LESSON OVERVIEW One of the Conditions of Participation (COP), §484.36, addresses home health aide services. A frequently cited deficiency is in the Standard: Assignment and duties of the home health aide, §484.36(c). The State Operations Manual indicates that surveyors should always make at least one home visit to observe a home health aide providing direct service. In addition, the surveyor may question the aide informally and will certainly review records of patients receiving home health aide services. The Standard: Coordination of patient services is another problematic area frequently cited during surveys. The standard in §484.14(g) mandates that all personnel providing services maintain liaison to ensure coordination of care and that the clinical record establishes that effective interchange and reporting of patient care does occur. A lack of documentation by home health aides frequently contributes to deficiency in this standard. The Interpretive Guidelines gives specific guidance to surveyors to look for documentation by home health aides in the clinical record describing significant information or changes in the patients' conditions and to whom these changes were reported. As agencies struggle to provide the highest quality, most cost-effective care for patients in the Prospective Payment System (PPS), the importance of observation and reporting by home health aides has taken on even greater importance. Patient problems need to be identified and acted upon as quickly as possible. It is likely that the home health aide spends more time with the patient than any other discipline and thus is more likely to note changes in the patient sooner. The subject of observing, recording, and reporting should be addressed frequently with them.

OVERVIEW The incidence of workplace injuries is very high among home health workers in general and home health aides in particular. The Bureau of Labor Statistics reports that one in ten home health aides has a work-related illness or injury every year. The resulting absences from work due to injury usually last from one to two weeks. There are shortages of home health aides throughout the United States and time lost from work by aides is troublesome and expensive for homecare agencies. In addition to the loss of worker productivity, the agency is also responsible for paying for medical expenses through its Workers' Compensation Plan. Overall, the injury rate for homecare aides exceeds the injury rate for similar workers in hospitals by 50 percent, and is 70 percent higher than the national average for private industry workers. It is not surprising that there are more frequent work-related injuries in home health for two primary reasons - there is far less control over the environment, and homecare workers travel from place to place either by car or public transportation throughout the work day. The home health aide may encounter a workplace environment that includes hostile animals, rotting steps, uneven floors, and clutter and then may have to brave inclement weather while juggling supplies and papers.The incidence of highway accidents is much higher among homecare workers than other types of health care workers. (Injuries that occur when commuting prior to the start of the workday or after it ends are not counted as work-related injuries.) The injury most common to home health aides is over-exertion, notably while lifting and transferring patients. Other common injuries include body actions such as over-reaching, twisting, or losing balance without falling. Many injuries occurring in the workplace can be prevented. The purpose of this in-service program is to provide information about injuries common to home health aides and to give measures that the aides can undertake to prevent or reduce them.

OVERVIEW Casts are formed materials usually made of plaster or fiberglass that are used to protect and support injured bones and soft tissue. Casts act to reduce pain, swelling, and muscle spasms around injured bones or joints. Sometimes casts are applied after patients have surgery. Orthopedic doctors treat fractures throughout the body except for the skull and face by applying casts. Extremity fractures are most common and usually occur in men younger than age 45 and then become more common in women over age 45 due to bone loss. Accidents and falls are the most common cause of fractures.Home health aides may be assigned to patients who have casts following an injury or illness. They need to know what is required when caring for a patient with a cast. The purpose of this in-service program is to emphasize the knowledge and skills needed when caring for patients with casts.
