Book excerpt: INTRODUCTION : La Maladie des Petits Vaisseaux Cérébraux (MPVC) est une cause fréquente d'AVC récidivants et de troubles cognitifs. Son diagnostic repose sur des critères d'IRM cérébrale, examen qui reste peu accessible pour un dépistage à large échelle dans une population. Il existe des arguments laissant penser que les micro vascularisations cérébrale et oculaire peuvent être affectées par les mêmes processus pathologiques, mais les bilans ophtalmologiques sont potentiellement plus accessibles que l'IRM cérébrale pour dépister une MPVC. Il n'existe cependant pas à ce jour d'étude des examens rétiniens réalisés durant la phase aigue d'un AVC, ni de caractérisation de l'imagerie vasculaire rétinienne utilisant l'OCT-Angiographie dans cette population. MÉTHODES : Dans cette étude monocentrique, les patients étaient recrutés de novembre 2019 à juin 2020 durant leur hospitalisation en Unité de Soins Intensifs Neurovasculaire. Les critères d'inclusion étaient de présenter un AVC ischémique ou hémorragique en phase aiguë, ainsi que des signes de MPVC sur l'IRM cérébrale réalisée, à partir de laquelle était déterminé le score total cSVD. Les critères IRM de MPVC étaient ensuite comparés en aveugle à un bilan d'imagerie rétinienne incluant rétinophotographies, OCT maculaire et OCT-Angiographie. RÉSULTATS : 36 patients ont été inclus. L'âge moyen était de 67,8 ans ± 13,6, 72,2% étaient de sexe masculin, 86,1% présentaient un AVC ischémique et 13,9% un AVC hémorragique. 13,9% des patients ont bénéficié d'un traitement de reperfusion pour l'AVC ischémique. Le score médian NIHSS était de 2,5 [1-8]. 69,4% avaient un antécédent d'hypertension et 22,2% de diabète. 38,9% des patients présentaient une anomalie rétinienne sur le bilan initial. Nous n'avons pas mis en évidence de corrélation entre la présence d'anomalie rétinienne et le score total cSVD. Nous n'avons pas mis en évidence d'anomalie microvasculaire spécifique en OCT-Angiographie. CONCLUSION : Notre étude confirme la co-occurrence de lésions microvasculaires rétiniennes chez des patients en phase aiguë d'AVC avec signes de MPVC. Notre effectif réduit de patients n'a pas permis de mettre en évidence de corrélation statistiquement significative entre la sévérité des anomalies rétiniennes et la sévérité de la MPVC.

Book excerpt: La crise d'épilepsie à la phase aigue d'un AVC est un évènement rare, dont la conséquence sur le devenir du patient n'est pas définie. Plusieurs études s'accordent sur des facteurs de risque cliniques (sévérité de l'AVC) ou radiologiques (localisation corticale, remaniement hémorragique...), mais aucune ne prend le scanner de perfusion initial en compte. Objectif : Déterminer à l'aide du scanner de perfusion à la phase initiale de l'AVC, des facteurs de risque de crise de phase aigue. Patients et méthodes : A partir de patients éligibles à la thrombolyse intraveineuse ayant bénéficié d'un scanner de perfusion, et ayant constitué un AVC ischémique sus-tentoriel recueillis, nous avons mené une analyse d'imagerie en cas témoins, comparant les paramètres de CBF, CBV, MTT et TTP, des zones de coeur et de pénombre de l'ischémie, entre une population ayant présenté une crise de phase aigue, et une population sans crise. Une analyse de facteurs clinico-biologiques sur l'ensemble de la population recueillie a été réalisée en parallèle. Résultats : 279 patients ont été recueillis. 15 (5,7%) ont présenté une crise de phase aigue, et 32 sont inclus dans l'analyse d'imagerie (11 cas et 21 témoins). Il n'existe pas de paramètre de perfusion significativement associé à une crise de phase aigue. Le taux de cholesterol total est corrélé négativement avec la crise de phase aigue (p=0,04), tandis que le remaniement hémorragique est un facteur de risque de crise (p=0,04). Conclusion : Il n'est pas mis en évidence de facteur de risque de crise de phase aigue, à partir du scanner de perfusion initial. Davantage de patients sont nécessaires pour confirmer ou infirmer la tendance sur le CBF.
