Available in PDF, EPUB and Kindle. Book excerpt: This book describes various aspects of the basic physiological processes critical to tissue perfusion and cellular oxygenation, including the roles of the circulatory system, respiratory system, blood flow distribution and microcirculation. In the context of monitoring critically ill patients in the early hours of circulatory shock, it is essential to recognize changes in traditional parameters such as mean arterial pressure and cardiac output, and to assess the need for active intervention. However, even if global macrocirculatory variables are restored, abnormalities in tissue oxygenation may persist. Tissue hypoperfusion is connected to the development of organ failure and, if it goes unrecognized, may worsen the prognosis. As a result, there is a growing interest in methods for monitoring regional perfusion in peripheral tissues to predict or diagnose ongoing hypoperfusion. In this work, eminent experts from a range of disciplines convey a working knowledge of how regional monitoring in shock can complement the conventional global parameters of oxygen transport, and demonstrate that real-time bedside assessment of tissue oxygenation is readily achievable using noninvasive monitoring techniques. Accordingly, the book offers a valuable, easy-to-use guide for the entire ICU team and other clinicians.

In this rapidly changing area of medicine this text aims to bring the latest understanding of the pathophysiology together with a practical guide to diagnosis and management using a thoroughly evidence based approach. This will appeal to a wide audience of health care professionals who will treat patients with acute heart failure, including doctors, medical students, nurses, and other professionals in the Emergency Department, General Internal Medicine, Anaesthetics, Cardiology (Medicine and Surgery) and Critical Care settings. It may be used as an evidence based guide for the junior practitioner or as an aide-memoire for the more senior. Always keeping the underlying pathophysiology at the forefront of the discussion, the reader is encouraged to understand the aetiology of the acute situation and how to direct management in order to correct the abnormal physiology. Each chapter is heavily referenced allowing the reader to easily refer to the original experimental studies and pursue topics in more detail if required. This text is a valuable addition to any practitioner who treats patients with acute heart failure and wants a deeper understanding of the condition.

Available in PDF, EPUB and Kindle. Book excerpt: The first five volumes of the Correspondence of Jeremy Bentham contain over 1,300 letters written both to and from Bentham over a 50-year period, beginning in 1752 (aged three) with his earliest surviving letter to his grandmother, and ending in 1797 with correspondence concerning his attempts to set up a national scheme for the provision of poor relief. Against the background of the debates on the American Revolution of 1776 and the French Revolution of 1789, to which he made significant contributions, Bentham worked first on producing a complete penal code, which involved him in detailed explorations of fundamental legal ideas, and then on his panopticon prison scheme. Despite developing a host of original and ground-breaking ideas, contained in a mass of manuscripts, he published little during these years, and remained, at the close of this period, a relatively obscure individual. Nevertheless, these volumes reveal how the foundations were laid for the remarkable rise of Benthamite utilitarianism in the early nineteenth century. Bentham’s early life is marked by his extraordinary precociousness, but also family tragedy: by the age of 10 he had lost five infant siblings and his mother. The letters in this volume document his difficult relationship with his father and his increasing attachment to his surviving younger brother Samuel, his education, his interest in chemistry and botany, and his committing himself to a life of philosophy and legal reform.

Book excerpt: This is a book about liberal democratic values and their implications for the design of political institutions. Its distinctive feature is the use of some simple mathematical techniques (known as social choice theory) to clarify and defend a rather complex utilitarian conception of the liberal democratic 'way of life' based on John Stuart Mill's work. More specifically, the text focuses on three well-known 'social choice paradoxes' which are commonly held to destroy any possibility of an ideal harmony among liberal democratic values; and draws upon suggestions implicit in Mill's writings to develop an ethically appealing liberal democratic social choice framework in which the aforementioned paradoxes no longer cause concern. The revised framework is a rather complex version of utilitarianism and should be of special interest to welfare economists, social choice theorists, democratic political theorists and philosophers concerned with utilitarian ethics.

Available in PDF, EPUB and Kindle. Book excerpt: This book offers a detailed utilitarian analysis of the ethical issues involved in war. Utilitarianism and the Ethics of War addresses the two basic ethical questions posed by war: when, if ever, are we morally justified in waging war, and if recourse to arms is warranted, how are we permitted to fight the wars we wage? In addition, it deals with the challenge that realism and relativism raise for the ethical discussion of war, and with the duties of military personnel and the moral challenges they can face. In tackling these matters, the book covers a wide range of topics—from pacifism to armed humanitarian intervention, from the right of national defense to pre-emptive or preventive war, from civilian immunity to the tenets of just war theory and the moral underpinnings of the rules of war. But, what is distinctive about this book is that it provides a consistent and thorough-going utilitarian or consequentialist treatment of the fundamental normative issues that war occasions. Although it goes against the tide of recent work in the field, a utilitarian approach to the ethics of war illuminates old questions in new ways by showing how a concern for well-being and the consequences of our actions and policies shape the moral constraints to which states and other actors must adhere. This book will be of much interest to students of the ethics of war, just war theory, moral philosophy, war and conflict studies and IR.

Book excerpt: The exploits of the twentieth, twenty-first, twenty-second, and twenty-third (Service) Battalions of the Northumberland Fusiliers during the Great War—“Harder Than Hammers.” Although called the Tyneside Scottish, very few of the men who made up this Brigade were of Scottish descent. Many came from local villages or were from the Northumberland pits. They saw action at the Battle of the Somme and after it were allowed to put tartan behind their cap badges because of their bravery. “This remarkable product of much research includes lists of those who received gallantry awards and of officers and other ranks. It is an informative book which will be of great help to anyone researching the Tyneside Scottish during WWI and which will also act as a keepsake for those who have a particular interest in the regiments.” —Northumberland & Durham Family History Society

Book excerpt: Reconstruction of the climate variability of the past 500 years is a topic of great scientific interest not only in global terms, but also at regional and local levels. This period is interesting on account of the increasing influence of anthropogenic forcing and its overlap with natural factors. The Polish Climate in the European Context: An Historical Overview summarises the results of research into climate variability based on a combination of instrumental, documentary, dendrochronological and borehole data from Poland. The first part of the book provides a Central European perspective of research in these fields, which forms the general background for a presentation of the state of the art of climatic change studies in Poland during the past 500 years (Part 2). This is followed by a selection of papers dealing mainly with different aspects of climate variability in Poland and Central Europe (Part 3). "This book is a valuable tool integrating Polish, Central and Eastern European climate research into the global context. It is, as such, a must for climate researchers worldwide." (Gaston Demarée, Royal Meteorological Institute of Belgium) "This volume marks a significant step forward in our understanding of European climatic history." (Christian Pfister, University of Bern)
