Book excerpt: In the last three decades of the 20th century, the World Health Organization and the World Bank put forth two health care reform initiatives utilizing vastly different approaches. However, there are no studies that have systematically examined how the Barbados and Trinidad and Tobago governments translated the comprehensive primary health care (CPHC) and the selective primary health care (SPHC) initiatives into their national health policies, plans, and initiatives. The purpose of this study was to identify the recommendations in the CPHC and SPHC initiatives that were incorporated into the national health policies, plans, and initiatives of Barbados and Trinidad & Tobago. Quantitative and qualitative content analysis were used to review available health policy data that contained records of health actions at the time these two health care reform initiatives were proposed. Quantitative content analysis of the documents indicated that the Barbados government instituted three CPHC initiatives, while the Trinidad & Tobago government only instituted two CPHC initiatives into their national health policies, plans, and initiatives, between 1978 and 2000. Moreover, the study found that politicians in Barbados and Trinidad & Tobago set the national health agenda, so as to provide adequate health care that was acceptable to its citizenry, while working with limited financial resources.
