Book excerpt: The purpose of this study was to examine the impact socioeconomic status has on the pain scores of patients with rheumatoid arthritis. Data was collected from older adults who participated in an land-based or aquatic exercise program at the New Britain-Berlin Youth Men's Christian Association (N = 13). The participants were asked to complete two self-reporting questionnaires: the Rheumatoid Arthritis Pain Scale (RAPS) and a socioeconomic status survey. The RAPS was utilized to analyze the pain scale total score as well as the overall pain score while the socioeconomic status survey addressed the following domains: education level, amount of people providing for, amount of cars in the household and annual income. The RAPS was distributed prior to a four week exercise program, midway, and at the conclusion of the exercise programs. A correlational analysis revealed there was no significant difference between socioeconomic status and the pain scores of the participants. Additionally, there was no significant different found between pain scores from participants who completed the land exercise class compared to the aquatics program.

Book excerpt: Arthritis is a leading cause of disability for adults in the United States making the chronically progressive condition one of the most common causes of chronic pain. The purpose of this study is to examine the effects of frequency and duration in a community-based Arthritis Foundation Aquatic Program (AFAP) on the pain level of older adults with arthritis. The study population consisted of 27 adults with arthritis currently attending AFAP classes at the local wellness center. Data were collected using a facility developed self-report form. Results indicated a significant decrease in pain from Time 1 to Time 2. However, results demonstrated that there were no significant associations between the frequency or/and duration of their involvement in the aquatic exercise program and this improvement in arthritis pain. This research study suggests that AFAP is assisting in the treatment of the symptoms and pain management in older adults with arthritis.

The 2010 Patient Protection and Affordable Care Act required the Department of Health and Human Services (HHS) to enlist the Institute of Medicine (IOM) in examining pain as a public health problem. In this report, the IOM offers a blueprint for action in transforming prevention, care, education, and research, with the goal of providing relief for people with pain in America. To reach the vast multitude of people with various types of pain, the nation must adopt a population-level prevention and management strategy. The IOM recommends that HHS develop a comprehensive plan with specific goals, actions, and timeframes. Better data are needed to help shape efforts, especially on the groups of people currently underdiagnosed and undertreated, and the IOM encourages federal and state agencies and private organizations to accelerate the collection of data on pain incidence, prevalence, and treatments. Because pain varies from patient to patient, healthcare providers should increasingly aim at tailoring pain care to each person's experience, and self-management of pain should be promoted. In addition, because there are major gaps in knowledge about pain across health care and society alike, the IOM recommends that federal agencies and other stakeholders redesign education programs to bridge these gaps. Pain is a major driver for visits to physicians, a major reason for taking medications, a major cause of disability, and a key factor in quality of life and productivity. Given the burden of pain in human lives, dollars, and social consequences, relieving pain should be a national priority.

Book excerpt: This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.
