The principal conclusion arrived at was that World Bank and IMF staff should be engaging in collaboration earlier in the reform process. To guide the collaboration, six foundations were identified. These include: develop a medium-term fiscal framework; foster national ownership by making reforms politically feasible; focus and streamline conditionality; agree on sequencing and timing of reforms; and strengthen data collection. These principals will be tested for effectiveness in several focus countries.

For Sub-Saharan Africa, especially the epidemiological situation is unclear, since comprehensive systems of mortality and health statistics are often absent.The economic analysis of health naturally places a special focus on the interrelation between health and economic well-being: the overall disease burden decreases when a country grows richer, and the share of communicable diseases decreases in the process of economic development, whereas the share of non-communicable diseases increases. In those parts of Sub-Saharan Africa that are mainly dominated by traditional subsistence farming, however, it is difficult to examine questions of income and health for simple fundamental reasons. A vital prerequisite for an empirical investigation is the thorough and accurate measurement of income. Yet, both the measurement of the burden of disease and the measurement of income are research tasks that are far from being fulfilled for Sub-Saharan Africa. A further issue that is related with economic well-being and health is education. For poor rural regions predominated by traditional subsistence farming it is far from clear whether investments in human capital are worthwhile.The present study addesses this research gap by producing empirical evidence on the measurement of the burden of disease, the structure of income, and returns to education in rural West Africa. Concretely it deals with the collection and analysis of mortality, morbidity, and socio-economic data in the Nouna Health District in the North-West of Burkina Faso. The study was accepted as a doctoral thesis at the University of Heidelberg. Earlier versions of some of its chapters have been published as working papers or in international journals.

The data used are from the first year of the Ghana Living Standards Survey. While primarily descriptive, the paper contains information with clear policy implications. Several findings stand out. First, rural residents are, on average, clearly worse off than urban residents. The poorest group are residents of the rural savannah while the wealthiest are those who live in the capital, Accra. Second, education of the household head is strongly positively correlated with household welfare. Third, households where the head is self-employed, especially in agriculture, are generally found at the lower end of the distribution of welfare, while those headed by a wage earner, either in the private or public sector, are better off. Fourth, unemployment among household heads is not correlated with household welfare. Finally, although the poorer groups are less likely to seek medical help when they are ill, malnutrition among young children in these groups is not much higher than that among the better off groups.

The ESF was established explicitly as a temporary financial institution outside of the normal bureaucratic structure of the government. While its primary emphasis was to provide temporary employment opportunities, it differed from more typical government works projects in being demand-driven. This paper is concerned with measuring the effect of the ESF program on employment and income of workers in the ESF projects. The analysis is based on the results of a survey administered to workers in ESF infrastructure projects and to the population at large by the Instituto Nacional de Estadistica (INE) of Bolivia. The paper identifies the characteristics of the workers in the ESF infrastructure projects and compares them with the population in general, and workers in the construction sector in particular. The authors perform a counterfactual simulation, asking what would have been the position of the workers in the absence of the ESF program. Based on the simulation, they infer what the employment and income effect of the ESF program has been on its beneficiaries.

Typical budgets for farmers are presented, and their position in the income and consumption distribution descibed. Section 4 concerns itself with the analysis of policy change, and speculates on what might be the effects of moving towards the policy of allowing domestic prices to be determined by world prices. Finally, section 5 summarizes the major policy conclusions and outlines areas where further research is likely to be useful in improving policy advice.
