Book excerpt: Suicide is a major public health concern in the United States (US), claiming over 36,000 lives each year and nearly 100 lives each day, and suicide among military and Veteran populations is of particular concern. Veterans returning from the Iraq and Afghanistan conflicts, referred to as Operation Enduring Freedom/Operation Iraqi Freedom (OEF/OIF) Veterans, may be particularly at risk, although the limited available data has shown mixed results. Several aspects of military experience may increase the risk of suicide, including mental health and substance abuse. Many risk factors specific to the OEF/OIF population have yet to be thoroughly evaluated and incorporated into clinical management. Ideally, suicide risk assessment tools need to account for the relationship among different risk factors and identify risk factors or combinations of risk factors that are particularly associated with suicidal self-directed violence. To be practically useful, such tools will be brief enough to be conducted in a primary care setting and will identify a threshold beyond which preventive action should be taken. Risk assessment tools should be able to discriminate those at high- and low-risk for suicidal self-directed violence. Likewise, studies of emerging risk factors need to evaluate the contribution of a new potential predictor of suicide and self-directed violence in the context of known risk factors in order to weigh the contribution of the new risk factor against those that are currently known. The objective of this report is to review recent evidence about risk factors and risk assessment tools within Veteran and military populations to provide evidence for clinical practice guideline development specific to these populations. The key questions addressed in this report were: Key Question #1. What assessment tools are effective for assessing risk of engaging in suicidal self-directed violence in Veteran and military populations? Key Question #2. In addition to the risk factors included by current assessment tools, what other risk factors predict suicidal self-directed violence in Veteran and military populations?

These challenges, plus new technologies, require our leaders to use creative measures to provide positive protection to our Soldiers and equipment. In April 1998, Field Manual (FM) 100-14 (FM 5-19) introduced to the Army the first doctrinal publication on risk management. It detailed the application of a step-by-step process to conserve combat power and resources. This milestone manual outlined a framework that leaders could use to make force protection a routine part of planning, preparing, and executing operational, training, and garrison missions. Before the outset of the global war on terrorism it became apparent that FM 100-14 would require updating to meet the needs of the future. Army assessments also indicated that the existing manual needed to be expanded to provide clear standards and guidance on how the risk management process was to be applied. This led to this current revision. During development of this revision the Army broadened its understanding of the risk management process to encompass all operations and activities, on and off duty. This holistic approach focuses on the composite risks from all sources rather than the traditional practice of separating accident from tactical hazards and associated risks. This revision has been refocused to clearly reflect the Army's new composite approach, and has been retitled Composite Risk Management (CRM). CRM represents a culture change for the Army. It departs from the past cookie cutter safety and risk management mentality through teaching Soldiers “how to think” rather than telling them “what to think.” This manual expands the context of the original FM by focusing on the application of composite risk management to the military decisionmaking process (MDMP) and the Army training management system. It further assigns the responsibilities for conducting risk management training during initial entry training and professional military education. It is a tool that works in conjunction with the Army's on-going initiative to firmly attach CRM to all Army processes. It is a milestone document for the standardization and institutionalization of the techniques, tools, and procedures that lead to sound decisionmaking and valid risk acceptance by leaders at all levels. This revision is a full rewrite of FM 100-14. It marks a break with the past by integrating the CRM process into Army operations. CRM is not a stand-alone process, a “paper work” drill, or an add-on feature. Rather, it is used as a fully-integrated element of detailed planning. It must be so integrated as to allow it to be executed intuitively in situations that require immediate action. CRM should be viewed as part of the military art interwoven throughout the Army's military decisionmaking and training management cycles.

It represents both initial findings of the Army Suicide Prevention Task Force and informs the future of Suicide Prevention within the Army. Suicide is a devastating event. What was once considered a private affair or family matter now threatens our Army's readiness. Equally alarming to the rising rate of suicide in the Army is an increasing number of Soldiers who engage in high risk behavior. Equivocal deaths,1 deaths by drug toxicity, accidental deaths, attempted suicides, and drug overdoses are reducing the ranks and negatively effecting the Army's ability to engage in contingency operations in Iraq and Afghanistan. These deaths further strain efforts to sustain institutional operations. No one could have foreseen the impact of nine years of war on our leaders and Soldiers. As a result of the protracted and intense operational tempo, the Army has lost its former situational awareness and understanding of good order and discipline within its ranks. This report's comprehensive review exposes gaps in how we see, identify, engage and mitigate high risk Soldiers. These gaps exist in our systems and processes due in part to Army Transformation and nearly a decade of war. Policy, process, structure and programs have not kept pace with the expanding needs of our strained Army. While leadership schools emphasize battlefield skills, leaders are not as adept at negotiating the art of leadership in a garrison environment as they were prior to OEF and OIF. Failure to execute policies designed to ensure good order and discipline in garrison sends a message of permissive complacency. Failure to refer a Soldier with a drug positive urinalysis to the Army Substance Abuse Program (ASAP); initiate administrative separation for positive urinalysis; administratively separate Soldiers committing multiple instances of misconduct; and report unlawful activity all contribute to a breakdown in good order and discipline. This, in turn, has led to an increasing population of high risk Soldiers whose transmittable behavior can erode Army values and unit readiness. Additionally, our units, Soldiers and Families are feeling the strain and stress of nine years of conflict. The cumulative effect of transitions borne of institutional requirements (professional military education, PCS moves, promotions) coupled with family expectations/obligations (marriage, child birth, aging parents) and compounded by deployments is, on one hand, building a resilient force while on the other, pushing some units, Soldiers and Families to the brink.

Book excerpt: Since the 1990s, Junior Reserve Officer Training Corps (JROTC) programs have experienced unprecedented expansion in American public schools. The program and its proliferation in poor, urban schools districts with large numbers of Latina/o and African American students is not without controversy. Public support is often based on the belief that the program provides much-needed discipline for "at risk" youth. Meanwhile, critics of JROTC argue that the program is a recruiting tool for the U.S. military and is yet another example of an increasingly punitive climate that disproportionately affect youth of color in American public schools. Citizen, Student, Soldier intervenes in these debates, providing critical ethnographic attention to understanding the motivations, aspirations, and experiences of students who participate in increasing numbers in JROTC programs. These students have complex reasons for their participation, reasons that challenge the reductive idea that they are either dangerous youths who need discipline or victims being exploited by a predatory program. Rather, their participation is informed by their marginal economic position in the local political economy, as well as their desire to be regarded as full citizens, both locally and nationally. Citizenship is one of the central concerns guiding the JROTC curriculum; this book explores ethnographically how students understand and enact different visions of citizenship and grounds these understandings in local and national political economic contexts. It also highlights the ideological, social and cultural conditions of Latina/o youth and their families who both participate in and are enmeshed in vigorous debates about citizenship, obligation, social opportunity, militarism and, ultimately, the American Dream.
