Book excerpt: There are significant health-related disparities in outcome among women in the United States with different types of gynecologic cancer. The authors hypothesize that a poor outcome among minorities with gynecologic cancer exists because of biological differences in tumors related to race and ethnicity; cultural, social, and psychological barriers to accessing care; less than optimal screening services and prevention strategies; and unequal provision of quality health care and tailored therapeutics. To find out more about these issues, the authors will conduct an analysis of the genomic and proteomic expression of gynecologic cancers to determine if there are molecular differences that partially account for the poor outcome among minority patients with gynecologic cancer. This analysis will be expanded in future years to include larger underserved cohorts. The comprehensive epidemiological data that it generates will facilitate more detailed genetic and epigenetic analysis. Epidemiological surveys will be used to identify demographic and behavioral differences that lead to poor outcomes. An evaluation also will be conducted on the use of psychosocial interventions to decrease morbidity among minorities. The authors also will develop vaccine strategies and specific antibody reagents for the detection of unique targets that are differentially expressed between African Americans and Caucasians with endometrial cancer. The incidence, severity, and overall burden of cancer in the United States vary by race, ethnicity, and other demographic features. This project will focus on identifying the reasons underlying poor outcomes among minority groups with gynecologic malignancy so that education, screening, prevention, and treatment algorithms can be tailored to high-risk populations in an effort to have the greatest impact on reducing morbidity and mortality among the underserved.

Book excerpt: Background- Black women are two times more likely to die from cervical cancer than White women, despite Black women having higher cervical screening rates than White women. One of the factors contributing to the racial disparity in cervical cancer mortality has been Black women's decreased likelihood to adhere to medical recommendations for follow-up after an abnormal Pap smear. Over the past decade, there has been attention placed on clinical provider implicit bias as a contributing factor to racial disparities that exist within healthcare. Though implicit bias has been recognized as a barrier to adherence to medical recommendations, there have been no studies to date that have explored patient perceived experiences of provider bias as a possible factor in adherence to follow-up recommendations after an abnormal Pap smear for Black women. Methods- A cross-sectional study design was used to determine a significant predictive relationship between perceived discrimination and intention to follow-up after an abnormal Pap smear among black women in comparison to other races. Seven predictors of discrimination along with intention to follow-up after an abnormal Pap smear were measured via survey. Results- 250 subjects were surveyed to capture their experiences of patient-perceived discrimination and their intention to follow-up after an abnormal Pap smear result. Black women did not indicate a significant relationship between patient perceived discrimination and intention to follow-up after an abnormal Pap smear when compared to other races. Conclusion- Despite results, Black women still continue to experience a disparity in cervical cancer outcomes, with literature supporting a decrease in follow-up after an abnormal Pap smear. Additional studies should be conducted to explore the role that patient perceived discrimination plays in patient follow-up after an abnormal Pap smear result.

Failure to significantly improve morbidity and mortality during the past several decades is likely due to several factors, including the lack of research being performed by specific disease subtype, lack of definitive knowledge of the cell of origin and disease progression, and incomplete understanding of genetic and non-genetic risk factors. Ovarian Cancers examines the state of the science in ovarian cancer research, identifies key gaps in the evidence base and the challenges to addressing those gaps, considers opportunities for advancing ovarian cancer research, and examines avenues for translation and dissemination of new findings and communication of new information to patients and others. This study makes recommendations for public- and private-sector efforts that could facilitate progress in reducing the incidence of morbidity and mortality from ovarian cancers.
