Book excerpt: The inappropriate use of antibiotics is a primary cause of the ongoing increase in drug resistance amongst pathogenic bacteria. The resulting decrease in the efficacy of antibiotics threatens our ability to combat infectious diseases. Rapid point-of-care tests to identify pathogens and better target the appropriate treatment could greatly improve the use of antibiotics. Yet there are few such tests currently available or being developed despite the rapid pace of medical innovation. Clearly something is inhibiting the much-needed development of new and more convenient diagnostic tools. This study delineates priorities for developing diagnostics to improve antibiotic prescription and use with the goal of managing and curbing the expansion of drug resistance. It calls for new approaches particularly in the provision of diagnostic devices and in doing so outlines some of the inadequacies in health science and policy initiatives that have led to the dearth of such devices. The authors make the case that there is a clear and urgent need for innovation not only in the technology of diagnosis but also in public policy and medical practice to support the availability and use of better diagnostic tools. This book explores the complexities of the diagnostics market from the perspective of both supply and demand unearthing interesting bottlenecks some obvious some more subtle. It calls for a multifaceted and broad policy response and an overhaul of current practice so that the growth of bacterial resistance can be stemmed.

Book excerpt: Human protozoan infections are an important target for development of new vaccines and drugs. No completely efficacious vaccines for human protozoan infections are available and in the case of malaria resistance to the most efficacious antimalarials has become a global challenge. In ocular toxoplasmosis complete eradication of the body is not possible, exposing patients to new reactivations. The need of treatment or vaccines for and of less toxic drugs for Leishmania are urgent tasks for protozoologists research community. New research strategies have appeared that enlarged the possibilities for treatment and vaccine development. Reverse vaccinology, bioinformatic search of second use drug candidates and ex vivo analysis have afforded new fields for development.

Available in PDF, EPUB and Kindle. Book excerpt: In this Perspective, the authors discuss the need for more sustainable and scalable ways of incentivising pharmaceutical industry innovation in response to infectious disease threats to public health. The authors consider incentives for innovation both in the current context of the COVID-19 crisis, and in the context of preventing other emerging or re-emerging infectious disease threats from becoming crises further down the line. The paper discusses the potential for innovative financial instruments, such as subscription-based models and bond-based approaches, to act as incentives. The authors also highlight the need to tackle a range of critical success factors beyond financial considerations. This includes tackling issues related to data access and the governance of data sharing, securing appropriate clinical trials infrastructure, ensuring suitable approval processes and procurement practices, and planning for manufacturing and supply chain capacity to mitigate against unintended consequences in affordability and access. Finally, the authors observe that the pharmaceutical industry will need to engage with society in ways that extend beyond producing a pill, diagnostic test or vaccine, in order to deliver on commitments made in response to any incentive systems that are put in place.

Book excerpt: After the United Nations adopted the 17 Sustainable Development Goals (SDGs) to "end poverty, protect the planet, and ensure prosperity for all," researchers and policy makers highlighted the importance of targeted investment in science, technology, and innovation (STI) to make tangible progress. Science, Technology, and Innovation for Sustainable Development Goals showcases the roles that STI solutions can play in meeting on-the-ground socio-economic and environmental challenges among domestic and international organizations concerned with the SDGs in three overlapping areas: agriculture, health, and environment/energy. Authors and researchers from 31 countries tackle both big-picture questions, such as scaling up the adoption and diffusion of new sustainable technologies, and specific, localized case studies, focusing on developing and middle-income countries and specific STI solutions and policies. Issues addressed include renewable energy, automated vehicles, vaccines, digital health, agricultural biotechnology, and precision agriculture. In bringing together diverse voices from both policy and academic spheres, this volume provides practical and relevant insights and advice to support policy makers and managers seeking to enhance the roles of STI in sustainable development.
