would lead to greater scrutiny and therefore greater legitimacy. The Committee also note that the current machinery of government is fundamentally the result of the Haldane Report of 1918. It is time for a fresh look at the entire structure of government and a new Haldane inquiry.

These include competitive advantage, competitive positioning, horizontal strategy and organisational design, and innovation as an all-encompassing organisational adjustment mechanism to a changeable environment. But these are not presented as a silver bullet, and Ferguson addresses other approaches to reform, including the need to rebuild the Public Sector Act, the need to reconsider the interface between political and administrative arms of government (and determine what is in the ‘public interest’), and the need for greater independence for the public service within a clarified role. It also applies these principles to set out proposals for reforming British government. The report concludes that many aspects of Britain's governing structures and principles work well, despite opinions to the contrary that are sometimes expressed. PASC also finds, however, that there are significant features of Britain's political system that inhibit good government. In particular, PASC concludes there are too many ministers and a political culture too focused on responding to media demands. This has resulted in an excessive number of initiatives being launched and laws being introduced, which in turn reduces government's ability to decide and follow a clear and consistent direction. PASC urges government to place less emphasis on responding to short-term political pressures and instead urges a stronger focus on ensuring good basic administration in government. PASC's conclusions about government in Britain are based on five requirements that it has identified as prerequisites for good government: (1) Good people: the need to recruit and cultivate people with the right skills and abilities to undertake the work of government effectively; (2) Good process: appropriate structures, systems and procedures in place to develop and implement policies successfully; (3) Good accountability: adequate arrangements for holding both elected and appointed officials to account for their decisions and actions; (4) Good performance: effective performance assessment to identify how well government is meeting its objectives and where it could improve; (5) Good standards: high ethical standards exhibited by people in public life, underpinned by robust ethical regulation and strong ethical leadership.

The lack of coordination from the centre and strong political leadership will result in the failure of key policies like the 'Big Society' agenda. The Committee recommended a comprehensive change programme articulating clearly what the Government believes the Civil Services is for, how it must change and with a timetable of clear milestones. This change programme must also include proposals for the Civil Service to retain and develop the new skill sets required to meet the demands of the Big Society policy agenda, and address long-running concerns about the decline in specialist expertise in Whitehall, the failure to innovate and take risks, and the failure to work across departmental silos. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
