Social Security at the Dawn of the 21st Century, the outcome of the most recent review, is intended to significantly extend the access of an international readership to accurate and up-to-date information and analyses on social security, which has without question developed during the twentieth century into one of the most important publicly financed and administered institutions in modern society. The chapters are grouped into two parts. Part one treats subjects related to policy trends and regional developments, with special emphasis on such important issues as redesigning social security programs, new management practices, and the informal care dilemma. It features major aspects of developments in Asia-Pacific and Latin America. Part two focuses on specific program areas, with special emphasis on problems and reforms in employment policy, pension systems, and public disability schemes. Information is also provided on new approaches to ensuring adequate access to health care and on policies in response to changes in family structures as well as an recent experience with social assistance programs. Dalmer D. Hoskins has held the post of Secretary General of the International Social Security Association (ISSA) since 1990. Before his election to this post, he held positions in the United States Social Security Administration and the Department of Health and Human Services. Donate Dobbernack is currently chief of communications and publications within the International Social Security Association (ISSA). Before assuming responsibilities in this area, she was chief of the technical activities program of the Association, dealing with international enquiries and studies on various aspects of social security and related fields. Christiane Kuptsch is a research officer with the International Social Security Association (ISSA) and the editor of the quarterly publication Trends in Social Security. She is a regular contributor to the Encyclopaedia Britannica on the issue of developments in social protection.

From the reforms that took place in Chile in 1981, most pension and health care systems in the region have seen reform, and been fully or partially privatized. Many other countries considering reform of their own systems have been influenced by the policies implemented in Latin America. Yet despite the importance and influence of these reforms, until now there has not been an integrated and comprehensive analysis of the changes and their effects. This book is the result of four years of painstaking work, data collection, field research and international collaboration, and so fills the vacuum in the literature with a systematic comparison of pension and healthcare reforms in the 20 Latin American countries. It identifies reform models, and elaborates taxonomies to facilitate their understanding and comparison. Some key features of the reforms to emerge are: labour force and population coverage, equity and solidarity, sufficiency and quality of benefits, state regulation, competition and degree of privatization, efficiency and administrative costs, social participation in management, financing sources and long-term sustainability. Effects of the reforms on social security principles are measured based on recent standardized statistics and other information. Goals or assumptions of the reforms are contrasted with actual outcomes, and the pros and cons of private versus private provision assessed. Two years after Ronald Reagan's income-tax simplification of 1986, Brazil adopted a similar reform even though it threatened to exacerbate income disparity and jeopardize state revenues. And Chile's pension privatization of the early 1980s has spread throughout Latin America and beyond even though many poor countries that have privatized their social security systems, including Bolivia and El Salvador, lack some of the preconditions necessary to do so successfully. In a major step beyond conventional rational-choice accounts of policy decision-making, this book demonstrates that bounded--not full--rationality drives the spread of innovations across countries. When seeking solutions to domestic problems, decision-makers often consider foreign models, sometimes promoted by development institutions like the World Bank. But, as Kurt Weyland argues, policymakers apply inferential shortcuts at the risk of distortions and biases. Through an in-depth analysis of pension and health reform in Bolivia, Brazil, Costa Rica, El Salvador, and Peru, Weyland demonstrates that decision-makers are captivated by neat, bold, cognitively available models. And rather than thoroughly assessing the costs and benefits of external models, they draw excessively firm conclusions from limited data and overextrapolate from spurts of success or failure. Indications of initial success can thus trigger an upsurge of policy diffusion.

Also reviewed are ways to eliminate unnecessary administrative agencies and to privatize public enterprises. The government's new health insurance program is examined.
