Book excerpt: Patients with insulin-dependent diabetes mellitus (IDDM) must adhere to a complex treatment regimen. Adherence is often poor during adolescence, when the regimen behaviours may interact negatively with many of the developmental tasks. In a randomized controlled outcome study, 15 adolescents and their families participated in a family-based intervention for improving adherence to the diabetic regimen. Fourteen comparison group adolescents with IDDM spent a comparable amount of time learning stress management techniques. Adolescents in both groups attended a diabetes management review session with the clinic nurse. Inclusion criteria included mean glycosylated hemoglobin (HbAlc) levels $>$9.0% over the previous nine months. At a 6-month follow-up, adolescents in the family intervention group were testing blood glucose levels more regularly (p $$1% (0% vs. 29%, $\chi\sp2$ = 6.32, p =.05). Methodological issues were discussed.

Book excerpt: Person-oriented and variable-oriented analyses were conducted to investigate sociodemographic differences in biological, psychosocial and disease care factors in youth with type 1 diabetes. Additionally, diabetes knowledge was evaluated as a potential mediator of SES effects on HbA1c and disease care. The sample included 349 youth, age 9-17 years (79.9% Caucasian, 71.3% lived with two biological parents, M SES = 46.24). Person-oriented t-tests confirmed commonly reported ethnic differences in HbA1c and disease care behaviors. However, variable-oriented analyses controlling for confounding sociodemographic influences showed most disease care effects attributed to ethnicity were better explained by SES. While diabetes knowledge was not a significant mediator of meal composition, it appeared to suppress the effect of social class on carbohydrate and fat consumption, such that more diabetes knowledge minimized the negative effect of lower SES on optimal carbohydrate and fat consumption. Results may inform future interventions for youth at risk of poor metabolic control.

Book excerpt: Objective: The purpose of the study was to test interactive models that examined the moderating effects of diabetes-specific family conflict on the relationship between parental involvement in type 1 diabetes mellitus (T1DM) care and illness-specific psychosocial outcomes for youth in poor metabolic control and their parents. Methods: Youth (N=72) ages 10-18 years with poorly controlled diabetes (A1c>8.5%) and one parent/caregiver completed measures assessing parental involvement in T1DM care, family conflict, youth diabetes-specific quality of life, and pediatric parenting stress. Results: Low shared treatment responsibility between parent and youth for T1DM treatment tasks and high levels of diabetes-specific family conflict interacted to predict poorer youth and parent psychosocial outcomes. Conclusions: Interventions targeting both family sharing of treatment responsibility and family conflict about diabetes-related issues may foster improvements in youth diabetes-specific QOL and pediatric parenting stress.

Book excerpt: The purpose of this study was to examine the relationship between family functioning, health-related quality of life, and metabolic control in order to identify areas for intervention that can improve medical and psychosocial outcomes for children and adolescents with type 1 diabetes. Children (N = 45) ages 8-17, both male and female, with type 1 diabetes, along with one caregiver (parent or legal guardian) (N = 45) were asked to complete the Pediatric Quality of Life Inventory (PedsQL) generic and diabetes-specific form to assess health-related quality of life along with the Diabetes Family Behavior Checklist and the Family Relationship Index of the Family Environment Scale to assess family functioning. Recent Hemoglobin A1c (A1c) was obtained from the physician at their visit or by parent report to assess metabolic control. Results indicated a significant relationship with poorer metabolic control relating to poorer physical health-related quality of life, as reported by children. All reports indicated a significant correlation between metabolic control and both general and diabetes specific health-related quality of life. There was a significant relationship with mother ̓s educational level on the outcome variables; educational level was therefore used as a control variable in all regression analyses. Child-reported general family functioning accounted for a significant amount of variance in child reported general health-related quality of life. Parent score on the non-supportive scale for diabetes specific family functioning accounted for a significant amount of variance in parent reported general health-related quality of life and diabetes specific health-related quality of life for their child. Results demonstrated the importance of obtaining both child and parent perspectives on issues regarding general family functioning and health-related quality of life and diabetes specific family functioning and health-related quality of life. In addition, they illustrated the importance of assessing health-related quality of life for children with diabetes as opposed to merely looking at the physical effects.
