Book excerpt: Contractures are a common complication of neurological and musculoskeletal conditions and are characterized by a reduction in joint mobility. No muscle is completely relaxed while the body is in a conscious or non-paralyzed state. Muscle tone keeps the bones in place and allows us to maintain posture. When there is a shortening of a muscle or tendon, this results in moderate to severe dysfunction in ROM it is called a contracture. The most common contractures are found in the ankle/foot, hip, knee, elbow, and wrist/hand/finger areas of the body. LESSON OBJECTIVES After completion of this program, the home health aide will be able to: Define a contracture Describe the four different types of joints Differentiate between the different types of range of motion (ROM) Identify three injuries or diseases that predispose patients to contractures Identify the general guidelines for ROM exercises Contents of this lesson: A clearly written fact sheet A 10-question post-test to measure understanding of the subject matter An answer sheet with a place for the instructor's comments and signature An illustrative, homecare-specific case study Suggested supplemental learning activities An attendance log and certificate of completion

Book excerpt: Home Health Aide On-the-Go In-service Vol 10, Issue 8, Falls Risk and Prevention OVERVIEW Preventing falls is a crucial part of a home health aide's job, as they are frequently with the patient in an ambulatory environment. The aide must be able to anticipate what situations might cause a fall, and what conditions the patient suffers from that might make a fall more likely. This in-service focuses on a necessary awareness of the risk factors for falls at home. Aides will learn methods to prevent falls, which includes recognizing elements in the home that may require modification or medical intervention. LESSON OBJECTIVES After completion of this program, the home health aide will be able to: List which diseases or conditions may cause falls in the elderly List the risks of falls Explain ways to prevent falls in the home Contents of this lesson: A clearly written fact sheet A 10-question post-test to measure understanding of the subject matter An answer sheet with a place for the instructor's comments and signature An illustrative, homecare-specific case study Suggested supplemental learning activities An attendance log and certificate of completion

As aides need training, you can make as many copies as you want - there's no restriction when used with aides assigned from your office location. Remember that Home Health Aides must have 12 hours of in-service training every year. LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: * List three reasons a person may have increased risk of skin breakdown * Name four ways of decreasing pressure to body areas * List three common areas of the body most likely to suffer increased pressure in patients confined to bed and chair, and * List three actions for protecting patients from skin breakdown. OVERVIEW The prevalence and incidence of skin breakdown among homecare patients has always been of great concern. It has become financially problematic for homecare agencies operating within the prospective payment system (PPS) since wound care is expensive. Data on pressure ulcers have become important markers for quality of care issues in the Adverse Event and Outcome Reports that may soon be made available to the public. Many patient receiving homecare are at high risk for developing skin breakdown, and most of those who are highest risk also receive home health aide services. Prevention and early detection of skin breakdown by home health aides can be of tremendous benefit to the patient and to the agency.

As aides need training, you can make as many copies as you want - there's no restriction when used with aides assigned from your office location. Remember that Home Health Aides must have 12 hours of in-service training every year. LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: * Identify three behaviors that may reflect alterations in cognitive or emotional status * List two warning signs of impending physical aggression * Name three measures to take when dealing with patients with behavior problems * Name two reasons for behavior problems. OVERVIEW Mental disorders causing behavior problems are very common. The National Institute of Mental Health (NIMH) estimates that one in five adults in the United States suffers from a diagnosable mental disorder in a given year. Of the ten leading causes for disability, four of them are mental disorders. In addition to the diagnosable mental disorders, patients may suffer acute anxiety following surgery or a major illness. Therefore, many homecare patients have some degree of alteration in mental processes or behaviors that may interfere with the goals of the medical and nursing treatment. Often the behavior problems diminish the patient's ability to achieve optimal day-to-day functioning. Working with patients with behavior problems is challenging to home health aides. While their basic curriculum included emotional needs and ways to work with patients, they often feel somewhat ill-at-ease when patients display inappropriate or hostile behavior. Many aides are fearful of saying or doing the wrong thing. The fact that they are often alone with patients in their homes can add to the discomfort in working with patients who have behavior problems. This in-service is designed to explain some common behavior problems encountered in homecare patients. It provides tips for dealing with these patients with the goal of increasing the aide's comfort level.

As aides need training, you can make as many copies as you want - there's no restriction when used with aides assigned from your office location. Remember that Home Health Aides must have 12 hours of in-service training every year. LESSON OBJECTIVES After completion of this program, the home health aide will be able to: * Describe two major functions of the kidneys * Name the two leading causes of kidney failure * Identify two observations home health aides should report, and * List two treatment options for patients with end-stage renal disease. LESSON OVERVIEW About 7.4 million adults in the United States have physiological evidence of kidney disease. Nearly 100,000 of them are diagnosed with renal failure each year. Diabetes mellitus is the most common cause of renal failure, with more than 40% of new cases attributable to diabetes. Hypertension causes more than 15,000 new cases of kidney failure each year, making it the second most common cause. Both diabetes and hypertension are common among homecare patients so it is not surprising that home health aides are often assigned to patients with renal failure. The purpose of this in-service is to educate home health aides about renal disease and to provide information about caring for affected patients.

Remember that Home Health Aides must have 12 hours of in-service training every year. LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: Name two common types of spinal surgery List three restrictions patients with spinal surgery may have Identify three complications that could occur after spinal surgery, and List three occasions when the aide should contact the supervisor. OVERVIEW The American Academy of Spine Physicians reports that four-out-of-five adults suffer from back problems at one time or another. Not all back problems require surgery and many conditions can be improved with conservative measures such as physical therapy, anti-inflammatory medications, epidural steroid injections, improved body mechanics, and weight loss. Some common conditions that occur with aging, such as degenerative disc disease, which causes the wearing away of the cushion (disc) between the vertebrae, may cause lower back pain. This condition cannot be treated with conservative measures and requires a spinal fusion. Osteoporosis may also cause weakening of the vertebral structures and result in the need for spinal fusion. Kyphosis, scoliosis, and spondyliothesis may also precipitate the need for spinal surgery. Some patients may require home health care following spinal surgery. Home health aides may be assigned to assist with bathing, dressing, and household activities.

LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: Explain three ways the aging process affects the human body, Name two myths about aging, and Explain two ways to encourage, support, and care for aging patients. OVERVIEW The world population is aging. According to the Center for Disease Control (CDC), the United States is on the brink of a longevity revolution. Today there are 37 million people in the United States who are 65 years or older. That number represents 13 percent of the current population and it's expected to grow to 80 million people by 2050. Today there are five million Americans older than 85; by 2050 there will be 20 million. Important questions face the healthcare industry. Who are these old people? Are they sick and disabled? Or are they relatively healthy as they age? These questions have important ramifications for homecare agencies. In order to provide aging patients the best care and the best outcomes possible, homecare aides must know the difference between reality and stereotypes. This in-service lesson looks at some of the myths of aging and talks about how home health aides can better care for their patients by understanding the aging process.

In addition to that, you have your own professional and personal obligations. It is common for all people, and especially for caregivers, to experience some stress and even "burnout" due to the pressures and responsibilities of caregiving. This in-service reviews the importance of caring for yourself first. It outlines how to do that by recognizing and dealing with stress. LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: * Define elder abuse * List three types of elder abuse * Describe four signs and symptoms of elder abuse * Name two factors that may contribute to elder abuse * Describe the role of the home health aide as it relates to elder abuse recognition and reporting LESSON OVERVIEW Elder abuse in domestic settings is an increasing problem. While the true incidence of elder abuse is unknown, hundreds of thousands of elderly adults are abused each year in the United States. Reports of elder abuse to Adult Protective Services agencies increased 150% between 1986 and 1996. The problem of elder abuse is so widespread that in most states certain professionals are required by law to report cases of domestic elder abuse. The greatest reporting of elder abuse comes from health care providers. Because home health aides typically spend more time with the frail elderly patient, they may observe signs of elder abuse that are not evident to other providers. Their observations and reports to professionals can be instrumental in triggering investigations and providing help to these patients.

Book excerpt: Home Health Aide On-the-Go In-service Vol 10, Issue 2, Preventing Hospitalization/Emergent Care OVERVIEW The transition of care between homecare and the hospital must be seamless, as flaws in this process lead to expensive rehospitalizations, frustrated patients, and poor public perception of an agency's quality of care. Home health aides play a vital role in this process and can impact the success of the transition. This in-service defines the aide's role in managing the complications associated with patient transition from the hospital to homecare. Aides will learn how they can help prevent rehospitalization by recognizing patient risk for falls, pressure ulcers, and other conditions. LESSON OBJECTIVES After completion of this program, the home health aide will be able to: Discuss the prevention of hospitalization/emergent care in homecare patients Discuss risk assessment for hospitalization, falls, and pressure ulcers Define and discuss the home health aide's role in preventing rehospitalizations and emergent care Understand the role and importance of risk assessment, patient education, disease and medication management, prevention of falls and pressure ulcers, and effective care transitions in reducing rehospitalizations Discuss the importance of effective communication techniques Contents of this lesson: A clearly written fact sheet A 10-question post-test to measure understanding of the subject matter An answer sheet with a place for the instructor's comments and signature An illustrative, homecare-specific case study Suggested supplemental learning activities An attendance log and certificate of completion

LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: Describe three methods for improving safety in the kitchen Explain three ways to make a home safe for patients, and State three things that attract burglars. OVERVIEW Home health aides care for patients with many different diseases and needs. However, one thing applies to all patients: the need for a safe and effective home environment that allows for proper recovery. While the nurse or physical therapist may conduct a home safety assessment for patients, it is often the home health aide who must observe and report changes in the environment that could affect patient safety. In addition, aides are often responsible for preparing meals for patients. Given that many home fires start in the kitchen while cooking, it is important that aides know the basics of fire prevention. This in-service reviews the home environment and ways to keep it safe for patients. It specifically outlines ways to prevent fires, falls, carbon monoxide poisoning, and burglaries. Finally, this in-service covers methods to improve home safety for visually impaired patients.
