Book excerpt: La prise en charge du cancer du sein métastatique a évolué ces 20 dernières années avec le développement de nouveaux traitements de chimiothérapie dont l'impact sur la survie n'est pas clairement établi. Nous avons analysé l'impact de la 1ère ligne de chimiothérapie sur la survie au cours du temps en comparant deux populations de patientes suivies pour un cancer du sein métastatique à l'Institut de Cancérologie de l'Ouest entre 1992 et 1995 (période I n=185) et entre 2001 et 2004 (période II n=196). Il n'y a pas de différence significative en survie globale entre les deux périodes (26 vs 29 mois p=0.12). Il existe une augmentation significative de la survie sans progression au cours du temps pour la population globale (10.2 vs 12 mois p=0.08) et dans le sous-groupe des patientes traitées par une 1ère ligne de chimiothérapie (8.4 vs 11.8 mois p=0.022). Le délai sans rechute > 24 mois, la positivité des récepteurs aux œstrogènes et la période II sont des facteurs pronostiques favorables sur la survie globale. Les nouveaux agents de chimiothérapie améliorent la survie sans progression mais ne semblent pas améliorer la survie globale.

Book excerpt: Le contraste entre l'augmentation de l'incidence des cancers du sein et la stabilité de la mortalité observée depuis vingt cinq ans en France est en grande partie expliqué par l'amélioration de la prise en charge et le diagnostic précoce. Notre travail avait pour objectifs d'évaluer l'impact de l'évolution des modes de prise en charge diagnostique et thérapeutique sur les survies globale (OS) et relative (RS), ainsi que sur la qualité de vie (QdV) des patientes atteintes de cancer du sein. Une première étude nous a permis d'évaluer les OS et RS ainsi que de déterminer les facteurs pronostiques. Nos résultats montrent que les OS et RS sont influencées par le stade TNM, le grade SBR, les récepteurs hormonaux, le statut multifocal de la tumeur, l'extension locorégionale et la période de diagnostic. Une étude de QdV, prospective, multicentrique nous a permis d'évaluer l'impact de différentes techniques chirurgicales : ganglion sentinelle (SLNB), SLNB + curage axillaire complémentaire (ALND) et ALND sur la QdV des patientes atteintes de cancer du sein en Bourgogne. Les résultats de cette étude révèlent que la SLNB permet d'améliorer la QdV des patientes si elle n'est pas suivie par un curage complémentaire. Enfin, une étude évaluant l'impact du moment d'administration de la chimiothérapie sur les OS et RS des patientes atteintes du cancer du sein, nous a permis d'évaluer l'effet de la chimiothérapie néoadjuvante sur la survie en utilisant la RS comme critère de jugement principal. Malgré une tendance favorable, la chimiothérapie néoadjuvante comparativement à la chimiothérapie adjuvante n'améliore pas les OS et RS des patientes atteintes de cancer du sein.

Book excerpt: Introduction : La récidive locorégionale est associée à un risque de survenue de métastases à distance et de décès dans le cancer du sein. L'objectif de cette étude était d'évaluer l'impact du délai de récidive locorégionale sur la survie sans métastase dans une série de cancers du sein traités par chimiothérapie néoadjuvante. Méthodes : Entre 2002 et 2012, 1191 patientes de la cohorte NEOREP à l'institut Curie ont été incluses. Une analyse multivariée a été réalisée à l'aide d'un modèle statistique de type « Random Survival Forest ». Résultats : Le délai médian de suivi était de 59,8 mois. 7,1% des patientes ont présenté une récidive locorégionale. Le délai de récidive locorégionale et la récidive locorégionale étaient les facteurs les plus discriminants dans la survenue de métastase. Un délai de récidive locorégionale inférieur à 50 mois était associé à un mauvais pronostic en termes de survie sans métastase à 3 et 5 ans. L'analyse par sous-types moléculaires (Luminal, triple négatif et Her2 positif) retrouvait également un seuil pronostique à 3 ans de 50 mois pour les 3 sous-types. A 5 ans, le seuil restait de 50 mois pour les tumeurs triple négatives mais était de 75 mois pour les tumeurs luminales et HER2 positives. Conclusion : Une récidive locorégionale avant 50 mois est un facteur pronostique important dans la survenue de métastases dans le cancer du sein traité par chimiothérapie néoadjuvante. Passé ce seuil, la survie sans métastase est peu influencée par le délai de récidive.

Ovarian cancer is cancer that begins in the cells that constitute the ovaries, including surface epithelial cells, germ cells, and the sex cord-stromal cells. Cancer cells that metastasize from other organ sites to the ovary (most commonly breast or colon cancers) are not then considered ovarian cancer. According to the American Cancer Society, ovarian cancer accounts for 4 percent of all cancers among women and ranks fifth as a cause of their deaths from cancer. The American Cancer Society statistics for ovarian cancer estimate that there will be 25,400 new cases and 14,300 deaths in 2003. The death rate for this disease has not changed much in the last 50 years. Unfortunately, almost 70 percent of women with the common epithelial ovarian cancer are not diagnosed until the disease is advanced in stage -- i.e., has spread to the upper abdomen (stage III) or beyond (stage IV). The 5-year survival rate for these women is only 15 to 20 percent, whereas the 5-year survival rate for stage I disease patients approaches 90 percent and for stage II disease patients approaches 70 percent. Ovarian tumors are named according to the type of cells the tumor started from and whether the tumor is benign or cancerous. The three main types of ovarian tumors are: Epithelial Tumors, Germ Cell Tumors and Stromal Tumors.
