Book excerpt: The emphasis on cancer management in the past was based primarily on control rates from multidisciplinary input in management. There has always been a recognition that one would like to achieve the best result with the least complication, but never has there been any major emphasis on evidence-based outcome studies, nor on functional preservation and quality of life. The authors of this book have dealt very effectively with the various tumor types in head and neck cancer with the experts in the ? eld of management. The contents range from epidemiology and treatment outcome, treatment techniques with the potential impact on the quality of life such as dysphagia, to the various options relative to high technology radiation therapy programs for mana- ment. The potential for improving form and function through surgical care as an integrated part of the program is dealt with very effectively as well as the potentials for chemotherapy and the use of targeted agents have on quality of life issues. The volume also addresses toxicity, quality of life, and techniques for prevention of adverse effects, as well as the potentials for rehabilitation and supportive care. The authors have clearly done an extraordinarily good job in addressing the mul- plicity of problems that impact upon the functional preservation and quality of life in head and neck radiation therapy. Book excerpt: Palliative care has become increasingly important across the spectrum of healthcare, and with it, the need for education and training of a broad range of medical practitioners not previously associated with this field of care. Part of the Integrating Palliative Care series, this volume on surgical palliative care guides readers through the core palliative skills and knowledge needed to deliver high value care for patients with life-limiting, critical, and terminal illness under surgical care. Chapters explore the historical, philosophical, and spiritual principles of surgical palliative care, and follow the progression of the seriously ill surgical patient's journey from the pre-operative encounter, to the invasive procedure, to the post-operative setting, and on to survivorship. An overview of the future of surgical palliative care education and research rounds out the text. Surgical Palliative Care is an ideal resource for surgeons, surgical nurses, intensivists, and other practitioners who wish to learn more about integrating palliative care into the surgical field.

Book excerpt: Since the late 1960s, the survival rate in children and adolescents diagnosed with cancer has steadily improved, with a corresponding decline in the cancer-specific death rate. Although the improvements in survival are encouraging, they have come at the cost of acute, chronic, and late adverse effects precipitated by the toxicities associated with the individual or combined use of different types of treatment (e.g., surgery, radiation, chemotherapy). In some cases, the impairments resulting from cancer and its treatment are severe enough to qualify a child for U.S. Social Security Administration disability benefits. At the request of Social Security Administration, Childhood Cancer and Functional Impacts Across the Care Continuum provides current information and findings and conclusions regarding the diagnosis, treatment, and prognosis of selected childhood cancers, including different types of malignant solid tumors, and the effect of those cancers on childrenâ (TM)s health and functional capacity, including the relative levels of functional limitation typically associated with the cancers and their treatment. This report also provides a summary of selected treatments currently being studied in clinical trials and identifies any limitations on the availability of these treatments, such as whether treatments are available only in certain geographic areas.

Book excerpt: Cancers of the larynx, while survival outcomes increase, result in massive treatment damage from radiation and surgery. Patients often lose ability to speak and to eat. Preserving the larynx is a fine balance of cancer eradication, life extension, and quality of life. This issueof Otolaryngologic Clinics led by Dr Babak Sadoughi should be of interest to Otolaryngologists, Oncologists, Radiologists, and Speech Therapists. The issue approach reaches all the most important aspects of diagnosing and treating the patient with laryngeal cancer with a focus on preserving the voice in early laryngeal cancer and preserving and restoring function in advanced laryngeal cancer. Topics include: Functional Anatomy and Oncological Barriers of the Larynx; Evaluation of the Dysphonic Patient; Role of Advanced Laryngeal Imaging in Glottic Cancer; Laryngeal Function after Radiation Therapy; Management of Dysphonia after Radiation Therapy; Contemporary Surgical Management of Early Glottic Cancer; Voice Prognosis after Transoral Laser Microsurgery of the Larynx; Voice Rehabilitation after Transoral Laser Microsurgery of the Larynx; Quality of Life after Conservation Surgery for Laryngeal Cancer; Salvage Conservation Surgery of the Larynx; Airway Preservation in Ablative Laryngeal Surgery; Voice Restoration after Total Laryngectomy. A special article for Residents, written by a Resident, emphasizes essential "take home messages" for laryngeal function preservation diagnosis and treatment.

This book was released on 2004 with total page 354 pages. Available in PDF, EPUB and Kindle. Book excerpt: A working group of sixteen experts from seven countries re-evaluated the evidence of the carcinogenicity of betel-quid and areca-nut chewing and some areca-nut related nitrosamines. Betel-quid and areca-nut chewing are widely practised in many parts of Asia and in Asian-migrant communities elsewhere in the world. There are hundreds of millions of users worldwide. They evaluated betel quid with tobacco as carcinogenic to humans (Group 1) on the basis of sufficient evidence of an increased risk of cancer of the oral cavity, pharynx and oesophagus. The working group reviewed epidemiological studies of human cancer, mainly studies from India, Pakistan and Taiwan (China). Studies on betel quid with tobacco and areca nut with tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. The working group also evaluated betel quid without tobacco as carcinogenic to humans (Group 1), on the basis of sufficient evidence of an increased risk of oral cancer. Studies on betel quid without tobacco and areca nut without tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. Areca nut, a common ingredient of betel quid and many different chewing preparations, including those available commercially, has been observed to cause oral submucous fibrosis

Available in PDF, EPUB and Kindle. Book excerpt: Oral problems are common in cancer patients. They can be a part of the disease itself; they can develop as a result of treatment; they can accompany other symptoms and present as a comorbidity; and they can even first present after the original disease itself has been cured. Oral symptoms can be complex to treat, both preventing administration of potentially life-saving treatment, and also directly causing additional life-threatening complications. However, many symptoms can be treated, and lead to tremendous relief of suffering for the patient. Oral complications of cancer and its management is the first book to focus on this unique set of symptoms, and provides an international, multi-professional reference with up to date, evidence-based, clinically relevant advice. It covers topics such as oral anatomy and physiology, oral assessment and oral hygiene, as well as more common oral complications of cancer and its treatment. Surgical, radiotherapy and chemotherapy side effects are detailed in all of the chapters whilst specific problems are discussed in detail in separate chapters. Other chapters focus on oral infections, and major oral symptoms experienced by patients with cancer. In addition there are chapters addressing the needs of specific groups of patient (e.g. paediatric patients, patients with advanced cancer), and the health economic impact of oral complications. This book should become the complete reference source for the wide multi-disciplinary team involved in treating the cancer patient with oral problems; this includes oncologists (both medical and clinical), oncology nurses, oral medicine specialists, palliative care physicians and nurses, special care dentists, special care dental nurses/hygienists, and therapy radiographers.

Book excerpt: PET/CT is an integral part of the evaluation of patients who have head and neck cancer. In this issue, the state of the art in PET/CT imaging is discussed. The issue starts with an overview of FDG-PET/CT, PET and MRI for normal anatomy, including pitfalls and artifacts. This topic is followed by a review of FDG-PET/CT for initial and subsequent therapy evaluation; progressing to PET and MRI. Other articles discuss SUV as a prognostic factor in head and neck squamous cell carcinoma, PET in head and neck cancer radiotherapy treatment planning, PET in decision making for neck dissection after radiation treatment, and newer methods for improving yield from FDG-PET imaging for accurate staging, determining tumor biology, and assessing prognosis. The issue focuses on some of the most cutting-edge applications, such as new tracers PET in head and neck squamous cell carcinoma (including FLT and hypoxia imaging). PET scans in thyroid cancer is also discussed.
