Social factors relating to gender and gender inequality exacerbate the spread of HIV and sexually transmitted infections (STI) in China. A better understanding of the proximate social determinants of HIV related to gender will be crucial to effectively curbing HIV and other STIs in China. Aspects of China’s governance - including administrative procedures, the developing legal system, social institutions, and the public health infrastructure – are instrumental in shaping strategies and responses to HIV. International studies suggest that women who are more economically and socially vulnerable may also have a greater risk of HIV infection, yet few initiatives have focused on discrete areas where achievable and sustainable gender policy measures could be linked to the public health response. This study presents perspectives ranging from criminology to social psychology to better understand how gender perspectives can inform HIV policy in the context of China.

Book excerpt: In the epidemiology of HIV/AIDS, Female Sex Workers (FSW) play significant role in facilitating the heterosexual transmission of the virus. Therefore, developing preventive practices with this group related to HIV/AIDS and STI is crucial to control the further spread of the virus among the general population. This research investigates the behavioral risk factors among FSWs in central Bangladesh that influence their preventive practices. The consistent use of condoms by FSWs during commercial sex act is the main preventive practice variable of interest. The research found that preventive practices are associated with the most recent weekly income; knowledge and awareness level; and self-reported presence of STI symptoms. On the basis of the research it is recommended that more emphasis should be given to participatory, user-friendly approaches during education sessions to disseminate risk reduction messages. This research will also help community-based organizations and HIV/AIDS professionals to understand the patterns of risk behaviors among street-based FSWs and help in the design of intervention programs in the early stage of the epidemic.

Examining the relationship between elite masculine networking practices and vulnerability to HIV infection, Occupational Hazards includes the stories of countless government officials and businessmen who regularly visit commercial sex workers but resist HIV testing for fear of threatening their economic and political status. Book excerpt: Literature has documented the association between psychological wellbeing and HIV risks, but such studies are limited for female sex workers (FSWs) worldwide. Data on this area are particularly scarce in China, where millions of FSWs play an important role in the escalating HIV epidemic. This book has three aims: first, to investigate the present knowledge on behavioral studies of FSWs in China and identify the literature gaps; second, to examine the risk factors associated with FSWs' suicidal ideation and attempts; and third, to explore the relationship between depressive symptoms and FSWs' condom use behaviors. The first study is based on a systematic review of English literature regarding FSWs in China. The second and third study draw data from a community-based HIV/STD intervention study with FSWs in Guangxi, China. The data reveal that FSWs in China have poor psychological wellbeing with high rates of suicidal behaviors and depression. Their mental health problems are associated with HIV- related sexual risks. Health education and promotion efforts among FSWs in China need to take into consideration their level of psychological distress.

Early surveillance of FSWs found seroprevalence rates of 4.8% in Tijuana and 4.9% in Ciudad Juarez, Mexico (Patterson et al., 2006b), while the most recent study of FSWs from these two cities found a seroprevalence rate of 6% (Strathdee et al, in press). Prevention efforts that are culturally sensitive and appropriately tailored are needed to stem the rise of HIV in this high-risk population. The current study examined the applicability of the Social Ecological model for HIV prevention in a sample of female sex workers (FSWs) (N=472) participating in a safe sex counseling intervention in Tijuana, Mexico. Within the framework of the Social Ecological Model we explored differential predictors of condom use by comparing women who work in bar settings to those who work on the street hypothesizing that there would be differences across venue (bar vs street) with higher condom use in bar workers. Baseline models were established using path analysis in bar (N=233); (CFI= .890, RMSEA =.062) and street workers (N=203); (CFI =.928, RMSEA = .040). For bar workers individual level factors, (self-efficacy, condom use beliefs, alcohol use before sex) and institutional level factors (condom access) were directly and indirectly related to condom use, while in street workers interpersonal level factors (social support, previous abuse, client power) and drug use were the most important factors associated with condom use. Multiple group partial structural invariance was then performed comparing street workers (N=79) to bar workers (N=103) on the 5 paths that their baseline models shared, showing that these paths were invariant across groups ([delta][chi]2(5) =6.30). Multiple group comparisons found similarities between the bar and street workers, including the relationship between self-efficacy and condom use. Baseline model comparisons found differences including the type of substance used before sex, and the relationship between monetary incentives for unprotected sex and condom use self-efficacy, supporting the need for the tailoring of interventions for women based on location of their sex work.
