The Appalachian region, extending from Alabama in the South up to the Allegheny highlands of Pennsylvania, has historically been characterized by its largely rural populations, rich natural resources that have fueled industry in other parts of the country, and the strong and wild, undeveloped land. The rugged geography of the region allowed Native American societies, especially the Cherokee, to flourish. Early white settlers tended to favor a self-sufficient approach to farming, contrary to the land grabbing and plantation building going on elsewhere in the South. The growth of a market economy and competition from other agricultural areas of the country sparked an economic decline of the region's rural population at least as early as 1830. The Civil War and the sometimes hostile legislation of Reconstruction made life even more difficult for rural Appalachians. A History of Appalachia also examines pockets of urbanization in Appalachia. Chemical, textile, and other industries have encouraged the development of urban areas. At the same time, radio, television, and the internet provide residents direct links to cultures from all over the world. The author looks at the process of urbanization as it belies commonly held notions about the region's rural character.

In recent decades, developing countries have made substantial investments in infrastructure, achieving dramatic gains for households and producers by expanding their access to services such as safe water, sanitation, electric power, telecommunications, and transport. Even more infrastructure investment and expansion are needed in order to extend the reach of services - especially to people living in rural areas and to the poor. But as this report shows, the quantity of investment cannot be the exclusive focus of policy. Improving the quality of infrastructure service also is vital. Both quantity and quality improvements are essential to modernize and diversify production, help countries compete internationally, and accommodate rapid urbanization. The report identifies the basic cause of poor past performance as inadequate institutional incentives for improving the provision of infrastructure. To promote more efficient and responsive service delivery, incentives need to be changed through commercial management, competition, and user involvement. Several trends are helping to improve the performance of infrastructure. First, innovation in technology and in the regulatory management of markets makes more diversity possible in the supply of services. Second, an evaluation of the role of government is leading to a shift from direct government provision of services to increasing private sector provision and recent experience in many countries with public-private partnerships is highlighting new ways to increase efficiency and expand services. It applies the well-being monitor developed by Statistics Netherlands that has been endorsed by a significant part of the international, statistical community. It features The Netherlands as a case study, which is an especially interesting example; although it was one of the world’s richest countries around 1850, extreme poverty and inequality were significant problems of well-being at the time. Monitors of 1850, 1910, 1970 and 2015 depict the changes in three dimensions of well-being: the quality of life 'here and now', 'later' and 'elsewhere'. The analysis of two centuries shows the solutions to the extreme poverty problem and the appearance of new sustainability problems, especially in domestic and foreign ecological systems. The study also reveals the importance of natural capital: soil, air, water and subsoil resources, showing their relation with the social structure of the ‘here and now ́. Treatment and trade of natural resources also impacted on the quality of life ‘later’ and ‘elsewhere.’ Further, the book illustrates the role of natural capital by dividing the capital into three types of raw materials and concomitant material flows: bio-raw materials, mineral and fossil subsoil resources. Additionally, the analysis of the institutional context identifies the key roles of social groups in well-being development.
