Social factors relating to gender and gender inequality exacerbate the spread of HIV and sexually transmitted infections (STI) in China. A better understanding of the proximate social determinants of HIV related to gender will be crucial to effectively curbing HIV and other STIs in China. Aspects of China’s governance - including administrative procedures, the developing legal system, social institutions, and the public health infrastructure – are instrumental in shaping strategies and responses to HIV. International studies suggest that women who are more economically and socially vulnerable may also have a greater risk of HIV infection, yet few initiatives have focused on discrete areas where achievable and sustainable gender policy measures could be linked to the public health response. This study presents perspectives ranging from criminology to social psychology to better understand how gender perspectives can inform HIV policy in the context of China.

Given the high vulnerability of and limited research on transwomen in China, we described IPV, sexual risk behaviors, HIV and sexually transmitted infections (STIs) testing rates and results, and investigated the pathways that link IPV to HIV infection among this population. We conducted a cross-sectional survey and collected blood samples for HIV and syphilis testing among transwomen in Shanghai, China (N = 199). With logistic regression, we examined sexual risk behaviors and HIV/STI testing history among participants with and without IPV experience. Over half of the respondents reported IPV (57.3%) and the prevalence of unprotected sex range from 51.9% (with sex workers) to 87.8% (oral sex); 85.9% had ever tested for HIV and 49.3% for other STIs. Self-reported positivity results were: HIV (2.3%), HSV-2 (8.3%), gonorrhea (18.8%), and syphilis (17.8%). Lab-confirmed positivity was 5.0% for HIV and 6.5% for syphilis. Respondents with a history of IPV were significantly less likely to report HIV testing in the past 12 months (aOR 0.20, 95%CI 0.10-0.38). Transwomen self-reported a high prevalence of IPV, which was related to a lower probability of HIV testing. The prevalence of HIV and other STIs was lower than reported in previous studies of Chinese transwomen, while the HIV/STI testing rates were higher. Findings suggest transwomen in China are at risk for IPV and need enhanced HIV prevention services to promote HIV testing in an IPV setting.

Book excerpt: Background People living with HIV/AIDS (PLWHA) in rural Anhui, China face tremendous challenges, which may influence access to healthcare. This two-part study explored these issues. First, a quantitative study examined whether (a) two dimensions of HIV-related stigma (i.e., perceived stigma and internalized shame) or (b) three dimensions of social support (i.e., emotional support, tangible support, and affectionate support) are associated with access to care among HIV-positive men and women in rural Anhui, China. Second, a critical performance written and performed by the author applies ethnodrama strategies to explore HIV stigma and social support in Anhui Province. Methods The conceptual model integrates theoretical concepts of stigma and social support into Andersen's Behavioral Model of Health Services Use. This was a secondary analysis of baseline data from participants (N = 522 PLWHA) in the Together for Empowerment Activities (TEA) study, a randomized controlled intervention trial among HIV-affected families in rural Anhui, China. The data were collected between October 2011 and March 2013 via computer-assisted personal interviewing. The analyses involved mixed-effects, multiple linear regression with village-level random effects to control for variance clustering by the village. Conquergood and ethnodrama strategies informed the elements included in the performance: movement, original text, quotes from PLWHA, and spectacle such as photographs, set, and props. Results Higher levels of perceived stigma were associated with poorer access to care (estimated difference = - 0.08, p = 0.015). With respect to social support, only higher levels of affectionate support were associated with greater access to care (estimated difference = 0.43, p 0.0001). Tangible support was inversely associated with access to care (estimated difference = - 0.17, p = 0.003). Neither internalized shame nor emotional support was associated with access to care (p 0.05). Discussion This study underscores complexities between HIV-related stigma, social support and access to care in rural Anhui, China. Future research should explore whether adverse effects exist for managing HIV disease in this region. Performance represents a potentially powerful, non-traditional approach to discussing these issues and disseminating some findings; future work should explore whether this holds in Anhui Province.

Under the advancement of transportation, the open up of borders between cities and countries, and the process of globalisation, people move from their place of origin to other places for better living conditions and employment opportunities. Mobile people and migrants are identified as the high risk population of HIV infection. Poverty, discrimination, limited access to healthcare and social services, separation with supports and families, being alienated in resident communities, and gender inequalities are the unfavourable conditions which make the migrant population vulnerable to HIV infection. Existing literatures have investigated and examined the potential risk factors among the mobile population. Inconsistencies were found among research but high risk sexual behaviours and poor knowledge and attitude were observed and reviewed within the migrants. However, literatures which compared the people who migrated with other local people in the same population were not yet reviewed systemically. Therefore, this paper aimed to review the articles which compare the migrant group and the non-migrant group in Asian population to identify the association between migration and the risks of HIV infection. A literature search of five databases (PubMed, Medline, Cochrane, CNKI, Wanfang Med Online) was performed and nine articles were eventually selected for review. The migration status of literature was studied as explanatory variable and compared across studies. Outcome variables of interest were grouped into four categories as: demographic characteristics, sexual practices, awareness towards HIV/AIDS, and disease prevalence. To conclude, compared to people who did not migrate, migrants in Asia were more tend to be less educated, have multiple sex partners, engage in high risk sexual intercourse and commercial sex, but their overall condom usage were lower.
