Book excerpt: Contexte : Si la douleur aiguë a une fonction physiologique, la douleur chronique constitue un phénomène pathologique aux dimensions multiples : physiologique, psychologique, et sociale. Sa prévalence élevée en fait un problème de santé publique dont les pouvoirs publics se sont saisis en lançant plusieurs plans de lutte contre la douleur. Le rapport d'évaluation du dernier plan préconisait d'intégrer un enseignement spécifique pour les étudiants de troisième cycle médical. L'élaboration d'un tel enseignement pour les internes de médecine générale pourrait tenir compte de leurs difficultés et de leurs attentes. Peu d'études existent sur ce sujet. Cette étude explore les difficultés rencontrées par les internes de Médecine Générale et leurs attentes concernant leur formation initiale Méthode : Il s'agissait d'une étude exploratoire, qualitative, par focus group d'internes en médecine générale de la faculté de médecine de Lille. Les entretiens ont été conduits jusqu'à saturation des données, enregistrés et retranscrits intégralement. Les données ont été analysées de manière thématique avec un double codage. Résultats : Six focus groups ont été réalisés, réunissant quarante internes. Des difficultés à tous les niveaux de prise en charge ont émergé : thérapeutique, relationnel, psychologique et social. Le statut et le contexte d'exercice des internes entraînaient des difficultés. Un grand nombre de difficultés étaient attribuées à une formation insuffisante sur les différentes dimensions de la douleur chronique. Les internes montraient un intérêt pour un enseignement de mode participatif se basant sur leur expérience. Conclusion : Les difficultés rencontrées par les internes participants pourraient être en rapport avec la confrontation de leur culture biomédicale à un problème multidimensionnel et complexe. Une approche biopsychosociale permettrait-elle de surmonter ces difficultés ? Les attentes des internes concernant leur formation semblaient conforter l'évolution d'un paradigme d'enseignement vers un paradigme d'apprentissage pris par l'enseignement de médecine générale.
