The worry and uncertainty associated with combat deployments provokes anxiety in family members left at home. Lengthy separations may challenge the personal, social, and economic coping resources of families at home. In this war, thanks to medical advances, many service members who previously would have died of their injuries are returning home to live long, although altered lives. As a result, families are facing the additional challenge of assisting service members who have experienced amputation, traumatic brain injury, and psychological wounds. These challenges are faced not only by service members in the active component of the armed forces, but also by service members in the National Guard and Reserves. In response, the Department of Defense has launched unprecedented efforts to support service members and families before, during and after deployment in all locations of the country as well as in remote locations. These support efforts are focused not only on medical care, but also mental health care and logistical support. Research about families and war tends to move forward in fits and starts associated with major conflicts, and there is currently an increasing flow of family research moving into the scientific domain. Book excerpt: Over 1,000,000 service members were deployed away from their families in the first 5 years since the start of the U.S. military campaign in Afghanistan and Iraq, and over 350,000 service members experienced two or more deployments. Although deployments disrupt the contribution of service members to household production, little evidence exist about the effect of deployments on spouses. This dissertation examines an effect of deployment on spousal labor force participation and household wellbeing. Combining administrative pay records with responses to surveys of active duty personnel, I find that deployment reduces spousal labor force participation by 2.8 percentage points. This reaction varies by the age of the youngest child in the family. Deployment reduces spousal labor force participation by 4.9 percentage points in families with children under age 6. I also find that spouses decrease their labor force participation several months before service members are actually deployed, and return to the labor force several months after service members are actually deployed, and return to the labor force several months after service members return from deployment. I a slo find that deployment increases reenlistment rates. Those soldiers who were deployed since September 11, 2001 are more likely to stay in the military based on the spousal taste for employment. Those spouses who have a high taste for a career and perceive that military life may impose constraints on their job opportunities encourage service members to leave the military.

More than 1.5 million service members were deployed between 2002 and 2007, many of them more than once, and the fast pace of deployment has been felt throughout the military. Soldiers and marines have faced a steady cycle of predeployment training and exercises, deployment itself, and postdeployment reassignment and unit regeneration. Service members not on deployment are nonetheless busy planning and supporting military operations, caring for injured service members, and attending to recruiting, training, and other responsibilities at home and abroad. Many service members are married, and deployments have disrupted their family routines and created stress from separation and reintegration. At the same time, the long hours, tension, uncertainty, and violence of deployments have stressed the service members sent to fight. Remarkably, despite the pressures from deployments on service members and their families, reenlistment rates have been stable since 2002. The purpose of this monograph is to enhance understanding of whether deployments affected service members' willingness to stay in the military, as the stress caused by deployments would suggest, and how it was that reenlistment held steady.

The results of this study reinforce some of what we already know concerning deployments and children, but they also reveal some very interesting, counterintuitive findings that challenge the conventional wisdom concerning Army adolescents. This study goes beyond merely explaining the impact 8 years of war is having on the children of our Soldiers; rather, it explores the specific factors that increase or alleviate stress on Army adolescents. The results reveal that Army adolescents, contrary to what many believed, are much more self-aware and resilient. Furthermore, they are capable of understanding the multiple implications of having a parent serve in the all-volunteer Army during a time of war. Army children may experience the anxiety and stress that often surround a parent's deployment, but results conclude that there are factors that policymakers, leaders, and parents can use to increase a child's ability to cope with a life of repeated deployments. Book excerpt: As of December 2012, Operation Enduring Freedom (OEF) in Afghanistan and Operation Iraqi Freedom (OIF) in Iraq have resulted in the deployment of about 2.2 million troops; there have been 2,222 US fatalities in OEF and Operation New Dawn (OND)1 and 4,422 in OIF. The numbers of wounded US troops exceed 16,000 in Afghanistan and 32,000 in Iraq. In addition to deaths and morbidity, the operations have unforeseen consequences that are yet to be fully understood. In contrast with previous conflicts, the all-volunteer military has experienced numerous deployments of individual service members; has seen increased deployments of women, parents of young children, and reserve and National Guard troops; and in some cases has been subject to longer deployments and shorter times at home between deployments. Numerous reports in the popular press have made the public aware of issues that have pointed to the difficulty of military personnel in readjusting after returning from Iraq and Afghanistan. Many of those who have served in OEF and OIF readjust with few difficulties, but others have problems in readjusting to home, reconnecting with family members, finding employment, and returning to school. In response to the return of large numbers of veterans from Iraq and Afghanistan with physical-health and mental-health problems and to the growing readjustment needs of active duty service members, veterans, and their family members, Congress included Section 1661 of the National Defense Authorization Act for fiscal year 2008. That section required the secretary of defense, in consultation with the secretary of veterans affairs, to enter into an agreement with the National Academies for a study of the physical-health, mental-health, and other readjustment needs of members and former members of the armed forces who were deployed in OIF or OEF, their families, and their communities as a result of such deployment. The study consisted of two phases. The Phase 1 task was to conduct a preliminary assessment. The Phase 2 task was to provide a comprehensive assessment of the physical, psychologic, social, and economic effects of deployment on and identification of gaps in care for members and former members, their families, and their communities. The Phase 1 report was completed in March 2010 and delivered to the Department of Defense (DOD), the Department of Veterans Affairs (VA), and the relevant committees of the House of Representatives and the Senate. The secretaries of DOD and VA responded to the Phase 1 report in September 2010. Returning Home from Iraq and Afghanistan: Assessment of Readjustment Needs of Veterans, Service Members, and Their Families fulfills the requirement for Phase 2.

During their service in the military, U.S. active members may be sent to any part of the world, including parts of the world where Malaria is an issue. In Liberia in 2003, for example, there was a 28 percent attack rate in Marines who spent a short time ashore, and half of the 80 Marines affected needed to be evacuated to Germany. This was not only costly to the U.S. military but dangerous as well. To fight against this disease, there exists a Malaria Vaccine program in the U.S. military. However, there exists a variety of potential vaccine targets for the most severe and important form of malaria; malaria from the species Plasmodium falciparum. Issues also arise with the fact that there are three possible stages to create vaccines against-preerythrocytic, blood, or transmission. The Department of Defense (DoD), through the commanding general of the U.S. Army Medical Research and Materiel Command (USAMRMC), requested that the Institute of Medicine (IOM) conduct a programmatic review of the military Plasmodium falciparum malaria vaccine research and development program. There was to be a focus on vaccine against the preerythrocytic and blood stages. The IOM formed a committee of 11 experts with collective expertise in malaria vaccine research, parasite immunology, malarial biology, clinical trials and regulatory affairs, industrial and public-sector vaccine development, biologic products research and development (vaccinology), military research and development programs, tropical medicine, and public health. The committee focused different tasks including determining whether the DoD malaria vaccine research and development program is scientifically sound and able to achieve the vaccine program objectives within specified timelines, recommending how to overcome significant, identified barriers, and identifying major strategic goals and timelines based on the material received and presentations made by the DoD's program representatives. Battling Malaria: Strengthening the U.S. Military Malaria Vaccine Program presents the committee's findings, current malaria vaccines, and recommendations for the development of the U.S. Military vaccine research.

Book excerpt: In response to the threat of global terrorism, operational units deploy more quickly world-wide than in military conflicts of the past. As a result, service members are deploying multiple times to Iraq and Afghanistan in support of the Global War on Terrorism (GWOT). Stress of deployment may prompt coping mechanisms such as smoking, and may affect mental and functional health. The purpose of this dissertation was to investigate the effects of multiple deployments and other extreme deployment experiences on the postdeployment health of service members supporting GWOT. Analyses in this dissertation utilized data from the Millennium Cohort Study, which enrolled over 77,000 service members between July 2001 and June 2003. Self-reported deployment history was compared with military electronic records for over 55,000 participants who completed a follow-up questionnaire between June 2004 and February 2006. Agreement was substantial for deployment status, frequency, and number of deployments (kappa = 0.81, 0.71, and 0.61 respectively). No clinically meaningful differences in functional health for the small proportion with inconsistent deployment information were observed. Based on these findings, electronic military deployment data were used for the prospective evaluations of deployment and smoking and deployment and functional health. Smoking increased 43.8% among nondeployers and 55.6% among deployers. In multivariable logistic regression, past smokers were 35 times more likely to smoke at follow-up than never-smokers (odds ratio, 35.3; 95% confidence interval, 31.8-39.3). Those who deployed multiple times, & gt; 9 months, or deployed with combat exposures were at greater risk for smoking uptake. On average, mental and physical functioning scores decreased significantly among participants who deployed & gt; 9 months or were deployed with combat exposures. Extreme deployments, including multiple deployments, deploying longer than 9 months, and deploying with combat exposures were consistently associated with smoking uptake, smoking relapse, and small decrements in mental and physical functioning, though functional health scores were remarkably consistent. Service members, however, may manage the stress of deployment by using various coping mechanisms which may have detrimental health consequences. Increased smoking rates unquestionably imply future long-term morbidity, underscoring the importance of smoking prevention and cessation programs targeted towards deployed military personnel.
