Book excerpt: "For nearly all infants, breastfeeding is the best source of infant nutrition and immunologic protection, and it provides remarkable health benefits to mothers as well. Babies who are breastfed are less likely to become overweight and obese. Many mothers in the United States want to breastfeed, and most try. And yet within only three months after giving birth, more than two-thirds of breastfeeding mothers have already begun using formula. By six months postpartum, more than half of mothers have given up on breastfeeding, and mothers who breastfeed one-yearolds or toddlers are a rarity in our society. October 2010 marked the 10th anniversary of the release of the HHS Blueprint for Action on Breastfeeding, in which former Surgeon General David Satcher, M.D., Ph. D., reiterated the commitment of previous Surgeons General to support breastfeeding as a public health goal. This was the first comprehensive framework for national action on breastfeeding. It was created through collaboration among representatives from medical, business, women's health, and advocacy groups as well as academic communities. The Blueprint provided specific action steps for the health care system, researchers, employers, and communities to better protect, promote, and support breastfeeding. I have issued this Call to Action because the time has come to set forth the important roles and responsibilities of clinicians, employers, communities, researchers, and government leaders and to urge us all to take on a commitment to enable mothers to meet their personal goals for breastfeeding. Mothers are acutely aware of and devoted to their responsibilities when it comes to feeding their children, but the responsibilities of others must be identified so that all mothers can obtain the information, help, and support they deserve when they breastfeed their infants. Identifying the support systems that are needed to help mothers meet their personal breastfeeding goals will allow them to stop feeling guilty and alone when problems with breastfeeding arise. All too often, mothers who wish to breastfeed encounter daunting challenges in moving through the health care system. Furthermore, there is often an incompatibility between employment and breastfeeding, but with help this is not impossible to overcome. Even so, because the barriers can seem insurmountable at times, many mothers stop breastfeeding. In addition, families are often unable to find the support they need in their communities to make breastfeeding work for them. From a societal perspective, many research questions related to breastfeeding remain unanswered, and for too long, breastfeeding has received insufficient national attention as a public health issue. This Call to Action describes in detail how different people and organizations can contribute to the health of mothers and their children. Rarely are we given the chance to make such a profound and lasting difference in the lives of so many. I am confident that this Call to Action will spark countless imaginative, effective, and mutually supportive endeavors that improve support for breastfeeding mothers and children in our nation."--Page v.

Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Book excerpt: Objective The purpose of the mixed methods study was to compare barriers and facilitators encountered by low-income African American women who engaged in different infant feeding practices. Methods Using the positive deviance approach and modified Integrated Model as theoretical frameworks, the research began with a qualitative study that informed a survey examining intention, skills, and environmental factors differentiating women's infant feeding practices. Results Twenty-eight and 190 low-income African American adult women participated in focus groups or interviews and a survey, respectively. In the qualitative study, positive deviants, who breastfed for at least four months, seemed to have stronger breastfeeding intentions, self-efficacy and breastfeeding support than other women. In the final multinomial multiple logistic regression, for a one unit increase in the PBC scale of the revised Breastfeeding Attrition Prediction Tool, there was a 50% (OR 1.5[1.3-1.7]) and an 18% (OR 1.18 [1.03-1.3]) increase in the odds of breastfeeding at least three months (positive deviance) compared to only formula-feeding and breastfeeding for less than three months, respectively. Women who did not smoke postpartum were 4.3 [1.5-12.3] and 5.6 [2.1-15.1] times as likely to be positive deviants; women who had C-sections were 3.6 [1.3-9.6] and 2.9 [1.0-7.8] times as likely to be positive deviants compared to only formula-feeding and breastfeeding for less than three months, respectively. Women who ranked WIC full breastfeeding packages as most valuable were 14.9 [4.8-45.5] and 16.1 [5.3-50.0] times as likely to be positive deviants compared to only formula-feeding and breastfeeding for less than three months, respectively. Discussion High breastfeeding self-efficacy was associated with positive deviance, although the qualitative study findings suggested that general self-efficacy may also influence breastfeeding success. The likelihood of not smoking being associated with longer duration of breastfeeding is consistent with previous research. The C-section results may reflect a longer length of hospital stay leading to increased mastery of breastfeeding prior to discharge. Analysis of participants' valuation of WIC infant feeding incentives led to the suggestion that WIC incentives be re-examined. Further exploration of these findings and subsequent interventions may lead to improved breastfeeding rates among low-income African American women.

Book excerpt: The American Academy of Pediatrics (AAP) and the World Health Organization (WHO) both recommend exclusive breastfeeding for the first six months of life. The AAP recommends continued breastfeeding with supplementation of solid foods for at least one year, while the WHO recommends continued breastfeeding with supplementation of solid foods for at least two years. Breastfeeding rates in the United States do not reflect these recommendations, or the breastfeeding goals of Healthy People 2020. Breastfeeding rates are especially low in low-income areas and among WIC recipients. This article details a proposal to implement a new protocol that will implement the WHO and UNICEF Ten Steps to Breastfeeding Success, and seek to earn a Joshua Tree, California hospital Baby-Friendly designation. This hospital serves a low-income population. Baby-Friendly designated hospitals have been shown to practice interventions that result in increased breastfeeding rates. Patient education is an important component to success with exclusive breastfeeding, and must be improved. Continued breastfeeding support is an important area, as well, and patients should have access to community resources and a local lactation consultant. In order for this to be successful, all LDRP staff must be uniformly educated on the benefits of breastfeeding, how to properly support exclusive breastfeeding, and what to teach new mothers. Included in this paper are a new staff breastfeeding protocol, incorporation of organizational change theory, an implementation plan, staff education plans and materials, evaluation tools, and plans to disseminate findings to the community, the stakeholders, and the general nursing population.

In Mexico, breastfeeding rates are one of the lowest of Latin America with 14.4% of exclusive breastfeeding (EBF) under six months. Most breastfeeding studies conducted in Mexico are based on the analysis of quantitative data from mothers. The objectives of this study were to evaluate the main obstacles to breastfeeding in low-income population in Tijuana, Mexico and to develop culturally appropriate messages to promote breastfeeding in low- income women from Tijuana, Mexico. We used qualitative methods to design messages based on a socio-ecological framework and social marketing principles. Our message development model had 4 phases: 1) Breastfeeding obstacles assessment: We selected four low-income communities with low accessibility to health care services in Tijuana to conduct focus groups and interviews with mothers, fathers and grandparents and interviews with key informants 2) Message content development: Messages were designed by a panel of public health professionals and lay women; 3) Message refining and tailoring: Message preferences for mothers were identified and segmentation of the target audience was conducted based on socio-demographic factors, perinatal factors and traditional or modern ideas; 4) Cognitive response testing for final messages. The present work includes five chapters. The first is a literature review that examines breastfeeding benefits, prevalence and situation in Mexican population, as well as how formative research using the socio-ecological framework can guide us to design culturally-sensitive breastfeeding promotion messages that can be used in community-based interventions. Second, third and fourth chapters discuss findings from the individual, group and social factors that influence breastfeeding practices in this population. The last chapter describes the model to design and pre-test breastfeeding promotion messages based on our previous formative research. A total of 169 participants were part of this study. We used the most relevant obstacles for mothers to develop 10 messages to promote breastfeeding in this population; 6 Individual: Enough milk, Pain, Aesthetics, Comfort, Baby behavior, Maintenance; 1 Group: Group Support (Family, Health Care Services, Worksite); and 3 Societal: Formula culture, Breastfeeding in Public and Cost. Obstacle ranking and message selection varied according to socio-demographic factors, perinatal experience and traditional or modern ideas. Migrant women recently arrived to Tijuana from the South ranked lack of breastfeeding support and culture as important obstacles to successful breastfeeding. Women with more traditional ideas, lower socio-economic status (SES) and education relied more on information from family members. Individual, group and societal factors interacted with each other. For example, individual factors such as perception of inadequate milk or comfort are influenced by group and societal factors such as the Western constructs that view the female body as weak, defective and untrustworthy; a culture with no exposure to breastfeeding and inadequate medical practices and insensitive health care providers. A comprehensive approach to breastfeeding recognizes that it is not merely an individual choice but that structural factors influence that choice, particularly for disadvantaged women. Awareness of the cultural, social and demographic influences on the population served provides a basis for the most appropriate advice to promote breastfeeding, based on the barriers faced. The 10 messages designed in this study can be used to develop educational material and create a social campaign to promote breastfeeding culture in this population. The messages and qualitative findings have direct applications for public programs that provide nutrition education to mothers in Mexico. Target audience for these messages should not be limited to mothers and also include family, health care providers, work environment and society as a whole.

Despite ongoing efforts to promote breastfeeding, current breastfeeding rates in the U.S. are still low, and socioeconomic and racial/ethnic disparities in breastfeeding practices have persisted. Improving breastfeeding practices among socially disadvantaged groups remains challenging. The goal of this dissertation is to improve breastfeeding rates and reduce racial/ethnic disparities in breastfeeding practices in the United States. Specifically, this dissertation aims to: (1) assess the influences of breastfeeding support from family, hospitals and workplaces on breastfeeding duration; (2) determine the extent to which racial/ethnic disparities in breastfeeding duration could be explained by breastfeeding support; and (3) estimate the population impact of multifaceted breastfeeding promotion interventions. Using data on approximately 4,000 mothers enrolled in the Special Supplemental Nutrition Program for Women, Infants and Children (WIC), from the triennial Los Angeles County WIC Survey (2014, 2017), this dissertation found that breastfeeding support from family, hospitals and workplaces was significantly associated with longer breastfeeding duration, and that Black mothers and English-speaking Latina mothers have significantly lower breastfeeding rates than White mothers, with the difference widening over increasing duration. Results from causal mediation analysis (specifically, g-computation) showed that lack of access to breastfeeding support from family, hospitals and workplaces accounted for approximately two-thirds of the difference in breastfeeding duration between White and Black mothers, and one-third of the difference between White and English-speaking Latina mothers. This dissertation also illustrated the use of agent-based modeling (ABM) for estimating the population impact of five selected breastfeeding promotion interventions (improving knowledge, implementing Baby-Friendly Hospital Initiative practices, providing breastfeeding counseling, strengthening family support, and fostering supportive workplace environments) implemented singly or in combination with each other. The ABM demonstrated that while improving knowledge and increasing the availability of Baby-Friendly Hospital Initiative practices improve breastfeeding initiation rates, breastfeeding counseling, family support and a supportive workplace environment are more effective in improving breastfeeding duration. Increasing the coverage of multiple interventions simultaneously had a synergistic effect on breastfeeding duration, with their effects being greater than the additive effects of increasing the coverage of these interventions singly.
