Book excerpt: Introduction : selon l'Organisation Mondiale de la Santé (OMS), cinq des dix pathologies les plus préoccupantes du XXIe siècle sont des maladies psychiques : schizophrénies, troubles bipolaires, addictions, dépressions et troubles obsessionnels compulsifs. Depuis les 20 dernières années on observe des avancées concernant la question du handicap psychique et du travail. Quels sont les freins persistants et les leviers à l'intégration des Personnes en Situation de Handicap Psychique (PSHP) en entreprise en milieu ordinaire. Sujets et Méthodes : une revue narrative de la littérature a été menée de juin 2020 à décembre 2021. Les recherches bibliographiques se sont concentrées sur les publications de quatre bases de données et sur deux ouvrages traitant du handicap psychique et du travail. Résultats : l'emploi est un facteur de rétablissement pour les personnes en situation de handicap psychique. L'emploi accompagné est un dispositif permettant d'accompagner la PSHP et l'entreprise dans l'accès et le maintien en emploi. Les études montrent qu'il augmente les chances de se maintenir en emploi par rapport aux autres méthodes. Il se développe en France mais reste balbutiant et réservé aux situations les plus complexes alors que la remise précoce au travail est préconisée. Conclusion : le système de l'entreprise et le système médico administratif qui nous gouverne génèrent des obstacles à la fluidité du parcours d'accompagnement des personnes en situation de handicap psychique. Un accompagnement des entreprises, l'accès à la formation et à la sensibilisation du personnel de ressources humaines et des encadrants pour lutter contre la stigmatisation semblent indispensables pour donner l'opportunité aux personnes en situation de handicap psychique de mieux s'intégrer dans le monde de l'entreprise. La formation et le travail en réseau des professionnels de santé semblent également nécessaires. Le médecin du travail est un acteur clé du maintien en emploi. Il doit, avec les services de santé au travail se saisir de cette problématique et être inclus dans les réflexions autour de l'accompagnement de cette population.
