The clinical aspect of these diseases has been kept in view throughout. The literature of the subject is now so enormous that without very greatly exceeding my limits it was obviously impossible to deal at all fully with it, but it would not be right for me not to acknowledge, even thus imperfectly, my great indebtedness to former writers, and especially the more recent ones, on Hysteria and Neurasthenia. The perusal of their works, so many of them distinguished by wealth of observation and breadth of knowledge, makes me fully conscious of my own shortcomings. Whilst, however, my endeavour has been to keep within moderate limits, and to give the results of my own clinical observation, I trust that I have left out nothing of importance, and that this little work may prove of some service as a practical guide, and that the statements made in it have at least the merits of accuracy and clearness. This affection has been the subject of many descriptions since the truly fundamental picture of it drawn by Beard. The American author was not indeed the first to observe and to isolate Neurasthenia, since its principal symptoms are found in the Spinal Irritation of Franck, the Proteiform Neuralgia of Cerise, the Nervosity of Bouchut, and the Cerebra cardiac Neuropathy of Krishaber; but Beard's monograph had the merit of being more synthetic and more complete than its predecessors, and above all it had the good fortune to appear at a propitious moment. While Nervous Exhaustion seemed to become multiplied in America and naturally drew the attention of neuro-pathologists (weir Mitchell after Beard), in the old world the new conditions created by the exigencies of modern life promoted the recrudescence of the Neurosis from Exhaustion of which Monneret had already spoken. It is an often-repeated commonplace to-day, that in consequence of the progress of civilisa tion and the increase in cerebral activity entailed thereby, neuroses in all their forms have become much more common than formerly. This state ment deserves the trouble of verification, and it must be confessed that precise observations for making this verification with some degree of exactitude are still wanting. Besides, even if the pessimists were right, it would not thereby be proved that the entire increase in the number of nervous complaints must be ascribed to the refinements of civilisation and the new require ments that our brains have to satisfy. Intoxica tions, including the least refined of them, alcoholic poisoning, might rightly claim a large share in the determining causes of the nervous disorders that seem to be special to our epoch. Book excerpt: Excerpt from A Treatise on the Nervous Diseases of Women: Comprising an Inquiry Into the Nature, Causes and Treatment of Spinal and Hysterical Disorders I gladly seize the opportunity of propitiating the reader in favour of my work, afforded by a preface; because I fear the peculiar plan I have adopted in treating my subject, may at the first glance be repulsive, especially to those of my brethren busily engaged in their professional duties. I have often thought that treatises on the practice of Medicine professing to be free from theory, and to contain nothing more than a description of diseases, and the methods of treatment, are of questionable utility. The condensed style in which they are usually written, admits of no detailed exposition of the principles laid down, or of the facts from which those principles are deduced. The writer, conse quently, appears to dictate, rather than to state an argument; to be the occupant of a professorial chair, rather than a fire side companion. The interest which physiology might give to the subject is lost sight of and thus the work is dry and uninteresting, and never studied. If used as a book of reference, the rules for correct diagnosis and treatment are so precise, as to want the necessary elasticity of application to the ever-varying forms of disease; or so general, that they only increase the doubt and difficulty they are intended to diminish. At best, the practice recommended is empirical; and often is so far from being devoid of theory, that it is founded on very doubtful hypotheses. Scientific medicine has a much wider range of immediate practical usefulness and prospective benefit than this mere practical medicine. Its rules are less applicable to individual diseases, than to the infinite variety of individual cases. It gladly makes use of empiricism; but it studies symptoms to demonstrate their causes, and relieves the former by removing the latter, or counteracting their effects. And when science fails to guide, it is still able to apply empirical knowledge with precision and effect.
