Book excerpt: The Army is transforming into a modular expeditionary force while at war. Our uniformed medical professionals are doing wonderful work of saving lives and helping rehabilitate our most valuable asset - Soldiers. However, our ability to do this in the future may be in jeopardy if we do not transform as swiftly as the warfighters. The U.S. military environment is swiftly moving toward joint interoperability and joint interdependability. The three service medical departments of the Army, Navy, and Air Force collaborate on issues, but it is a long process centered on their specific service interests. This limited collaboration must be changed to ensure the Soldiers on future battlefields are assured lifesaving care. Presently, each service independently constructs a force to provide medical support. It is in the Army's best interest to structure its assets with the other services for interoperability and interdependence. The Joint Medical Command will reduce redundancies, conserve resources, and implement efficient collaboration among the services. This jointness has been proposed many times since World War II. The time has come to make this commitment for the future of medical health services in the DoD, thereby ensuring superior medical support to the next generation of Soldiers.

In an era of budgetary constraints, it is apparent that the time has come to look at just how significant the formulation of a Joint Medical Command would be to the operational aspect of warfare. There have been many recent episodes of jointness in medical support on the battlefield, but nothing in doctrine supports the premise, and current service parochialisms often stand in the way of furthering this concept. This paper will look at advantages and disadvantages of joint medical support using key operational functions and the six joint health service support principles as a framework for analysis and conclusion. Many have written about benefits to peacetime healthcare, yet little exists in ways of analyzing this concept with respect to the warfighter from a joint support perspective.

For decades the Department of Defense (DOD) has received pressure to consolidate medical departments and therefore, DOD reciprocates that challenge to the uniformed services to create a single medical command. The Army, Navy and Air Force have always justified separate medical branches, by staunchly stating that war-time requirements demand unique medical skill sets for Soldiers, Seamen and Airmen. However, current combat environments have blurred the lines between distinctions of medical care. Medics and physicians from all military services work side by side to save lives and return warriors to duty. The Military Health System (MHS) is responsible for health care delivery in military hospitals and routinely use joint services health care providers. In 2005, Base Realignment and Closure (BRAC) established that, all military medical training sites for enlisted members will consolidate at Fort Sam Houston, Texas. Now is the perfect time to design, create, resource and mandate real JOINT medical training for our enlisted members. Military medical education can embrace process efficiencies to establish effective and realistic joint medical training.

The readiness mission provides medical services and support to the armed forces during military operations. The benefits mission provides health care to over 9 million eligible beneficiaries, including active duty personnel, retirees, and dependents worldwide. Past GAO and other reports have recommended changes to the military health system (MHS) structure. GAO was asked to (1) describe the options for structuring a unified medical command recommended in recent studies by DOD and other organizations and (2) assess the extent to which DOD has identified the potential impact these options would have on the current MHS. GAO analyzed studies and reports prepared by DOD's Joint/Unified Medical Command Working Group, the Defense Business Board, and the Center for Naval Analyses, and interviewed department officials. What GAO Recommends: GAO is recommending that DOD address the expected benefits, costs, and risks for implementing the fourth option and provide Congress the results of its assessment. In commenting on a draft of this report, DOD concurred with GAO's recommendations.

Book excerpt: Advances in trauma care have accelerated over the past decade, spurred by the significant burden of injury from the wars in Afghanistan and Iraq. Between 2005 and 2013, the case fatality rate for United States service members injured in Afghanistan decreased by nearly 50 percent, despite an increase in the severity of injury among U.S. troops during the same period of time. But as the war in Afghanistan ends, knowledge and advances in trauma care developed by the Department of Defense (DoD) over the past decade from experiences in Afghanistan and Iraq may be lost. This would have implications for the quality of trauma care both within the DoD and in the civilian setting, where adoption of military advances in trauma care has become increasingly common and necessary to improve the response to multiple civilian casualty events. Intentional steps to codify and harvest the lessons learned within the military's trauma system are needed to ensure a ready military medical force for future combat and to prevent death from survivable injuries in both military and civilian systems. This will require partnership across military and civilian sectors and a sustained commitment from trauma system leaders at all levels to assure that the necessary knowledge and tools are not lost. A National Trauma Care System defines the components of a learning health system necessary to enable continued improvement in trauma care in both the civilian and the military sectors. This report provides recommendations to ensure that lessons learned over the past decade from the military's experiences in Afghanistan and Iraq are sustained and built upon for future combat operations and translated into the U.S. civilian system.
