Book excerpt: The purpose of this study was to assess the impact of using an online personal health record (PHR) on clinical outcomes of patients with type 2 diabetes. This study used an intervention mixed methods approach with a quantitative randomized, controlled trial combined with a qualitative exploration of how patients used the PHR. A total of 140 participants with type 2 diabetes were randomized to either receive a PHR and training to use the PHR, or to a control group who received usual care. Both clinical and social cognitive outcomes were measured and compared at baseline and follow-up, three to six months after baseline. A linear regression was also used to model the relationship between PHR use, social cognitive factors, and hemoglobin A1c (HbA1c). The quantitative analysis revealed no differences in diabetes clinical outcomes (HbA1c or blood glucose) from baseline to follow-up between either the control and intervention groups, or within the intervention group, between individuals who continued to use the PHR after baseline (PHR users) and individuals who ceased use after baseline (PHR non-users). Additionally, the linear regression revealed no significant relationships between PHR use, social cognitive factors, and HbA1c. In-depth interviews were conducted with all members of the intervention group to explore how patients used the PHR in their diabetes self-management. Themes that emerged from the qualitative analysis were used to explain quantitative findings and identify continued research gaps that must be addressed in future research. Nine themes emerged from the qualitative analysis, seven of which expressed barriers that helped to explain the lack of quantitative difference between the groups. The additional two themes identified positive outcomes from PHR use that support hypothesized benefits from PHR use. Future research should focus on the integration of the PHR into patients’ regular diabetes self-care routine and identifying which features of the PHR have the potential to lead to clinical improvement.

It contains multimedia resources aimed at improving self-management, including information leaflets, interactive educational tools and videos. MDMW also offers access to clinical data via a novel electronic personal health record (ePHR), which sources data from primary care, secondary care, specialist screening services and laboratory systems. This merged record provides a more complete overview of diabetes than is available from any single data source. Patients use MDMW to share information with healthcare teams, through automatic data upload, secure messaging and online forums, further enhancing communication. Patients also set and record goals and receive automated highly-tailored advice and guidance based on the current status of their results.Aim: We aimed to evaluate user experience of the records access service and assess impact of MDMW on routinely collected process outcomes.Methods: In January 2015, an online evaluation survey was emailed to 3,979 active users of the MDMW ePHR to assess experiences and perceived benefits. Survey Monkey was used to capture results for each unique respondent, using a series of open and closed questions to detail the impact on patient satisfaction and ability to self-manage. In April 2017, clinical outcomes were assessed using time-series analysis by comparing the HbA1c recordings of the intervention cohort (active users) with those in the inactive background patient population, matched by age, duration of diabetes, socioeconomic status and gender.Results: 1,095 (27.5%) active users completed the user survey. Patients report that MDMW improves their knowledge of diabetes (90.3%) and their motivation to manage it better (89.3%). It allows them to make better use of consultation time (89.6%) and means that they do not need to keep paper records (84.4%) or phone their doctor for results (85.2%). Users found graphs helpful to monitor changes (95.9%) and 83.5% said the system helped them meet their diabetes goals. Additional results: 92.1% - system contained all expected features; 87% - reminded users of information discussed at consultations; 85.7% - system was up-to-date; 89.1% - system was easy to use; 94.2% - supporting information helped understanding of results; 93.7% - helped find information tailored to own diabetes; 96.7% - confident the system was secure; 88.2% - helped manage diabetes better; 89.8% - helps set own goals; 96.4% - system will significantly improve diabetes self-care in Scotland. Book excerpt: A Retrospective Study Comparing Shared Medical Appointments with Usual Health Care on Clinical Outcomes and Quality Measures in Veterans with Type 2 DiabetesAbstract by MARIANNE DeMEO HARRISThe Center for Disease Control (CDC) reports that chronic disease accounts for more than 75% of the nation's $2 trillion in medical care costs, and the direct and indirect costs of a chronic disease such as diabetes alone is estimated at $174 billion dollars a year. Diabetes and heart disease frequently occur reciprocally because over time, elevated blood sugar levels lead to microvascular alterations in the intimal layer of the blood vessels. Despite the serious risks of these two medical conditions, our current health care system has yet to develop effective strategies for managing diabetes, and minimizing heart disease risk. One model of care that shows promise, however, is shared medical appointments (SMA), also known as group medical visits where a multidisciplinary team of health professionals provide health care to a cohort of patients at the same time in a supportive, educational, and interactive environment. There are a limited number of studies on utilizing shared medical appointments to manage diabetes and heart disease, and most show mixed results. Therefore, the author proposed to continue to build evidence on this topic and promulgates the following hypothesis: Compared to veterans who receive usual care (UC), (n=617) veterans with type 2 diabetes who utilize shared medical appointments (n=371) will have significantly better clinical outcomes, and higher levels of provider adherence to accepted VA Department of Defense (DoD) diabetes clinical practice guidelines. This 3-year retrospective two-group observational study utilized an existing Veterans Administration (VA) VISN 10 database warehouse, and Computerized Patient Record System (CPRS). Emergency room data was also abstracted retrospectively over the same 3-year study period. Additional co-morbidities that were tracked included hypertension, dyslipidemia, coronary artery disease, and obesity, as well as demographic variables such as age, sex, marital status, and gender. Three moderator variables were tracked in the study: the presence of a mental health diagnosis, number of health care visits (UC and SMA) and participation in other VA self-management programs over the three-year study period. The study variables were analyzed using t-tests, X2, repeated measures ANOVA, and multiple regression to reveal the relationships among the variables. The clinical outcome variables of HbA1c, lipid panel and blood pressure were not significantly different in the SMA cohort over UC during the three year study period; however, several clinical practice guidelines were met annually for the SMA veterans that included having an angiotension-converting enzyme inhibitor/angiotension-receptor blockers and aspirin prescribed and having annual ophthalmology and podiatry exams. This research project enhances our knowledge on how using SMAs may produce improved provider adherence to diabetes care quality standards in veterans with type 2 diabetes who are at substantial risk for cardiovascular disease
