Book excerpt: The Office of the Assistant Secretary for Planning and Evaluation (ASPE) in the U.S. Department of Health and Human Services funded the Improving Employment Outcomes for People with Psychiatric Disorders and Other Disabilities Project to identify effective programs that help individuals with psychiatric disorders find and retain employment. A second goal of the project was to explore how these programs can be funded through the Affordable Care Act (ACA) and other sources. ASPE was particularly interested in knowing what supports will assist the following subgroups of people with psychiatric disorders: Individuals who are now or who are expected to be long-term clients of mental health services and who are in the process of applying for disability benefits. Individuals at risk of losing employment due to mental illness. Individuals, such as transition-age youth (TAY), who are experiencing an initialepisode of psychosis and require early-intervention (EI) services. This project aimed to find answers to the following overarching questions: What services are most effective at helping people in the previously describedthree subgroups find and keep employment? What are the work-support needs of and services currently available toindividuals with other disabilities? What can income and service-use trajectoriesof participants in particular programs tell us about service needs and programeffectiveness? What policies and funding can be adopted in a post-ACA environment toovercome employment barriers for people with psychiatric disorders and otherdisabilities? We conducted two targeted literature reviews: (1) employment programs and outcomes for people with psychiatric disorders (O'Day et al. 2013); and (2) employment programs and outcomes for people with other disabilities (Martin et al. 2013). We also analyzed data from the Survey of Income and Program Participation to examine service-use trajectories of vulnerable populations who might be expected to apply for Social Security Disability Insurance (SSDI) benefits. We also examined literature and policy documents that outlined funding options for employment services for people with psychiatric disorders and other disabilities. We highlight our findings in this summary.

The pressure created by these powerful legislative, societal, and economic forces has not been matched by the state of evidence-based practices in the field of employment retention and job accommodation in mental health. Current research evidence is fragmented, limited in scope, difficult to access, and adversely affected by the traditional divide between the fields of psychiatry and psychology on one hand and interdisciplinary employment research and practices on the other. As a result, policy makers, employers, disability compensation systems, and rehabilitation and disability management professionals have been left without a critical "how to" evidence-informed toolbox for occupational practices to accommodate and retain persons with mental health disabilities in the workplace. Currently, no single source of knowledge and research evidence exists in the field that would guide best practices. Yet the need for workplace accommodations for persons with mental health disabilities has been growing and, based on epidemiological trends, is anticipated to grow even more in the future. These trends leave physicians, psychologists, occupational therapists, vocational rehabilitation professionals, disability managers, human resource professionals, and policy makers poorly prepared to face the challenge of integrating and maintaining persons with mental health disabilities in the workplace. The aim of the Handbook is to close the gap between the needs of the professionals and networks that work with or study persons with mental heath disorders in an employment context and the actual knowledge base in the field. The Handbook will be written in language that can easily be understood by readers representing a multitude of disciplines and research paradigms spanning the mental health, rehabilitation, and employment fields of inquiry. The Handbook will contribute an integration of the best quantitative and qualitative research in the field, together with experts’ consensus, regarding effective work retention and accommodation strategies and practices in mental health. The book will consist of five major sections, divided into chapters written by recognized experts in these areas.

The wars in Iraq and Afghanistan have made it urgently necessary to address these issues once again as the nation faces “the largest wave of returning veterans with disabilities in recent history.” One in four veterans of these conflicts reports a service-connected disability, and unemployment among the youngest subset of veterans is particularly high. Veterans with disabilities need quality programs to help them get on a path to work and reintegrate into their communities. But there is limited evidence about what interventions can effectively help them do so. Past research suggests that symptoms and impairments explain only a part of what prevents people with disabilities from working, and that people with disabilities' own beliefs and attitudes about their conditions often keep them from gainful employment. Similarly, researchers at the U.S. Department of Veterans Affairs (VA) have suggested in personal interviews that disabled veterans' attitudes and beliefs about disability present at least as big a barrier to their ability to return to work as their actual physical or mental conditions. Drawing on its experience in disability, behavioral, and employment research, MDRC began testing the Progressive Goal Attainment Program (PGAP) for Veterans in 2012, in collaboration with the VA Connecticut Healthcare System. PGAP is a behavioral intervention for people struggling with a wide range of physical and mental health conditions. The program complements clinical services for the treatment of disabilities by specifically targeting psychological and social behaviors that contribute to pain, disability, and inactivity. The goal is to help those with disabilities resume daily activities and get them on a path to work. The PGAP demonstration in the VA Connecticut Healthcare System was designed to explore how feasible it is to implement the program in a veteran service setting. In the coming year MDRC will also test PGAP for Veterans in several locations in Houston, Texas, including the VA hospital and two local community providers.
