Book excerpt: This text was developed as a book aimed at surgeons and allied health professionals that provides an introduction to the unmet needs , epidemiological, socioeconomic and even political factors that frame Global Surgery. Following upon an understanding of these issues, the text is a practical guide that enables the reader on several levels: to work cross culturally , build relationships and negotiate the logistical challenges of bringing surgical care to low resource settings; to develop an approach to the management of various clinical conditions that would be unfamiliar to most “western” surgeons. Global Surgery is a recently coined term that encompasses many potential meanings. Most would agree that it focuses on the growing recognition of the crisis of access to quality surgical care in low resource settings. Such scenarios exist on every continent. Increasingly surgeons, allied health professionals (NGO), Public Health / Health Policy professionals as well as governmental and non-governmental organizations are engaging in this field. Many surgeons have an interest in Global Health and a desire to become involved but feel ill equipped to do so and unsure where to start. Global Surgery: The Essentials serves as a ready resource to equip surgeons to manage clinical scenarios that lie beyond the scope of their training or current practice but that they would reasonably be expected to encounter in the field.

Book excerpt: This seventh book in the series of Success in Academic Surgery look to sustain the field and facilitate the next generation of leaders in Academic Global Surgery. It brings together a catalogue of current knowledge, needs, and pathways to a career in the field. Academic Global Surgery involves educational, research and clinical collaborations between academic humanitarian surgeons in high-income countries (HIC), their low and middle-income country (LMIC) partners and their respective academic institutions. The goal of these collaborations is improving understanding of surgical disease, and increasing access to and capacity for surgical care in resource-poor regions. In the last few years, the rapid exchange of ideas through social media and other technologies has combined with an increasing appreciation of worldwide health disparities to put the issue of global health at the forefront of our consciousness. Although traditionally neglected within public health initiatives, surgical disease is now recognized as a major contributor to death and disability worldwide, while surgical therapy in resource-poor areas is increasingly being shown to be cost-effective. In response to this growing recognition, what began as mission trips and short-term clinical volunteerism in the developing world has evolved into a burgeoning new field with a broader scope. While the tremendous recent interest from medical students and residents in Globa l Surgery has stimulated an exponential growth of interest in this field, current surgical literature has highlighted the need for further development and delineation of this new discipline within academic surgery.

Book excerpt: This text captures the global standards of bariatric surgery practice at a time of change, excitement, and lots of controversy. The text sheds the light on best practices globally by providing a reliable reference to guide the practicing physician anywhere in the world, and from whatever specialty (surgeon, gastroenterologist or endoscopist) to navigate through the many current options of therapy in this rapidly changing field. The text provides high definition illustrations of these techniques to go with the didactic chapters written by the thought leaders in the field. In addition to the technical part, an important part of the book focuses on quality and outcome measures. The rapid growth and innovations impose the need for strict guidelines and quality control. Thought leaders who created the concept of “Centers of Excellence” shed light on outcome measures and different ways to monitor quality. This will appeal to administrators and different ancillary service providers. The medical section plays a major role as combination therapy seems to be the future. An entire section is dedicated to medical weight management with discussions of the dietary and psychological component of care. The text also provides a dedicated discussion of the metabolic aspect of bariatric surgery, cosmetic surgery and issues of training future surgeons. Thease features differentiate the book from others that only discuss the surgical component, and will broaden the level of interest to all who are involved in the management of this complex disease.

Book excerpt: Few books have been published to assist local and visiting clinicians in addressing opportunities to expand surgical care in low- and middle-income countries. With contributions from international experts and surgical leaders, this book serves as a valuable resource for local and visiting faculty, trainees, and students in their endeavors to deliver surgical services in existing facilities as well as to develop new infrastructures for specialized surgical care. This book presents an anthology of important lessons gleaned from the experiences of participants in selected academic global surgery centers and existing partnerships all over the world. Contributing authors summarize methods to build partnerships and expand capacity with a focus not only on how to, but also on what worked, what didn’t work, and what could be done differently. They discuss specific examples ranging from implementing colon cancer screening to building a transplant program. In addition, several chapters describe approaches to diagnosis and management of specific surgical pathologies in limited resource environments as a complement to standard surgical texts in general surgery, thoracic surgery, neurosurgery, urology, obstetrics and gynecology, orthopedics, and surgery for contractures and burns. Practical and user-friendly, Global Surgery encapsulates the increasing enthusiasm of young faculty, trainees and medical students to improve access to surgical care in the world’s low- and middle-income countries.

This book was released on 2003-07-21 with total page 515 pages. Available in PDF, EPUB and Kindle. Book excerpt: Many patients who present to district (first-referral) level hospitals require surgical treatment for trauma, obstetric, abdominal or orthopedic emergencies. Often surgery cannot be safely postponed to allow their transfer to a secondary or tertiary-level hospital but many district hospitals in developing countries have no specialist surgical teams and are staffed by medical, nursing, and paramedical personnel who perform a wide range of surgical procedures often with inadequate training. The quality of surgical and acute care is often further constrained by poor facilities, inadequate low-technology apparatus and limited supplies of drugs, materials, and other essentials. The mission of the team responsible for Clinical Procedures in the World Health Organization Department of Essential Health Technologies (EHT) is to promote the quality of clinical care through the identification, promotion and standardization of appropriate procedures, equipment and materials, particularly at district hospital level. WHO/BCT has identified education and training as a particular priority, especially for non-specialist practitioners who practice surgery and anesthesia. It has therefore developed Surgical Care at the District Hospital as a practical resource for individual practitioners and for use in undergraduate and postgraduate programs in-service training and continuing medical education programs. The manual is a successor of three earlier publications that are widely used throughout the world and that remain important reference texts: General Surgery at the District Hospital (WHO 1988), Surgery at the District Hospital: Obstetrics Gynecology Orthopedics and Traumatology (WHO 1991), Anesthesia at the District Hospital (WHO 1988; second edition 2000). This new manual draws together material from these three publications into a single volume which includes new and updated material, as well as material from Managing Complications in Pregnancy and Childbirth: A Guide for Midwives and Doctors (WHO 2000).

Available in PDF, EPUB and Kindle. Book excerpt: The second, fully updated edition of this book applies and contextualizes up-to-date information on pediatric surgery for low and middle-income countries (LMICs). The book is organized in general anatomic and thematic sections within pediatric surgery, such as urology, oncology, orthopedics and gastroenterology and includes chapters addressing the unique challenges and approaches for pediatric surgery in low-resource settings. Each chapter has dual authorship LMIC author providing context-specific insights and authors from high-income countries (HICs) contributing experience from well-resourced settings. Written in a reader-friendly format, this book has a uniform structure in each chapter, with introduction, demographics, etiology, pathophysiology, clinical presentations, investigations, management, outcome, prevention, ethics, evidence-based surgery and references. This comprehensive volume fills the gap between up-to-date pediatric surgical scholarship and knowledge developed and applied in HICs, and the practical needs of practitioners in low-resource settings. This is an indispensable guide for postgraduate surgical trainees in Africa and other LMICs as well as general surgeons practicing in Africa and other LMICs, who need to care surgically for children.
