The Institute of Medicine held a workshop, summarized in this volume, to describe the dynamic technological, agricultural, and economic issues contributing to the food price increases of 2007 and 2008 and their impacts on health and nutrition in resource-poor regions. The compounding effects of the current global economic downturn on nutrition motivated additional discussions on these dual crises, their impacts on the nutritional status of vulnerable populations, and opportunities to mitigate their negative nutritional effects.

While there is ample description of the nature, content, and causes of food price fluctuations during the last 5 to 7 years, very little is known about the processes that led to policy responses or the relative power and behaviour of the participating stakeholder groups. Understanding how and why governments responded as they did is important to enhance the existing knowledge of the political economy of food price policy and to assist governments in their policy-making as they confront future food price fluctuations. These volatile and unpredictable prices may undermine incentives for farmers to respond to high price levels with the critical increase in production needed to bring food prices down. In practical terms, farmers decide what to plant and countries deciding when to import face uncertainty in the likely distribution of world food prices and greater consequences when using past price levels and distributions to guide current decisions. This uncertainty keeps food prices high for a longer period, leading to fundamental food security risks for consumers and governments. To achieve more efficient domestic price formation, policy actions need to focus on (i) strengthening the integration of local markets with international markets (through investments in infrastructure and market-oriented policies), especially local markets that are more volatile than international markets, (ii) reducing global food price volatility, including through more discipline on trade policy, and (iii) strengthening safety nets to rapidly and cost-effectively protect the poor and vulnerable from the food price spikes.

However, the same system also creates significant environmental, public health, and other costs that generally are not recognized and not accounted for in the retail price of food. These include greenhouse gas (GHG) emissions, soil erosion, air pollution, and their environmental consequences, the transfer of antibiotic resistance from food animals to human, and other human health outcomes, including foodborne illnesses and chronic disease. Some external costs which are also known as externalities are accounted for in ways that do not involve increasing the price of food. But many are not. They are borne involuntarily by society at large. A better understanding of external costs would help decision makers at all stages of the life cycle to expand the benefits of the U.S. food system even further. The Institute of Medicine (IOM) and the National Research Council (NRC) with support from the U.S. Centers for Disease Control and Prevention (CDC) convened a public workshop on April 23-23, 2012, to explore the external costs of food, methodologies for quantifying those costs, and the limitations of the methodologies. The workshop was intended to be an information-gathering activity only. Given the complexity of the issues and the broad areas of expertise involved, workshop presentations and discussions represent only a small portion of the current knowledge and are by no means comprehensive. The focus was on the environmental and health impacts of food, using externalities as a basis for discussion and animal products as a case study. The intention was not to quantify costs or benefits, but rather to lay the groundwork for doing so. A major goal of the workshop was to identify information sources and methodologies required to recognize and estimate the costs and benefits of environmental and public health consequences associated with the U.S. food system. It was anticipated that the workshop would provide the basis for a follow-up consensus study of the subject and that a central task of the consensus study will be to develop a framework for a full-scale accounting of the environmental and public health effects for all food products of the U.S. food system. Exploring Health and Environmental Costs of Food: Workshop Summary provides the basis for a follow-up planning discussion involving members of the IOM Food and Nutrition Board and the NRC Board on Agriculture and Natural Resources and others to develop the scope and areas of expertise needed for a larger-scale, consensus study of the subject.

A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. It focuses on the three most important food commodities in the world from a calorie viewpoint, namely wheat, rice and maize. Following the introductory chapter, Chapter 2 presents an overview of the determinants of agricultural commodity prices. Chapter 3 analyses political economy causes of trade policy interventions in response to commodity price spikes to reveal why governments alter trade interventions in order to cushion domestic prices from external shocks. To examine how much countries insulate their domestic markets from international markets, Chapter 4 estimates price transmission elasticities for wheat, rice and maize by employing a novel estimation approach. The first essay (Chapter 2) probes demand and supply shocks in global grain markets. Using annual data from 1961 to 2012, price fluctuations for wheat, rice and maize are decomposed into three components: supply shocks, demand shocks and other shocks. The ""other shocks"" here include policy responses of some governments, such as export restrictions and panic purchases when prices spike upwards. The chapter estimates how much each of these shocks contributed to the evolution of grain prices during the last half century. The results suggest that supply and demand shocks have contributed very little to price fluctuations, compared to other shocks. The other shocks seem to have exacerbated exogenous price rises due to a supply shortfall, particularly in periods when prices spike. The second essay (Chapter 3) studies the implications of reference dependence and loss aversion features of individual preferences in trade policy determination and shows that these behavioural features help explain why governments change their trade restrictiveness in order to cushion domestic prices from international price shocks. We show that this change comes irrespective of whether special interest groups are lobbying the government. Using a global dataset on agricultural price distortions, we find the available empirical evidence is consistent with our model predictions. The empirical evidence further suggests that governments' reactions to international prices are stronger for staple food items than for non-staples. A voluminous literature has estimated price transmission from international to domestic markets, ignoring unobserved common factors that affect all domestic markets. In the third essay (Chapter 4), this thesis estimates long-run and short-run transmission elasticities of world commodity price shocks to domestic markets using a framework that takes into consideration common factors that are correlated with regressors. The estimates are based on an annual panel dataset for rice, wheat and maize for both developed and developing countries over the period 1960-2007. The results from the common factor framework are compared to those that do not account for common factors. The findings suggest price transmission elasticities for rice, wheat and maize are around 0.4, 0.5 and 0.75 respectively. The findings suggest that ignoring common factors is likely to result in upwardly biased estimates of price transmission elasticities. The final chapter of the thesis (Chapter 5) contains a summary of the findings of the three main chapters in the thesis.

It does so using new annual estimates since 1955 of agricultural price distortions in 75 countries, updated to 2008. Responses by food importers to upward price spikes are shown to be as substantial as those by food exporters, thereby weakening the domestic price-stabilizing effect of intervention by exporters. They also add to the transfer of welfare to food-surplus from food-deficit countries -- the opposite of what is usually thought of when considering inter-sector trade retaliation. Phasing down World Trade Organization-bound import tariffs toward their applied rates would help reduce the legal opportunities for food-deficit countries to raise their import restrictions when international prices slump. To date there is no parallel discipline in the World Trade Organization that limits increases in export restrictions when prices spike upward, however. Bringing such discipline through new World Trade Organization rules could help alleviate the extent to which government responses to exogenous price spikes exacerbate those spikes.
