Book excerpt: Using the Africa Centre for Population Health's demographic and HIV surveillance program, a population-based open cohort that has been conducting annual surveillance since 2000, this dissertation evaluates the effectiveness of ART in a real-world setting at multiple levels, including for individuals, within couples, and in households. First, this dissertation analyzes the effect of ART uptake in serodiscordant couples in the risk of HIV acquisition by the uninfected partner. Second, the effectiveness of immediate eligibility for ART on household-level HIV acquisition is assessed. Finally, the effect of immediate ART eligibility and uptake on survival in HIV-infected individuals is examined using two causal modeling approaches.

Book excerpt: Infectious diseases are the leading cause of death globally, particularly among children and young adults. The spread of new pathogens and the threat of antimicrobial resistance pose particular challenges in combating these diseases. Major Infectious Diseases identifies feasible, cost-effective packages of interventions and strategies across delivery platforms to prevent and treat HIV/AIDS, other sexually transmitted infections, tuberculosis, malaria, adult febrile illness, viral hepatitis, and neglected tropical diseases. The volume emphasizes the need to effectively address emerging antimicrobial resistance, strengthen health systems, and increase access to care. The attainable goals are to reduce incidence, develop innovative approaches, and optimize existing tools in resource-constrained settings.

Available in PDF, EPUB and Kindle. Book excerpt: In Biomedical Hegemony and Democracy in South Africa Ngambouk Vitalis Pemunta and Tabi Chama-James Tabenyang unpack the contentious South African government’s post-apartheid policy framework of the ‘‘return to tradition policy’’. The conjuncture between deep sociopolitical crises, witchcraft, the ravaging HIV/AIDS pandemic and the government’s initial reluctance to adopt antiretroviral therapy turned away desperate HIV/AIDS patients to traditional healers. Drawing on historical sources, policy documents and ethnographic interviews, Pemunta and Tabenyang convincingly demonstrate that despite biomedical hegemony, patients and members of their therapy-seeking group often shuttle between modern and traditional medicine, thereby making both systems of healthcare complementary rather than alternatives. They draw the attention of policy-makers to the need to be aware of ‘‘subaltern health narratives’’ in designing health policy.

Available in PDF, EPUB and Kindle. Book excerpt: In some parts of South Africa, more than one in three people are HIV positive. Love in the Time of AIDS explores transformations in notions of gender and intimacy to try to understand the roots of this virulent epidemic. By living in an informal settlement and collecting love letters, cell phone text messages, oral histories, and archival materials, Mark Hunter details the everyday social inequalities that have resulted in untimely deaths. Hunter shows how first apartheid and then chronic unemployment have become entangled with ideas about femininity, masculinity, love, and sex and have created an economy of exchange that perpetuates the transmission of HIV/AIDS. This sobering ethnography challenges conventional understandings of HIV/AIDS in South Africa.

Book excerpt: A follow-up to the Nelson Mandela Foundation's 2002 national household survey of HIV/AIDS prevalence in South Africa, this 2005 report seeks to provide further understanding of the HIV pandemic. Using data that tested for HIV incidence rather than just using mortality statistics, this study looks at which socio-demographic groups are most vulnerab≤ whether new policies have been successful in fighting the disease; what exactly is being done by key players, such as the government, churches, and other civil society organizations; and how the spread of HIV can be reduced in South Africa.

Book excerpt: While there is a general acknowledgement within the church itself that the Church was initially slow to respond to the magnitude of the problem of HIV and AIDS, during the recent past, as the effects of HIV and AIDS within the congregations and communities of the church have become progressively more evident, the Catholic Church has emerged as an increasingly central role-player in a range of initiatives to combat the pandemic. This publication describes the work of the 'Choose to Care' initiative and the way it has been successfully scaled-up through the diocesan and parish network so that programmes are formed by local needs but work with common guidelines and can draw on central support.

Book excerpt: This report presents three hypothetical case studies for how the AIDS epidemic in Africa could evolve over the next 20 years based on policy decisions taken today by African leaders and the rest of the world; and considers the factors likely to drive the future responses of African countries and the international community. The scenarios draw on the age-old tradition of story-telling, rather than using data projections, to explore the wider context of the AIDS epidemic, reflecting the complexity of the subject matter.

Available in PDF, EPUB and Kindle. Book excerpt: This text treats theoretical epidemiology as the discipline of how to study the occurrence of phenomena of interest in the health field. It delineates major principles of study design and data analysis in research into the frequency of occurrence of illness and related phenomena in human populations whether in the community or in clinical situations. Thus it is concerned with the principles of the dominant type of research in applied medical science and in health care administration. Moreover, the principles of research of this type in medicine are not peculiar to the study of illness, health and health care, nor to studies on humans. They apply equally to statistical research in psychology, behavioral science, technology, and other fields.

It provides approaches and examples of integration of interventions within health services.
