Book excerpt: Le diabète est une maladie chronique caractérisée par la présence d'un excès de glucose dans le sang appelé hyperglycémie. On distingue 2 types de diabète : le diabète de type 1, dit insulinodépendant et appelé diabète juvénile, et le diabète de type 2 dit non insulinodépendant et considéré comme diabète de l'adulte. En France, en 2016, on comptait plus de 3,3 millions de personnes traitées pharmacologiquement pour un diabète tous types confondus, soit 5% de la population. Le diabète de type 1 est l'une des maladies chroniques les plus fréquentes chez l'enfant. Il représente environ 10% des cas de diabètes en France et dans le monde. La moitié des cas se déclare avant l'âge de 20 ans. Actuellement en France, l'incidence du diabète de type 1 est d'environ 15 cas pour 100 000 enfants de moins de 15 ans. C'est ainsi qu'entre 2013 et 2015, 6 424 enfants ont été identifiés comme nouvellement atteints de diabète de type 1. Les prévalences chez les jeunes atteints de diabète de type 1 sont donc élevées, soulignant la nécessité de mettre en place des dispositifs de prévention. Les outils d'information existants ne sont pourtant pas adaptés aux enfants et aux adolescents. Afin de mettre en place un outil d'information adapté pouvant être intégré à leur parcours d'éducation thérapeutique, cette thèse avait pour objectif d'évaluer les connaissances des adolescents diabétiques de type 1 et de leurs parents sur l'impact et la gestion des risques bucco-dentaires liés à cette maladie. Enfin, une fiche d'information pédagogique adaptée à ce public a donc été produite et distribuée aux participants à l'étude.

Cependant, ses conséquences sur la santé bucco-dentaire sont peu évaluées en littérature. C’est pourquoi, l’objectif de notre étude est d’évalue l’état de santé bucco-dentaire chez l’enfant ayant un diabète de type 1. Il s’agit d’une étude dans un groupe de 40 enfants au cours d’un séjour en maison sanitaire. Les données médicales, bucco-dentaires (CAO, IG, OHIS), ainsi que le débit, le pH, le pouvoir tampon salivaire et la proportion de streptococcus mutans, et de Lactobacillus sp. (Cario-Analyse®) ont été recueillis. Dans notre échantillon, l’âge moyen est 11 ans (ET=1,34), l’HbA1c moyenne est de 8% (ET=0,97), l’ancienneté moyenne du diabète est de 6 ans (ET=2,75). L’état bucco-dentaire de notre échantillon est GLOBALEMENT BON : l’alimentation est peu cariogène et les habitudes d’hygiène bucco-dentaire sont correctes. Au niveau de la maladie carieuse, le CAO moyen est de 0 ,8 (ET=1,38) et 80% des enfants ont un indice microbiologique total important. Au niveau des gencives, l’indice gingival moyen est de 0,5 (ET=0,54). Cependant, ces résultats sont à tempérer, les enfants ayant un HbA1c ≥7,5 présentent significativement plus de gingivites, de lactobacilles et de flore totale. Les enfants ayant un diabète installé depuis au moins 5 ans, présentent une diminution significative de leur débit salivaire. Ces résultats semblent indiquer un état bucco-dentaire moins bon dans ces sous populations. Seul 17,5% des enfants ont un niveau de connaissances « acceptable » sur les liens de santé bucco-dentaire/diabète. Ils ont significativement moins d’inflammation gingivale avec un meilleur niveau d’hygiène orale. Notre étude met donc en lumière le rôle du chirurgien-dentiste dans la prise en charge globale médicale des enfants diabétiques. Le praticien doit éduquer sur les complications orales du diabète et promouvoir de bons comportements en matière de santé bucco-dentaire

On compte déjà plus de 2,7 millions de patients diabétiques. La dégradation de l'état parodontal est deux fois plus importante dans cette population. Pourtant seulement 33% de ces patients consultent annuellement leur chirurgien-dentiste comme préconisé par l'HAS. Les médecins généralistes prennent-ils en compte les soins bucco-dentaires dans la prise en charge de leurs patients diabétiques de type 2? Par une étude prospective quantitative et descriptive portant sur les médecins généralistes installés dans le département de la Vienne invités à répondre à un questionnaire anonyme auto-administré. Nous avons répondu à notre objectif principal qui était de démontrer que les médecins généralistes ne vérifient pas annuellement que le suivi bucco dentaire est effectué chez leurs patients diabétiques de type 2. A 82%, les médecins généralistes ont répondu ne pas vérifier au moins annuellement que la consultation chez le chirurgien dentiste soit bien réalisée. Plus de 60% d'entre eux n'informent rarement ou jamais leurs patients diabétiques sur les complications bucco-dentaires liées à leur pathologie répondant ainsi à notre objectif secondaire. La raison principale de cette lacune de surveillance est leur manque de connaissances sur ce sujet. Il n'existe aucune différence significative dans les sous groupes sexes, milieux d'exercice et âge concernant nos hypothèses principale et secondaire. Notre étude a sensibilisé les médecins répondeurs et à la lumière de leur commentaire une formation continue aurait probablement un impact rapide sur les pratiques.
