Les médecins comme les pharmaciens semblent ne pas être suffisamment formés et informés pour sensibiliser les patients au danger que peut représenter la conduite sous l'influence de certains médicaments. Si certains traitements peuvent avoir des effets néfastes sur l'aptitude à conduire un véhicule, certaines pathologies peuvent constituer en elles-mêmes une contre-indication à la conduite. C'est le cas notamment de la maladie d'Alzheimer qui soulève la question plus générale de la conduite chez la personne âgée puisqu'il n'existe pas actuellement, en France, de législation en faveur d'un contrôle médical régulier chez les personnes âgées. En revanche pour d'autres maladies telles que l'épilepsie, c'est l'efficacité du traitement qui conditionne l'aptitude à la conduite. Depuis 2005, la loi impose aux laboratoires pharmaceutiques d'apposer un pictogramme sur le conditionnement, sans obligation de taille ou d'emplacement. En tant qu'acteur de santé publique, le pharmacien doit participer à la sensibilisation de la population à ce sujet. Il doit également déclarer en pharmacovigilance tout effet cognitif ou comportemental qui lui serait rapporté suite à la prise d'un médicament. Il peut aussi être un interlocuteur privilégié auprès des patients suivant un traitement de substitution. Il a un rôle de prévention à jouer, tout particulièrement dans la délivrance de produits de médication officinale.

Book excerpt: RESUME : La consommation des médicaments psychotropes en France est l'une des plus importantes en Europe, et 4 % des accidents mortels de la route seraient imputables à cette exposition. Notre objectif est de vérifier l'impact des psychotropes sur les capacités de conduite automobile, chez des patients atteints de troubles psychiatriques stabilisés, et d'étudier l'intérêt du simulateur de conduite en situations d'expertise. L'étude porte sur 30 conducteurs, âgés de 18 à 64 ans, répartis en deux groupes (patients versus témoins). Les patients prennent au moins deux classes médicamenteuses de psychotropes différentes. La méthodologie utilise un appareil de simulation de conduite, doté d'un logiciel utilisé par des professionnels agréés. Nous avons coté les ordonnances, grâce à une grille de calculs conceptualisée en s'inspirant des niveaux de risques signalés pour chaque médicament. Les résultats objectivent des scores de conduite significativement meilleurs pour le groupe témoin. Néanmoins, environ la moitié des patients qui prennent un traitement équivalent à : un antipsychotique, un antidépresseur et une benzodiazépine, réussissent le test. Nous concluons que l'exposition aux médicaments psychotropes perturbe les performances de conduite, cependant cette altération n'apparaît pas formelle. Le simulateur constitue un médiateur innovant dans l'évaluation de conducteurs traités, compte tenu des insuffisances des tests psychotechniques, et des échelles des pictogrammes. La question du secret professionnel est envisagée.
