Book excerpt: Introduction : Point-of-care testing (POCT) systems enabled a wide range of biological tests to be done quickly at patient bed-side, contrary to central laboratory testing (Lab) which required an incompressible time for sample delivery, analysis and results retrieval. POCT blood gas analyzer in the Intensive Care Units (ICU) can also give results for blood concentration of sodium (Na), potassium (K) and hemoglobin (Hb) . The aims of our study were to: (1) define the optimal pre-analytical conditions suitable with the ICU organization for reliable POCT analysis ; (2) evaluate the impact of Na, K and Hb obtain by POCT on the therapeutic decision-making process. Materials and methods : A three-step prospective study in three care units using POCT. Step-1 : The concordance of POCT and Lab results was obtained in the cardiac surgery operating room (OR), where pre-analytical phase was considered as ideal (control situation). The POCT-Lab concordance was evaluated and compare with the OR in two ICUs: a 37-bed polyvalent ICU and a specialized 10-bed ICU. Step-2 : A multidisciplinary quality improvement program was initiated in the 37-bed ICU because of the poor reliability of Hb. The improvements of the pre-analytical process were assessed by a new POCT-Lab concordance evaluation. Step-3 : The impact of POCT on patient care was evaluated by prospectively collecting medical decisions based on POCT results. The concordance of results was assessed by the Bland-Altman plots. Results : Step-1 (n=491 samples) : in the OR condition the POCT results are reliable with Lab. (Bias: Na = 0 [-3 ; 3] mmol/L, K = 0 [-0,1 ; 0,4] mmol/L, Hb = -3 [-6 ; 1]g/L) and obtained immediately (2 [2 ; 2] min). Those results can be extended to the ICUs for Na and K, but not for Hb analysis in the 37-bed ICU (Bias Hb = -19 [-64 ; 27] g/L, POCT lapsed time = 24 [13 ; 38] min). Step-2 (n=278) : the pre-analytical process was improved by the quality improvement program with a substantial enhancing of the POCT-Lab concordance (Bias Hb= -2 [-10 ; 5] g/L) and results obtention time (15 [5 ; 29] min). Step-3 (n=129) : in a three-month period in the 37-bed ICU, 116 medical decisions were taken upon POCT analysis, about half of which were emergency decision. The median time between blood sampling to decision were 15 [7 ; 30] min. Conclusion : A strict management of the preanalytical conditions is an essential prerequisite for the biological POCT reliability. Thus, POCT provide a reliable and timely information bedside, which is a valuable asset for urgent therapeutic care as needed for ICU patients.
