Book excerpt: The National Household HIV Prevalence and Risk Survey of South African Children forms part of the Nelson Mandela/HSRC Study of HIV/AIDS: South African National HIV Prevalence, Behavioural Risks and Mass Media Household Survey 2002. This report provides information on HIV prevalence, orphanhood, risk factors for HIV infection and knowledge of HIV/AIDS among South African children. A total of 3 988 children aged 2 to 18 years participated in the survey. Caregivers of 2 138 children 2 to 11 years of of age answered a questionnaire on the child's behalf. A total of 740 children 12 to 14 years of age directly answered a separate questionnaire. An additional 1 110 children and between 15 and 18 years of age answered a youth questionnaire. Of the 3 988 children, 3 294 (82.6 per cent) provided a saliva specimen for HIV testing. The results show HIV prevalence among children 2 to 18 years of age to be 5.4 per cent. Prevalence was nearly constant across age groups and did not vary significantly. There were insufficient numbers to compare prevalence across race groups. The prevalence was higher than expected. Further studies are necessary to verify this finding.

Further, because many pregnant women are not tested for HIV and therefore do not receive treatment, the number of children born with HIV is still unacceptably high. What can we do to eliminate this tragic and costly inheritance? In response to a congressional request, this book evaluates the extent to which state efforts have been effective in reducing the perinatal transmission of HIV. The committee recommends that testing HIV be a routine part of prenatal care, and that health care providers notify women that HIV testing is part of the usual array of prenatal tests and that they have an opportunity to refuse the HIV test. This approach could help both reduce the number of pediatric AIDS cases and improve treatment for mothers with AIDS. Reducing the Odds will be of special interest to federal, state, and local health policymakers, prenatal care providers, maternal and child health specialists, public health practitioners, and advocates for HIV/AIDS patients. Book excerpt: Studies point to the existence of a global HIV/AIDS emergency among young people. An estimated 6,000 youths a day become infected, an average of one new infection every 14 seconds. The most socially and economically disadvantaged young people appear to be especially at risk of infection, and young women in developing contexts are at the greatest risk. The rate of HIV infection among girls is rapidly outstripping the rate among boys. Girls already account for nearly 60 percent of the infections in sub-Saharan Africa, where the pandemic is most virulent. This paper adopts the hypothesis that this pandemic can be confronted already in early childhood. During the first eight years of life the foundations are set for the capacities, beliefs and attitudes that support individuals later in life. In early childhood, people can therefore more easily learn and integrate appropriate risk avoidance behaviours that may prove useful in the global war on HIV/AIDS. The earliest years may represent a window of opportunity for the successful implementation of HIV/AIDS reduction and prevention programmes. (Contains 190 endnotes.) Book excerpt: It is estimated that one third of the world's population is infected with Mycobacterium tuberculosis (the bacterium that causes tuberculosis (TB)), and that each year, about 9 million people develop TB, of whom about 2 million die. Of the 9 million annual TB cases, about 1 million (11%) occur in children (under 15 years of age). Of these childhood cases, 75% occur annually in 22 high-burden countries that together account for 80% of the world's estimated incident cases. In countries worldwide, the reported percentage of all TB cases occurring in children varies from 3% to more than 25%. The Stop TB Strategy, which builds on the DOTS strategy developed by the World Health Organization (WHO) and the International Union Against TB and Lung Disease, has a critical role in reducing the worldwide burden of disease and thus in protecting children from infection and disease. The management of children with TB should be in line with the Stop TB Strategy, taking into consideration the particular epidemiology and clinical presentation of TB in children. These consensus guidelines were produced to help the National Tuberculosis Programmes on the management of tuberculosis in children.
