In The Dynamics of Russian Politics Russian and Western authors from the fields of political science, economics, ethnology, law, and journalism examine the reform's impact on key areas of Russian life, including big business, law enforcement, corruption, political party development, health care, local government, small business, and ethnic relations. Volume I presents the historical context and an overview of the reforms, then tracks how Putin's plans were implemented and resisted across each of the seven new federal okrugs, or megaregions, into which he divided Russia. Book excerpt: It has now been 25 years since the apocryphal report in the CDC Morbidity and Mortality Weekly Report dated June 5, 1981 entitled, “Pneumocystis Pneumonia - Los Angeles”, which announced what was to become HIV/AIDS. HIV has now affected virtually all countries that have looked for it and has had a devastating impact on the public health and medical care infrastructure around the world. HIV/AIDS has also disproportionately affected nations with the least capacity to confront it, especially the developing world nations in Sub-Saharan Africa, South and Southeast Asia, and the emerging republics of Eastern and Central Asia. The pandemic, unlike any other disease of our time, has had profound impacts on the practice of public health itself: bringing affected communities into decision making; demanding North-South partnerships and collaborations; and changing the basic conduct of clinical and prevention trials research. While much has been written in scholarly publications for medical, epidemiologic and disease control specialists, there is no comprehensive review of the public health impact and response to HIV/AIDS in the developing world. This edited volume seeks to systematically describe the emergence and form of the epidemics (epidemiology), the social, community and political response, and the various measures to confront and control the epidemic, with varying levels of success. Of particular importance are strategies that appear to have been useful in ameliorating the epidemic, while contrasting the situation in a neighboring country or region where contrasting prevention or care initiatives have had a deleterious outcome. Common to all responses has been the international multi-sectoral response represented by the Global Fund for HIV/AIDS, Malaria and Tuberculosis, the President's Emergency Plan for AIDS Relief, and the Gates Foundation, among others, to promote HIV pharmacologic therapy in resource-poor settings. The chapter authors will explore the political challenges in meeting HIV/AIDS prevention and care in concert with the public health realities in specific country and regional context.

Book excerpt: The book is the first attempt to investigate how and to what extent authoritarian (personalistic) regimes fail to provide fundamental goods and services. For two decades, Russian authorities spent much effort and money to improve health administration, but most success stories are borderline fake. The failure is by design; because personalistic regimes rely on personalized exchanges and bargains instead of impersonal rules and permanent organizations, all actors put self-interest ahead of patients’ needs. It is a severe problem because authoritarian principals proclaim social betterment as their central goal -- and many Russians take such claims at face value -- but incentivize their agents to imitate progress and tolerate slipshod performance. The benefits of this investigation are three-fold. First, the book provides an analytical framework of bad governance rooted in the rational institutionalist tradition and connected to competence-control theory. Second, it gives a general readership interested in how Russia works a sense of the key political players’ mindset and the regime-induced constraints under which elites operate. Third, although the book investigates health governance exclusively, its analytical framework is portable to other issue areas and could be applied to explain how and why Russia evolved into an ineffective, coercive, and predatory state under Putin’s leadership.

Regional studies on Central Asia to date have focused on states, politics, religion and inter-ethnic relations but not on the health system within the region. Soviet-style policies have also covered only other aspects relevant for the region. This book highlights the public health situation of the region with a focus on drug abuse, HIV/AIDS in the context of increased mobility, and drug trafficking routes which became even more porous after the break-up of the Soviet Union. Based on a qualitative study, the empirical data in the book was collected during long-term fieldwork conducted in Uzbekistan and Turkmenistan in 2010-2011 as well as shorter stays in Uzbekistan between 2012-2016. The analysis of the empirical material largely draws on the works of Foucault, particularly his concept of biopolitics when analyzing Soviet-style health management that is still practiced in the region. Applying the Foucauldian genealogical method, this study has been structured to trace the genealogy of epidemics to understand the historical path of drug abuse in the region as well as the discursive genealogy of drug politics and drug abuse. Applying the same genealogical method of Foucault, the formative and discursive trajectory of the institution of Uchyot was traced to contextualize the health governance methods that have historical legacy of Soviet-style governance and control of the total population. Drugs and Public Health in Post-Soviet Central Asia: Soviet-Style Health Management is a unique resource for academic specialists, practitioners/professionals, and advanced undergraduate and graduate students in public health, as well as a range of scholars and professionals in sociology, political science, anthropology, and anyone with an interest in the Central Asia region, drug addiction, or HIV.
