Book excerpt: The introduction of cone beam computed tomography has promoted the realization of guided surgery in dental implant treatment.The aim of this study was to present clinical results of treatment with implants inserted by the technique of guided surgery and immediate loading in edentulous jaws.Fully edentulous maxilla patients were diagnosed by cone beam tomography and treated with 10 Galimplant 3D u00ae implants for rehabilitation by the technique of guided surgery and immediate loading. Immediately after surgery a full resin provisional immediate restoration was placed. At 6 months, the final restoration was performed. The clinical follow-up period was at least 60 months of functional loading.30 patients were treated with 300 implants. The clinical findings demonstrate an excellent clinical response of this type of protocol, free of postoperative complications in patients undergoing surgery. From an implantologic point of view, the results indicate a success rate of 98.4%. Five implants were lost during the healing period with the provisional prosthesis. After a mean functioning period of 77.8 months from the final restoration, there was no late complications.This study indicates that treatment with dental implants by guided surgery in the upper jaw is a high success rate therapeutic alternative.

Usually the 3D model of the bone and surrounding structures is obtained via cone beam computed tomography (CBCT) and the patientu2019s oral condition can be acquired conventionally and then digitalized using a desktop scanner - partially digital workflow (DW) or digitally with the aid of an intraoral scanner - full DW.The aim of the present pilot study is to compare the accuracy of a full digital workflow to partially digital workflow for a limited edentulous space (1 to 3 dental units) using an inspection tool software. Null hypothesis tested: full digital WF is more accurate comparing to partially DW.Materials and methods: The study was conducted in accordance with ethical principles including the World Medical Association Declaration of Helsinki, approved by the University Bioethical Committee, registered (ClinicalTrials.gov Identifier: NCT03814655) and written consent of each subject was obtained.Patients with Kennedy Class III partially edentulism, good general health, acceptance of dental implant treatment, acceptance of pre-operative CBCT, not requiring bone graft, not limited mouth opening were included.The full DW included: intraoral scan of the partially edentulous site, antagonists and occlusion registration, CBCT with radiopaque customized tray placed over the partially edentulous arch; merging files in R2 Gate planning software (Megagen, Korea), implant placement and digital scanning of implants position (.stl u2013 Stereolithography file).In the partially DW, digital impression was replaced with a conventional impression and poured casts are scanned with a desktop scanner.Results: 20 patients were enrolled in this prospective, pilot study and 38 implants were inserted using tooth-supported surgical templates and a flapless technique. Neither complications nor unexpected events occurred during implants insertion.22 implants were inserted using full DW. Inserted and planned implant position were compared using Geomagic Qualify 2013 software (3D Systems,USA).The mean 3D errors of implant positioning for the two insertion protocols were: 0.62mm (u00b10.42) entry point, 0.98mm (u00b10.73) at the implant apex, 2.1 (u00b10.35) angular deviation for full DW, respectively 0.68(u00b10.57), 1.02mm (u00b10.43) and 2.4 (u00b10.22) for partially DW. The t test for implant position showed no statistically significant differences (p > .05) in accuracy between full DW and partially DW, rejecting the null hypothesis tested.Conclusions and clinical implications: Within the limits of the present pilot study, the surgical template used has proved high accuracy for implant insertion, both in full and partially DW.The results of this study suggest that a full digital workflow for computer-guided implant surgery in partially edentulous patients is a reliable method and could be used in clinical situations.

The use of virtual implant planning and subsequent navigation also allows for prosthetic and surgical collaboration with precise planning and accurate orchestration of the plan to achieve a high level of patient-specific results. More research is needed to evaluate the outcome of implant-supported surgical procedures assisted by dynamic navigation.Aim/HypothesisTo describe the workflow of a maxillary full-arch rehabilitation through the All-on-4u00ae concept assisted by dynamic navigation. Material and MethodsA female healthy patient with 57 years of age presented with need of maxillary full-arch rehabilitation. Orthopantomography and Cone Beam Computerized Tomography (CBCT) scan were taken and the DTX Studio Implant Software was used to evaluate the baseline clinical situation.The surgery was performed under local anesthesia. After incision, flap raising and bone reduction 5 tri-screws were fixated in the maxilla. A CBCT scan was made and in the DTX Studio Implant Software the position and size of implants were selected. After the fixation of the X-Guideu00ae receptor the navigation surgery was initiated. The drills used to prepare the implant bed were calibrated before the osteotomy. Four implants (All-on-4u00ae Maxilla MALOCLINIC protocol; Nobel Parallel CC implants) were inserted achieving 35 N/cm of primary stability; Multi-Unit Plus abutments were attached and a provisional pre-made prosthesis was connected achieving immediate function.ResultsThe X-Guide Navigationu00ae provided the precise position and orientation of the drills and implants in every step of the surgery. This tool enabled the placement of the implants in the most favorable prosthetic and surgical positions, avoiding important anatomic structures such as the maxillary sinus and the nasal cavities. All the implants were placed with an insertion torque of 35 N/cm which allowed the connection of the prosthesis on the same day of surgery, achieving immediate function. The main advantages of the dynamic navigation assisted surgery include its accuracy, time- and cost-effectiveness, and the ability to change the implant size, system and location during the surgical procedure. Nevertheless, the clinician must understand that a learning curve is required to gain proficiency using a dynamic navigation system. This system could benefit from the following improvements. For total edentulous cases, simpler fiducial markers could make the surgery faster and less complicated once the tri-screws fixation costs time and could limit the implants position.Conclusion and Clinical implicationsThe surgical procedure assisted by dynamic navigation for the full-arch rehabilitation of the maxilla was safe and predictable, allowing an accurate insertion of the implants in depth, angulation and crestal position. The system could benefit from improvements in order to make the management of full-arch cases simpler.

Methods: Computer-guided implant surgery was performed in 23 patients with 57 implants in total. Preoperative CBCT scans were fused with patientu2019s casts to produce the surgical templates. Postsurgery CBCTs were also performed after the implant placement. The collected data were used to analyze the possible differences of the implant fixture body placement between preoperative plan and postoperative actually placed implant, evaluating the angulation and distance errors.Results: The mean angulation errors between the preoperative planned and postoperative placed implant was 5.84u00b0 u00b1 1.87u00b0; the mean distance errors between the planned and actually placed implants were 0.67 u00b1 0.38 mm horizontally and 0.74 u00b1 0.48 mm vertically at the implant neck and 0.82 u00b1 0.51 mm horizontally and 0.74 u00b1 0.39 mm vertically at the implant apex for all 57 implants.Conclusions: Computer-guided implant planning and insertion with 3Shape software, seems to provide sufficient accuracy, although more clinical studies should be conducted. Deviations are mainly related to CBCT accuracy and the implementation of the digital radiographic data to the 3D model through the software planning system.

The purpose of this article is to describe a fully digital workflow that was used to perform computer-guided flapless implant placement in an edentulous patient without the use of conventional impressions, model casting and a radiographic guide. Material and Methods:The digital data for the workflow was acquired using an intraoral scanner and cone-beam computed tomography (CBCT). The image fusion of the intraoral scan data and the CBCT data was performed by matching the resin markers placed in the patientu2019s mouth. The definitive digital data were then used to design a prosthetically driven implant position, a surgical template and a computer-aided design/computer-aided manufacturing (CAD/CAM)-fabricated fixed dental prosthesis. Immediate restoration was performed using surgical templates and fixed dental prosthesis immediately after implant placement. After implant placement, accuracy of implant placement was evaluated using a 3D image processing software. Results: The average angular deviation between the planned and placed implant positions was 1.1u00b0 with a minimum deviation of 0u00b0 and maximum deviation of 3.2u00b0.Conclusions:The results of this study suggest that a full digital workflow for computer-guided implant surgery in edentulous patients is a reliable method.

Book excerpt: Background: The atrophy of edentulous ridge and pneumatization of the maxillary sinus often limit the implant placement on posterior maxilla. The success of maxillary posterior implant depends on the increase of the available bone and initial stability of the implant after maxillary sinus reconstruction. Less morbidity and complications after operation is major advantage of crestal approach than lateral approach. However, when the residual ridge height is less than 5 mm, it is known that lateral approach is appropriate and delayed implantation is recommended in severely atrophic ridge. However, in recent studies, unlike the conventional concept, it is believed that simultaneous placement of the long implant using the crestal approach is reliable, even if the residual bone is less than 4 mmAim: We present the three cases of implant placement simultaneously sinus augmentation using crestal approach in posterior maxilla site with 3 mm or less of residual alveolar bone height. The objective of this study was to suggest implant placement simultaneously crestal approach as the treatment option in severely atrophic maxilla.Material and Methods: After extraction of hopeless teeth, cone beam computed tomography was taken to determine the bone quality and quantity of intended implant sites. In first case, residual vertical bone height was 1.4mm and bone quality was type 4. In second case, 2.2mm of vertical bone height and type 2 bone quality was observed. In third case, 2.9mm of vertical bone height and type 3 bone quality was observed. In all three cases, sinus elevation with CAS kit (Crestal approach sinus kit, Osstem, Seoul, Korea) was performed and the trapped space was filled with xenograft (Bio-Oss, Geistlich, Wolhusen, Swiss). Xenograft was spreaded by bone spreader, so that would be reduce the tension of lifted sinus membrane. To ensure the stability of implant, underdrilling with CAS drill was performed. Internal connection type implants of 13 mm length with SLA surface were placed and initial fixation was stable in all three cases. After 6-7months later, 2nd surgery was performed.Results: In first case, there is no complications during the healing period and implant stability quotient (ISQ) was 85 at 2nd surgery. Five months later, final prosthesis was delivered. At one year follow up, marginal bone and augmented bone level was stable and soft tissue around implant was healthy. Patient had no discomfort and were satisfied with this results. In second case, ISQ was 85 at 2nd surgery and then final prosthesis was delivered. A great amount of bone fill in maxillary sinus was observed in radiography. Marginal bone and augmented bone level was stable at 1.5 year after final prosthesis delivery. In third case, ISQ was 74 at 2nd surgery and then final prosthesis was delivered. In radiographic view, augmented bone in sinus and marginal bone around implant was not resorbed in one year follow up. Also, newly formed maxillary sinus inferior border was clearly observed. Soft tissue around implant was healthy and no complications were reported. Conclusion: In this study, at the severely atrophic maxillary posterior site with 3 mm or less of residual vertical bone height, 10 mm sinus membrane elevation and bone graft was performed with crestal approach and implants were placed simultaneously. In short-term follow up observation, augmented bone and soft tissue around implant was stable and healthy. The use of crestal approach in severely atrophic maxilla and simultaneously implant placement was suggested as the treatment option, however, further follow-up is needed about long term success rate of these implants.
