Book excerpt: In a profession where the nursing shortage is well known and inevitable despite of vacancy reprieves, understanding determinants of job satisfaction may now be an essential component of retention strategies. Many factors are causing the health care industry to see the potential significant implications of the nursing shortage. Collaboratively creating an environment of empowerment, autonomy, and lower stress, nurses are content and satisfied. In turn, satisfied nurses can continue to deliver safe and quality patient care. The purpose of this study was to understand the relationships among the determinants of job satisfaction. Nurse-manager collaboration, empowerment, autonomy, and subjected job stress were the identified independent variables and job satisfaction was the dependent variable. The following instruments were used to measure the perspective study variables: Collaborative Behavior Scale-B (CBS-B); Conditions of Work Effectiveness Questionnaire-II (CWEQ-II); a subscale from the Index of Work Satisfaction; Subjective Job Stress; and Michigan Organizational Assessment Scale. The hypotheses proposed for the study included: there will be positive relationship between nurse-manager collaboration and job satisfaction; there will be a positive relationship between empowerment and job satisfaction; there will be positive relationship between autonomy and job satisfaction; there will be negative relationships between subjective job stress and job satisfaction; there will be significant relationships among nurse-manager collaboration, empowerment, autonomy, and subjective job stress. The research study was a descriptive correlation study design. A convenience, snowball sample methodology was used. Once correlation was identified, the study variables were further analyzed using multiple regression and dominance analysis. There were significant correlations among the study variables. Empowerment had the strongest correlation to job satisfaction (r = 0.71, p [less than or equal to] 0.05). It explained 50% of the variance in job satisfaction ([lower case beta] = 0.76, p [less than] 0.001). Dominance analysis revealed empowerment is the most importance predicator of job satisfaction (F = 46.57, df = 1, 70; R2 = 0.213). The data supported all hypotheses. A post hoc analysis was conducted to determine if any of the study variables were correlated with nurses' perceptions of unit commitment to patient safety, quality of work, and nurse safety. These three variables were measured with one question on the demographic requesting the respondent to rate each of these variables on a 10-point scale. Several statistically significant correlations were identified. Perception of NMC was highly and significantly correlated to perception of unit's commitment to nurse safety. This validates the desire that safety in nursing practice is affected by how effective the collaboration is between the nurse and the nurse manager. The perception of autonomy was significantly correlated to unit's commitment to patient safety. This may imply that the more the nurse believes that the higher their autonomy (scopespecific decision making power) patient safety increases. These results indicate the relationship between nurses' perceptions of NMC, empowerment, autonomy, SJS, and JS with the nurses' perceptions of the units' commitment to patient safety, quality of work, and nurse safety. This is the first time that such findings have been reported.

Book excerpt: "This study examined the intercorrelations among nursing home healthcare providers' stress, job distress, job satisfaction, and mood, as well as the relationships between these factors and patient outcomes. Additionally, this study investigated the impact Butler University's Music First! program had on nurse job perceptions. A total of 63 nurses from 9 long-term care facilities completed questionnaires asking about stress, job distress, and global job satisfaction. Nurses' depression, anxiety and hostility were also assessed with a 15-item Profile of Mood States questionnaire. Of these 63 nurses, 20 completed the questionnaires at three different time points during the implementation of the Music First! intervention. Patient outcomes included standardized assessments of agitation, psychiatric symptoms, cognitive functioning and depression, as well as documented instances of patient falls and reportable behaviors. Intercorrelations among nurse stress, job distress, job satisfaction, and affect ranged from r = -0.565 to r = 0.627. All correlations except one reached statistical significance. Similarly, correlations between patient outcomes and nurses' job-related self-reports documented strong relationships between nurses' affective responses to their jobs and patient outcomes. For example, the patients of nurses reporting greater stress at work were more agitated (r = 0.335) and exhibited more severe dementia (SLUMS r = -0.291), more severe psychiatric symptoms (r = 0.293), and more frequent reportable behaviors (r = 0.436) than patients of nurses reporting less stress. Finally, the Music First! program was associated with a near significant decrease in nurses' job-related stress (p = 0.054). Although these results are correlational and cannot determine the direction of these relationships, these findings do suggest that improving nurses' feelings about their jobs, improving patient behaviors or implementing programs designed to improve patients' quality of life can have a positive impact on the nursing home environment"--Leaves 2-3.

Book excerpt: The focus of this project is to determine whether leadership and empowerment programs implemented within Hayward Area Memorial Hospital (HAMH) would be effective at increasing job satisfaction and decreasing stress levels; while providing nurse retention. These programs would be guided by Newman's System Model, in which attempting to keep that stable line of defense, reducing levels of stress that weakens the line of defense resulting in unhappy employees, high expenses to the health care organization, and patient safety concerns. Stress is everywhere, within everyone, and every profession. More importantly in how stress is handled and the effects of stress has in regards to job satisfaction within nursing and how it may further affect the patient care. Stress affects everyone differently and severity can also vary greatly. Ohue, Moriyama, and Nakaya (2011) and Wright (2014) expresses how stress is exceeding many nurses capacity, resulting in medical problems and high levels of burnout. This often results in nurses leaving their current jobs or even the profession all together. Various studies have shown nurses feel they would benefit from higher levels of leadership and empowerment. They often feel unsupported, under-valued, lack of collaboration amongst other members of the health care team, lack of autonomy, and poor communication. With the provision of leadership and empowerment programs, it can enhance working relations, promote nursing engagement, incentives, amongst many other great opportunities (Van Bogaert, Clarke, Roelant, Meulemans, and Van de Heyning, 2010). Providing leadership and empowerment training to management, will then enhance the attitudes and productivity of nursing employees, while improving patient care and decreasing high turnover expenses to the organization. Nursing employees would participate in several different surveys to identify levels of stress, burnout, and the working environment. This would be done initially, in six months, and then annually. Also the provision of stress reduction classes would be provided on a monthly basis to assist with stress reduction and establishment of different coping techniques to assist with stress. The data collected from the surveys would be analyzed by a committee within HAMH utilizing appropriate statistical analysis to determine if the programs implemented are effective in reducing stress, increasing job satisfaction, and improving nurse retention.

The work environment impacts nurses’ perception and loyalty to organizations based on satisfaction versus dissatisfaction, including quality patient care. The purpose of this study was to explore work environmental factors that affect nurse job satisfaction in a hospital setting. Marilyn Ray’s Theory of Bureaucratic Caring was the theoretical framework for this study. The research utilized the McCloskey/Mueller Satisfaction Scale (MMSS) to examine factors in an acute care hospital associated with nurse job satisfaction. The study invited 130 nurses from a hospital in rural South Carolina to participate electronically via a link provided through their work email accounts. The sample included a total of 23 participants that completed the survey, including registered and licensed practical nurses (RNs & LPNs). The study revealed no significant work environmental factors that affect nurse job satisfaction. Results of the study indicated that nurses were neither satisfied nor dissatisfied with their current jobs (M=3.35). Participants rated the factors that affect job satisfaction in the following order from very satisfied to least: scheduling, co-workers, interaction opportunities, extrinsic rewards, praise and recognition, balance and family, professional opportunities, and control and responsibility.
