Book excerpt: En nous penchant sur les approches pragmatique et psycho-sociale en orthophonie, nous avons pu observer les propositions thérapeutiques proposées et les moyens utilisés, centrés sur le patient aphasique souvent très à distance de son AVC et du début de la prise en charge. Aussi, attachée à la qualité de la communication, nous avons pu constater un manque en terme d'entretien et de compensation des interactions et des initiatives du patient aphasique sévère avec son entourage (institutionnel et familial), dès son arrivée en institution. La mise en place précoce d'un moyen pour aider le patient aphasique à développer des stratégies d'adaptation et d'ajustement avec ses interlocuteurs pour faire face à son handicap communicationnel semble essentielle. Nous développons à cette fin un outil qui tente de compenser précocement les déficits de communication orale du patient aphasique sévère en institution. Un outil basé sur l'adaptation de l'environnement du patient aphasique en fonction de ses habiletés de communication résiduelles et en parallèle des rééducations menées. Ainsi, dans ce cadre, nous prenons en compte davantage la personne aphasique que son aphasie ; et ses interlocuteurs proches, notamment les soignants du quotidien : nous ne considérons pas le sujet aphasique isolément mais bien en relation étroite et constante avec son environnement et ses interlocuteurs pour une meilleure qualité de ses interactions. Avec cette étude, nous souhaitons montrer qu'agir ensemble sur l'environnement du patient aphasique sévère et répondre à ses besoins de communication de façon précoce permettrait d'atténuer les situations de handicap et notamment d'éviter la diminution des initiatives de communication et des interactions. Notre étude s'est déroulée pendant deux mois au CMPR de Lay-Saint-Christophe auprès de six patients aphasiques sévères. Deux outils ont été créés en lien avec deux situations incontournables de la communication : les soins du matin ; le choixdes repas.
